When completed, the health care directive becomes a legal document. It will go on file with your patient information and will be read by your health care providers. It stays in effect until you change or cancel it. North Memorial will accept your current health care directive, even if prepared in another state, as long as it meets the requirements of Minnesota state law. Patient Services Health Information Management – Medical Records To make arrangements for access to your medical record or to request paper copies, please contact Health Information Management at (763) 581-4952. Routine requests can be made between 8 a.m. and 3:30 p.m. A small fee may be charged for production of paper records. CaringBridge.org North Memorial provides access to CaringBridge free of charge so that you may stay connected with others by way of the internet. Making a private website is easy. Simply follow the steps on the CaringBridge website which is CaringBridge.org. A CaringBridge site makes it easy to update family and friends about your condition. Guests can leave messages of support in the guestbook. Patient Representative Your nurse will be able to address the majority of your care and comfort requests to help make your hospital stay as pleasant as possible. If you are having difficulty getting the care you need, you can also ask to speak to the charge nurse on your unit or to our hospital’s Patient Representative. Patient Representative office hours are Monday through Friday from 8:00 a.m. to 4:00 p.m. The phone number is: (763) 520-7150. At other times, call the operator (“0”) or (763) 520-5200, and ask to speak to the Administrative Manager. 3
Interpreter Services Deaf or Foreign Language North Memorial will provide American Sign Language or foreign language interpreters for patients and patient’s companions, free of charge. MARTTI – (My Accessible Real-Time Trusted Interpreter) MARTTI is a video relay machine that offers quick access to interpreter services. The interpreter, on a live monitor, is able to see into the room to assist the health care team, patient and family with their discussion of important medical issues. Services for Deaf and Hard of Hearing Patients and Companions DVDs in American Sign Language (ASL) with English captioning Our iNvision Patient TV system offers ASL videos with English captioning on the following topics: Health Care Directives, Notice of Privacy Practices, Patient Information and Services, Patients Rights and Responsibilities, and Services for Deaf and Hard of Hearing Patients. Not all patient rooms are equipped with iNvision at this time. If your room is not equipped with iNvision, please contact your nurse for alternative viewing options. Sound Amplification (Pocket Talkers) Pocket Talkers are available free of charge for hard of hearing patients or companions to use while at North Memorial. If you would like one for long term use after discharge, they are available for purchase at our Health & Care Shop located on the first floor of the medical center, near the NorthWay Deli & Grill. Door Knockers These devices attach to your door and send out a burst of light when someone knocks on the door. This way, you know when someone would like to enter your room. Let your nurse know if you want a door knocker installed. TTY/TDD All hospital rooms at North Memorial have analog phone lines that allow patients and their companions to use the TTY/TDD relay service. 4
