1. Show the sample plaques to your parents. Collect orders and payment. Bring everything back to the photo area before your game begins.
2. Individual/Team Plaques cost $30.00. Team Plaques cost $25.
3. With 4 fully paid orders, the coach & school can each get 1 team plaque for $10. The parent helper can get a memory mate plaque for $10.
4. You must have 4 fully paid orders for us print any. Please pick up your plaques after your game is finished.
5. Pay by Cash, Visa/MC/DISC or Check to “EVENT PRO” (If using credit card, include phone # of cardholder).
6. Show to parents now. Bring form and money back to photo table before your game. Parents must neatly spell player name and uniform#.
