Transportation Accident Operational Error Equipment Failure Natural Phenomenon Dumping<br />

List Materials and Volumes:<br />

Approximately 15 gallons <strong>of</strong> produced water, 3 gal Diesel fuel, 8 quarts motor oil and 2 gal antifreeze.<br />

Brief Incident Description:<br />

From notification (e-mail) and discussion with clean-up crew:<br />

Tanker truck overturned on shoulder <strong>of</strong> road. Some fluids leaked to ground, but most contained in tanker.<br />

Damages:<br />

Injuries #: Deaths #: Property Damage >$50,000 Evacuations:<br />

Corrective Action Taken:<br />

Remaining fluids removed. Top 2” <strong>of</strong> affected soil scraped and removed. Clean up crew (Birdsong) seeding<br />

ground and applying straw during investigation.<br />

Responding Agencies/Parties:<br />

ADEQ Water Division personnel (self) investigated on March 28, 20<strong>13</strong>. Clean up personnel from Birdsong,<br />

Inc. out <strong>of</strong> Heber Springs also present.<br />

Additional Comments:<br />

All fluids and affected soil removed. Birdsong personnel were seeding and applying straw during my<br />

investigation. I saw no evidence that any fluids left the affected area.<br />
