Access to Care (FACS) Prevention
Strategy
This item was moved forward in the agenda as Cllr Roger Giess
could only attend the meeting briefly.
The council fund voluntary organisations for a range of services.
The Council is moving from grant aiding to commissioning
services.
Lambeth Learning Disability Partnership Board 3

LLDPB 6 November 2003
There is currently a process which aims to establish which
voluntary organisations provide which particular service.
There are specific services, which the Council would like to
prioritise as areas of improvement.
All voluntary sector providers are being asked to submit bids. This
will be a legal contract of guaranteed funding for three years.
Voluntary sector providers should submit expressions of interest
by Friday 14 th November 2003; further proposals will be needed by
February. This will enable the council to establish which
organisations provide which specific service.
Consultants have devised Fair Access to Care (FACS) Prevention
Strategy in draft form. The prevention strategy is for those people
that do not meet the FACS eligibility criteria to receive a service
from Social Services.
Voluntary Sector Providers feed into the FACS prevention strategy
as people who do not meet the eligibility criteria could be provided
prevent ional services from voluntary sector providers.
Voluntary Sector providers have been sent copies of the 1 st
prevention strategy draft and should feed back to Fran Pitcher.
It was agreed that the subject had been brought to the board far to
late and Roger Giess made the point that stronger links between
all Partnership Boards within the Council could highlight such
areas in the future.
not Social Services.
Action Points
The Board went through the action points from the last meeting.
Particular note was given to:
Action point 2 – Day Service Modernisation
A number of people have written to Robert about Day Service
Modernisation. Also the User Forum has given their comments.
ACTION POINT 1 – The board agreed that all comments
should be included in the report that goes to the Executive
Committee.
Linda Jordan has the day Service Modernisation report but is yet
to comment.
Action point 4 – Service user seeking asylum
The Department of Health (DoH) responded to Linden’s letter
asking if the person in question was a minor. There has been no
further response from the DoH as yet.
Action point 5 – Prevention Strategy
This has been deferred to the next meeting in January.
5. Questions
Neil went through the question from
Margaret Eynon of the User Friendly
group.
Q. Why are the flats boarded up on Broadmore Gardens?
A. Chris Knaggs – Housing Manager from Lambeth Housing has
not been able to establish who owns these properties and
therefore cannot comment why the flats are boarded up. Chris will
pass the information to the empty homes officer.
Q. Manny Oraka asked why there are so many rough sleepers
people in Lambeth.
A. Linden stated that rough sleeper figures in Lambeth had
actually gone down recently.
6. Health Action Plans (HAP)
Irene Bezzina, Pauline Pepperrell and Gillian Bretherton presented
slide show overheads about what HAPs are, how they can help
you and how to get one.
A Health Action Plan writes what needs
to be done to help the person to stay
healthy. It also writes what support the
person may need to do this.
A health facilitator helps you make your
HAP. A Health facilitator can be a friend or a family member or
even yourself.
The targets of ‘Valuing People’ are that all people with a learning
disability should have a Health Action Plan by June 2005.
There is to be a HAP steering group set up soon, so decisions can
be made on how to let people know about HAP.
GP’s have been informed and the PCT
have training schemes for GP’s in
January and February.
PCP and HAP do have links but it was stated that each has a
purpose within its own right. One could work in tandem with the
other. Register services were highlighted as a good way to let
people know about HAP and PCP.
