the patient and arranged <strong>for</strong> a primary care provider to prescribe medication. The study<br />

found that the case manager’s involvement significantly improved rates of treatment,<br />

BMD testing, and appropriate care. 44<br />

Another study of elderly patients used wrist fracture as a sentinel event, and tested a<br />

multifaceted intervention, which included telephone-based education <strong>for</strong> patients and<br />

clinical guidelines <strong>for</strong> providers. The study found that the intervention group was more<br />

likely than the control to undergo BMD testing and to receive appropriate care. 45<br />

One proposed strategy is to develop “<strong>Bone</strong> <strong>Health</strong> Teams” composed of all health care<br />

professionals who interact with fracture patients at various stages of the continuum,<br />

from emergency department admission to discharge to rehabilitation. Such a team<br />

would monitor patients’ progress, encourage BMD testing, and make appropriate treatment<br />

recommendations.<br />

In addition, expanded provider education is needed. <strong>Bone</strong> health is not taught or<br />

structured as a distinct subject in medical schools. There is, <strong>for</strong> instance, no specialty<br />

in osteoporosis and bone disease (although some rheumatologists and endocrinologists<br />

are recognized as experts in the United States, and some centers specialize in<br />

osteoporosis treatment and management). Education <strong>for</strong> professionals needs to focus on<br />

bone health as an essential component of good health. Teaching medical and nursing<br />

students to recognize that bone health is an essential element of good health is an<br />

important addition to health education curricula. <strong>Bone</strong> health continuing education also<br />

should be included as an element <strong>for</strong> all licensing and credentialing programs, as well<br />

as <strong>for</strong> certification and recertification processes.<br />
