When your doctor says you are ready to go home, it is only the beginning of preparing you for discharge. You will receive printed discharge instructions, including: self-care procedures, activity restrictions, tobacco use/cessation information and additional resources. Your Family or Friends Can Help Having someone help you at your discharge can be helpful. They can: • Listen to discharge instructions given by the doctor or nurse. • Provide clean clothes and shoes for going home. • Pack belongings into the vehicle for the trip home. • Fill prescriptions at your local pharmacy the same day. • Bring the vehicle to the circular drive by the patient discharge door. The Next Step in Your Care Taking care of yourself after discharge contributes significantly to your full recovery. Make sure you understand and follow the instructions given by your doctor and other providers. Get Involved North Memorial Foundation Your generosity and commitment are important to us. SafeJourney – A Lifeline for Surviving Domestic Abuse North Memorial SafeJourney is a hospital-based domestic abuse advocacy program that provides support and information to individuals and families who do not feel safe in a relationship. SafeJourney services are free and confidential and are available to community members as well as patients of North Memorial. SafeJourney ensures that victims have access to more than treatment for medical injuries and a telephone referral. Medical staff members screen patients for violence issues and document any injuries resulting from domestic violence. 9
Volunteer advocates provide a lifeline for victims and families that need safe housing, orders for protection, financial assistance, documentation of injuries and individual safety planning.
