that you have read and agree to the Participation Agreement
and the Photo/Video Permission Release as outlined
below. Any objections need to be in writing and
submitted with your form.
Participation Agreement
The participant acknowledges and accepts the risk of physical
injury associated with participation in the activity described.
Except for gross negligence on the part of the sponsor, that
participant accepts personal financial responsibility for any bodily
or personal injury sustained during the activity. Further, the
participant promises to hold harmless the sponsoring organization
and its representatives for any injury related to the activity.
If a dispute over this agreement or any claim for damages
arises, the participant agrees to resolve the matter through a
mutually acceptable arbitration process.
Photo/Video Permission and Release
I hereby authorize Riverland Hills Baptist Church (RHBC) and/or
their agents to use photograph(s) I supply on the RHBC website.
I also understand that photographs and/or videos may be taken
during local/on campus/off campus events and that these may
also be used. I specifically understand that RHBC shall hereby
retain any and all rights in respect to the photograph(s) and/or
video/film project(s) and/or website/internet project(s), including
but not limited to, the rights to reproduce, copy/edit, exhibit,

Registration Fee: $45.00 per player
Deadline: March 8, 2010 37+

League limited to the first 64 men to

25+ - Thursdays
Game Times: 6:30, 7:30, & 8:30*
* Limited number of 9:30 games
Evaluation Nights (see dates/times above)
Players must attend at least one evaluation.
New players must attend both evaluations of
appropriate division.

All players must attend.
Players will be drafted as individuals not as a
group or team.
