REGISTRATION POLICIES AND PROCEDURES Finalizing Registration Mail Registration Forms to: Registration Department, P.O. Box 27540, Philadelphia, PA 19118. Final payment of the total fee is due five (5) weeks before camper’s camp session begins (partial payments will be accepted between registration and the final payment date). Include Camper Confirmation Form (the form you received by mail) with final payment. Be sure to mark your check or money order with your camper’s name and the name of her camp session(s). There will be a $30 fee for all returned checks. Final Payment Final payment of the total fee is due five (5) weeks before your camper's camp session begins. Partial payment will be accepted between registration and the final payment due date. Clearly identify your check or money order with your camper's name and session. Credit card payments can be made by calling the camp registrar. In order to hold your camper's space in her session and program, please observe payment deadline. (We sometimes have a waiting list!) Registrations received five (5) weeks or less before the start of the session require full payment with registration for placement. If you call, please follow up with a written note to confirm your change request. A new confirmation letter will be sent if there is space in the other session you request. Your cooperation in notifying us in time to place another child is appreciated. In-Season Registration If your camper has a WONDERFUL time at camp, and wants another camp program, we may have a place for her later in the summer. Call the registrar to see if there is availability in another session. Cancellations and Refund Policy If illness prevents a girl from attending camp, the fee will be refunded if the Girl Scout Office (or Camp Office on Sunday) is notified on or before the day the girl is to arrive. All refund requests must be written and accompanied by a doctor's note. No refunds will be made to any participants who do not attend for any other reason or who leave before the session ends. Camper’s Accident and Sickness Insurance The Girl Scout Council provides supplemental medical expense coverage for both accident and sickness for all participants in resident camp. This is included in your camp fee. In most cases, the insurance forms will be handled by the camp and the doctor or hospital. If you have any questions regarding insurance during camp, call the Camp Director. After camp is over, please check with the Human Resources department at the Shelly Ridge office. 6
PHYSICAL EXAMINATIONS Please note that every child must have a physical examination within 24 months and the medicine information section signed by a licensed doctor dated within 12 months of her camp stay. The Resident Camp Health & Safety Record for Girls is attached to the back of this packet and is also available online at www.gsep.org. Click on Camps, then click on the applications, forms and registration info button. Be sure to report everything that may affect your camper at camp--recent illness, injuries, allergies, etc. This health form must be carried to camp with your camper, and must be given to bus monitors or camp staff before she is allowed to board the bus or attend camp. Campers are not allowed at camp without a properly filled out and signed health form. Medications By law, all medications (prescription, over-the-counter, homeopathic, topical ointments AND vitamins) must be collected by the camp Health Supervisor. Only the medications written on the health form can be dispensed. Please make sure you note all medications on the health form. Keep medication in original containers, which identifies it as your camper’s medication, the instructions for dispensing, etc. This is a state law. Any medication not in the original container will NOT be dispensed. Campers may not keep any kind of medication in their possession. PA Department of Health requires that this section must be updated and signed by a Physician within 12 months in order for our Camp Heath Care Providers to administer medication to school age children. The Health Center has common use medications such as Tylenol, Advil, Mylanta and Calamine lotion in stock. Please do not send to camp. Also please note that if your camper is not taking normally prescribed medication while she is at camp, you MUST still inform the staff at the Health Center of her condition. The staff at the Health Center will ensure that your camper receives her medication. Emergency medications, such as those required for allergies, are kept close at hand by the Unit Staff. There is a doctor available for consultation. If your child becomes ill, or has an injury, we will call you. 7
