The <strong>in</strong>tention of the WFI is that its<br />

implementation should be a positive<br />

<strong>in</strong>dividualized program that is<br />

non-punitive. All component results are<br />

measured aga<strong>in</strong>st the <strong>in</strong>dividual’s<br />

previous exam<strong>in</strong>ations and assessments<br />

and not aga<strong>in</strong>st any standard or norm.<br />

However, medical practice standards may<br />

be used when results <strong>in</strong>dicate that<br />

lifesav<strong>in</strong>g <strong>in</strong>tervention is required.<br />

Confidentiality of medical <strong>in</strong>formation is<br />

the most critical aspect of the WFI. The<br />

unauthorized release of personal details<br />

which may be recorded as part of a medical<br />

evaluation causes legal, ethical, and<br />

personal problems for the employee,<br />

employer and exam<strong>in</strong><strong>in</strong>g physician. All<br />

<strong>in</strong>formation obta<strong>in</strong>ed from medical and<br />

physical evaluations should be considered<br />

confidential, and the employer will only<br />

have access to <strong>in</strong>formation regard<strong>in</strong>g fitness<br />

for duty, necessary work restrictions, and if<br />

needed, appropriate accommodations.<br />

Also, all medical <strong>in</strong>formation must be<br />

ma<strong>in</strong>ta<strong>in</strong>ed <strong>in</strong> separate files from all other<br />

personnel <strong>in</strong>formation.<br />

The WFI is a comprehensive program<br />

that addresses the needs of the total<br />

<strong>in</strong>dividual <strong>in</strong> a program to build and<br />

ma<strong>in</strong>ta<strong>in</strong> fit <strong>fire</strong> fighters and EMS<br />

personnel. Fitness—physical, mental, and<br />

emotional— requires an effective wellness<br />

program available to recruits, <strong>in</strong>cumbents,<br />

and retirees. Components of the WFI<br />

<strong>in</strong>clude medical evaluation, fitness, <strong>in</strong>jury/<br />

fitness/medical rehabilitation, behavioral<br />

health, and data collection.<br />

The WFI is designed to improve the<br />

quality of life for all <strong>fire</strong> fighters while<br />

simultaneously seek<strong>in</strong>g to prove the value<br />

of <strong>in</strong>vest<strong>in</strong>g wellness resources over time.<br />

The Cost Justification Chapter of the WFI<br />

Third Edition outl<strong>in</strong>es the significant cost<br />

sav<strong>in</strong>gs <strong>in</strong> lost work time, workers<br />

compensation, and disability when<br />

implement<strong>in</strong>g a comprehensive<br />

wellness-fitness program.<br />

HIGH BLOOD PRESSURE<br />

High blood pressure or hypertension is a<br />

recognized risk factor for heart <strong>disease</strong>.<br />

Untreated, elevated blood pressure can<br />

also damage the bra<strong>in</strong>, kidneys and eyes.<br />

While family history and genetics may<br />

contribute to the <strong>in</strong>cidence of high blood<br />

pressure, the impact of lifestyle choices<br />

cannot be underemphasized. Adoption of<br />

healthy lifestyles is critical for the<br />

prevention of hypertension, essential for<br />

the management of those with high blood<br />

pressure, and has been shown to decrease<br />

cardiovascular risk. v<br />

Major lifestyle changes that have been<br />

shown to lower blood pressure <strong>in</strong>clude<br />

weight loss <strong>in</strong> those who are overweight<br />

or obese; adoption of the Dietary<br />

Approaches to Stop Hypertension<br />

(DASH) eat<strong>in</strong>g plan; regular physical<br />

activity; moderation of alcohol and<br />

sodium consumption. vi Healthy behaviors<br />

also enhance the efficacy of<br />

antihypertensive medications used to<br />

treat those with high blood pressures. For<br />

example, adoption of the DASH eat<strong>in</strong>g<br />

plan has effects similar to a s<strong>in</strong>gle drug<br />

therapy, while comb<strong>in</strong>ation of several<br />

lifestyle modifications can achieve even<br />

better results. vii Adjunctive to behavior<br />

change, pharmacologic treatments have<br />

been shown to reduce the complications<br />

of hypertension. Cl<strong>in</strong>ical trials have<br />

demonstrated that goal blood pressure<br />

levels can be achieved <strong>in</strong> most patients<br />

who are hypertensive, with many proven<br />

medications to choose from.<br />

STEPS YOU CAN TAKE TODAY<br />

• Aerobic and muscle strengthen<strong>in</strong>g<br />

exercises that <strong>in</strong>cludes moderate<br />

physical exertion for at least 30<br />

m<strong>in</strong>utes on most days of the week.<br />

• Ma<strong>in</strong>ta<strong>in</strong> a healthy weight.<br />

• Eat a healthy diet that limits sodium<br />
