Please complete and return this form to operate golf carts(s) on the premises of the Indianapolis
Motor Speedway. Prior to issuance of any golf cart vouchers, all forms must be completed, sent to
the Indianapolis Motor Speedway and approved by July 8. Please refer to the instruction sheet for
list of all forms. Electronic vouchers will be emailed to the named person on this registration form
once all required documents are received and approved by the IMS Credentials office. The IMS
Credentials office opens on Monday, July 15, 2013 should you choose to pick up the voucher instead.
