Approximately 15 gallons of produced water, 3 gal Diesel fuel, 8 quarts motor oil and 2 gal antifreeze.
Brief Incident Description:
From notification (e-mail) and discussion with clean-up crew:
Tanker truck overturned on shoulder of road. Some fluids leaked to ground, but most contained in tanker.

Injuries #: Deaths #: Property Damage >$50,000 Evacuations:
Corrective Action Taken:
Remaining fluids removed. Top 2” of affected soil scraped and removed. Clean up crew (Birdsong) seeding
ground and applying straw during investigation.
Responding Agencies/Parties:
ADEQ Water Division personnel (self) investigated on March 28, 2013. Clean up personnel from Birdsong,
Inc. out of Heber Springs also present.
Additional Comments:
All fluids and affected soil removed. Birdsong personnel were seeding and applying straw during my
investigation. I saw no evidence that any fluids left the affected area.
