Growing children require a regular healthmaintenance<br />
program that includes immunizations,<br />
dental checkups, physicals, hearing exams,<br />
and eye exams.<br />
To address the Health Services component for the 2013-14 school<br />
year, each <strong>HPSE</strong> school must work toward achievement of the following:<br />
Elementary Schools<br />
To increase compliance rates, communicate with your area JCPS<br />
Health Services nurse practitioner (485-3387) for the following:<br />
• Physical exams for initial entry and sixth grade<br />
• Immunizations<br />
• Seasonal Flu Clinic (if available)<br />
• Dental exams for 5- or 6-year-olds<br />
Contact your assigned JCPS nurse practitioner to discuss available<br />
resources for the following:<br />
• Vision exams for 3-, 4-, 5-, or 6-year-olds upon initial entry<br />
Middle and High Schools<br />
To ensure that student immunizations, physical exams, and other<br />
school health requirements are met, communicate with your area JCPS<br />
Health Services nurse practitioner (485-3387) for the following:<br />
• Physical exams for initial entry and sixth grade<br />
• Immunization Certificate for required vaccinations<br />
• Seasonal Flu Clinic (if available)<br />
Conduct hand-washing lessons in each classroom to prevent the<br />
spread of communicable diseases. Specify how often and when. Select<br />
one:<br />
• School nurse (if applicable)<br />
• <strong>HPSE</strong> <strong>Program</strong> specialist: 485-3387<br />
• Addressed in science, health, or chemistry classes<br />
