In advance of the Indianapolis Motor speedway event, please see attached the registration,
rental and insurance paperwork required to permit your team to operate team Golf Carts,
Tuggers, 4 wheelers and or rental Golf Carts on the premises of the Indianapolis Motor
Speedway.
We recommend that in a timely fashion, you:
Read the information and details explained in the attached paperwork.
Follow the instructions noted in the paperwork.
Complete the required paperwork process well ahead of the event.

Prior to issuance of any golf cart vouchers, please complete the following by Monday, July 8, 2013:

Return to IMS the enclosed CONSENT & LIABILITY RELEASE and IDEMNITY AGREEMENT
for OPERATION OF GOLF CARTS signed by an authorized individual/officer of the
company; and
Provide an original certificate of insurance naming IMS Group as an additional insured. The
certificate of insurance must confirm a minimum of $1,000,000 per occurrence, combined
single limit, of general liability insurance coverage and shall provide that coverage is primary
as to any other coverage available to this insured. Facsimiles and e-mails are not acceptable;
and
Return the enclosed “2013 Golf Cart Registration” form completed for both privately owned golf
cart(s) and any Salopek Golf Car & Equipment Co., Inc. rental cart(s).
***PLEASE NOTE***
After approval of all three (3) forms noted above, one voucher per golf cart will be issued.
Vouchers are issued for both privately owned carts and those rented through Salopek Golf Car &
Equipment Co., Inc. Please note: Teams are to receive no more than two carts. Electronic
vouchers will be emailed to the named person on their registration form once all required
documents are received and approved by the IMS Credentials office. The IMS Credentials office
opens on July 15, 2013 should you choose to pick up the voucher instead.
Voucher redemption procedures will be included with the golf cart vouchers. The voucher will be
exchanged for a golf cart sticker credential.
Rental golf carts must be ordered exclusively through Salopek; unauthorized vendor carts are not
permitted.

OPERATION RULES FOR
ALL TERRAIN VEHICLES,
GOLF AND UTILITY CARTS
*** DUE TO THE SPECIAL NATURE OF THESE VEHICLES, THE
FOLLOWING RULES MUST BE OBSERVED ***
Pedestrians have the right of way – be courteous at all times;
Minimum age requirement to operate golf carts, all terrain vehicles and utility carts is 18 years;
Operating these vehicles at night requires proper illumination;
Modifying, disconnecting or otherwise interfering with the speed control, or any other equipment
on the vehicle is strictly prohibited;
Never operate the vehicle while impaired by alcohol, drugs, medication or injury; or transport
passengers whose judgment may be impaired for any reason but not limited to the consumption of
alcohol;
Apply the parking brake and remove the key when not in use;
Recommend unattended carts be secured properly with a cable and lock;
Obey traffic signage, directions, signals and instructions by IMS Safety Patrol and/or law
enforcement officials;
Do not operate carts outside the premises of the IMS property gates; i.e. city sidewalks and
streets;
Do not operate any of these vehicles on the golf course or golf course paths;
Refueling carts is not the responsibility of the IMS;
Obey golf cart seating capacities;
IMS is not responsible for lost or stolen carts.
*** AVOID THE FOLLOWING ***
Transporting passengers whose judgment may be impaired for any reason including but not
limited to the consumption of alcohol;
Transporting passengers not seated on seat(s) provided by the manufacturer;
Passengers sitting in a backwards position on the front cowling or standing while in operation;
Heavy loads that may shift while turning;
Using reverse - use extreme caution if you must use reverse;
Operating any vehicle without proper authorization, at excessive speed, recklessly or without due
consideration to pedestrians;
Parking under grandstands without authorization or in pedestrian traffic areas.
REPORT ALL INCIDENTS/CRASHES INVOLVING THE VEHICLE TO IMS GUEST SERVICES AND/OR LAW
ENFORCEMENT OFFICIALS. FAILURE TO DO SO WILL RESULT IN SUSPENSION OF OPERATING PRIVILEGES.
REPORT CRASHES/INCIDENTS REQUIRING MEDICAL ATTENTION TO LAW

2013 BRICKYARD 400 MILE RACE® GOLF CAR RENTERS
In order to save our customers additional expenses and added bookkeeping we are happy to
announce that for 2013, Salopek Golf Car & Equipment Co., Inc. will not require a prepaid
security deposit for golf car rental. Instead, we are asking all golf car renters to use a VISA or
MASTERCARD as payment. This will allow us to have the card number on file to cover any
extraordinary charges in the event of damaged, lost, or stolen vehicles. We hope this will save
our old friends/customers time and money by eliminating the wait for a refund of money tied up
in a prepaid security deposit. For golf car rental reservations or additional information, please

Rental Period: Wednesday, July 24, 2013 to Sunday, July 28, 2013
Golf Car Return Day: No later than 12:00 noon on Monday, July 29, 2013 (in the event of a
rain delay, the return day will be the day after the race is completed).
Any golf cars returned after 12:00 noon on Monday, July 29,
2013, will incur an additional $200.00 per golf car late fee. Any
golf car keys not returned will incur an additional $5.00 per golf
car key fee.
Payment/COI/Confirmation: Must be received in our office by Tuesday, July 2, 2013.
Orders received after July 2, 2013 will be honored, subject to
availability and will have a $75.00 per golf car up-charge fee.

*No checks will be accepted after July 2, 2013.
*If rental is paid by check, a VISA or MASTERCARD must be

LEASE AGREEMENT
Salopek Golf Car & Equipment Co., Inc.
This contract is to begin on July 24, 2013 by and between Salopek Golf Car & Equipment Co., Inc. (SGC) herein
after referred to as the “Lessor” and ___________________________________, herein after referred to as the
“Lessee.”
WITNESSETH:
That for valuable consideration received and hereby acknowledged by each, the Lessor does grant the Lessee
exclusive renting rights as hereinafter stipulated and mutually agreed that each of the respective parties to this
contract will fulfill the several obligations imposed on each and that both accept the full meaning and intent of this
contract, waiving recourse to any specific states rights laws which might allow either party to evade their respective
responsibilities.
It is further understood that no verbal understanding not incorporated in this document is to be conceded as binding
upon either but it is agreed that should it be found expedient and proper by both to alter, add, or omit any portion of
this contract during its existence, that it may be altered or changed only by mutual written agreement, signed by the
proper authorized officer of each respective party and that such changes do not invalidate the balance of the
contract.
DURATION OF THE CONTRACT:
The usage period for the units is from approximately July 24, 2013 (or when the vehicles are received by the
“Lessee”), until approximately July 29, 2013 (once all vehicles are returned to the “Lessor” and inspected for
unusual wear and tear or damages).
COMMISSION:
SEE ATTACHED SCHEDULE A – LESSEE UNDERSTANDS THAT ALL RENTAL RESERVATIONS MUST BE
RECEIVED BY THE LESSOR NO LATER THAN JULY 2, 2013 OR AN ADDITIONAL $75.00 PER GOLF CAR
WILL BE ADDED TO THE TOTAL RENTAL PRICE.
RENTAL CONDITIONS:
1. Lessee agrees to install: SEE PAGE 2 OF AGREEMENT.
2. The Lessee has until July 2, 2013 to cancel (or partially cancel) an order, by written notification. Once this
date passes, the Lessee will be responsible for the full rental amount of each vehicle set aside for the
Lessee.
3. Lessor agrees to furnish a mechanic on request to handle any major maintenance which may be required
on cars.
4. Title to said vehicles shall at all times be and remain the sole exclusive property of the Lessor. Said
vehicles may not be removed from the premises of the Indianapolis Motor Speedway.
5. The Lessee assumes responsibility for the safe housing indoors or under suitable cover for the golf cars
during the night hours. Ignition keys are to be removed from vehicles when not is use and at night time.
6. The Lessee shall maintain insurance for the vehicles, as required by the Lessor and Indianapolis Motor
Speedway, LLC.
7. Any acts of vandalism to the rental vehicle shall be reported in writing to the Lessor and Indianapolis Motor
Speedway, LLC upon discovery for documentation in accordance with standard incident reporting
procedures, along with any names and /or evidence that would lead to the apprehension of the vandals or
that would aid in filing an insurance claim. Property damage, fire, theft, and injuries resulting from the use
of the vehicles shall be reported in the same manner. Lessee takes full responsibility for any and all for the
above and will responsible for payment in full for lost or damaged golf cars.
8. Lessor shall have the right to withdraw any cars at anytime for maintenance or any abuses beyond normal
wear and tear.
9. Lessor shall have the right to assign this contract.
10. Normal wear and tear on golf cars will be expected, but abuse to cars will be the responsibility of the
Lessee.
11. ADVERTISING: Lessee shall not, without the prior consent of Salopek Golf Car & Equipment Co., Inc.,
use, nor shall it permit others to use, the equipment for the purpose of displaying any advertisements or
publicity materials (including company logo).

12. Lessee will be responsible for paying the full replacement value for lost or stolen vehicle(s).
13. In case of a rain delay additional rental fee will be charged.

MANDATORY CERTIFICATE OF INSURANCE REQUIREMENTS
Certificate of insurance that must accompany the commercial use of golf
cars/motorized vehicles permit request are required to:
Specifically state that use of a golf car or other motorized vehicle is covered
by insurance.
Include the name of the company that will be operating the equipment on the
Indianapolis Motor Speedway property as the named insured on the
certificate.
Include comprehensive general liability insurance with limits not less than
one million dollars ($1,000,000) per occurrence.
Include the appropriate additional insured language (as described below) in
the “Description of Operations” section of the certificate:
“IMS Group is an additional insured with coverage that
is primary as to any other coverage available to this
insured. Use of golf cars and other motorized vehicles
is covered.”
Under “Certificate Holder” please list the following as additional insured:

You will need to submit one Certificate of Insurance to Salopek with the
required information above and one to Indianapolis Motor Speedway with
their specific requirements.
