Dear students and families,<br />
Welcome to another summer with the <strong>Carroll</strong><br />
<strong>Academy</strong>. Our offerings in STEM, art, music, theatre,<br />
languages, reading, writing and other subjects allow<br />
students to grow and learn as they explore new<br />
areas of interest and enrich their current knowledge.<br />
This summer, we will offer “<strong>Carroll</strong> Kids Week,”<br />
a one-week collection of classes that will give<br />
students a sampling of the opportunities available<br />
this summer along with some one-time specialty<br />
offerings. Each day, students will enjoy a variety<br />
of interesting classes. Scheduling is flexible, so<br />
students may sign up for a full week, half a day, or<br />
any half- or full-day combination. Classes include<br />
introductions to Mathbusters, Young Scientists,<br />
improv, chess, acting, radio production and hosting,<br />
and many others.<br />

CARROLL ACADEMY EDUCATIONAL STATEMENT<br />
<strong>Carroll</strong> <strong>Academy</strong> is the community outreach program of <strong>Carroll</strong> <strong>University</strong>. Non-credit<br />
enrichment classes and summer camps are offered for ages two to adult in a variety of<br />
academic subject areas.<br />
Specifically the program aims to:<br />
• Provide high quality classes and camps that promote student learning. Class<br />
participants are challenged through individualized instruction and the practical<br />
application of skills.<br />
• Augment public, private, homeschool or self-directed study through concentrated<br />
coursework in specific subject areas.<br />
• Encourage intellectual, physical and/or social development and lifelong skills.<br />
Class participants grow and learn, explore new ideas, develop their creativity and<br />
transfer these skills to other areas of their lives.<br />
• Broaden students’ knowledge base by exposing them to new subjects and sources<br />
of learning. Students try new activities as they explore areas of interest. Middle<br />
and high school students strengthen their current skills and gain an overview of<br />
potential careers.<br />

The <strong>Carroll</strong> <strong>Academy</strong>, within the context of the total <strong>Carroll</strong> educational experience,<br />
seeks to prepare college student participants to achieve their full potential. The<br />
university’s educational philosophy is sustained by the four pillars of integrated<br />

Integrated Knowledge through breadth of learning. Students are encouraged to<br />

ENVIRONMENTAL SCIENCE:<br />
GARDENING AND COMPOSTING<br />
Students entering grades 2-5:<br />

Learn about gardening and composting in<br />
this hands-on class. We will use the <strong>Carroll</strong><br />
<strong>University</strong> Community Garden as our main class<br />
site, where we will plant, tend and water the<br />
garden, and explore composting. We will gather<br />
ripe vegetables that were planted in the spring<br />
by <strong>Carroll</strong> <strong>University</strong> students and distribute<br />
the vegetables through the Food Pantry of<br />
Waukesha County.<br />

You’re “On the Air.” Learn how to design,<br />
produce and host a radio program. This class<br />
will cover program design, interviews, legal<br />
issues, editing, and equipment. It will also<br />
include the actual production and hosting of a<br />
program by the class participants.<br />

Create interactive stories, animations,<br />
games, music and art using “Scratch”<br />

core computational concepts such as iteration<br />
and conditionals, and gain an understanding<br />
of important mathematical concepts such as<br />
coordinates, variables and random numbers.<br />
Creating projects in Scratch will help students<br />
develop a deeper level of fluency with digital<br />
technology, as they learn about the process of<br />
design. This project-design process combines<br />
many of the 21st century learning skills that<br />
will be critical to success in the future: thinking<br />
creatively, communicating clearly, analyzing<br />
systematically, collaborating effectively,<br />
designing iteratively and learning continuously.<br />

The <strong>Carroll</strong> <strong>Academy</strong> offers a year-round<br />
program that includes lessons, classes, recitals<br />
and a variety of opportunities. Professional<br />
teachers hold bachelors’ degrees; many have<br />
masters’ degrees. Some advanced level <strong>Carroll</strong><br />
music majors also teach private music lessons.<br />
PIANO<br />
Ages 4.5 years to adult<br />
For students of all ages and levels. Develop<br />
a solid foundation of piano playing and music<br />
reading skills as you progress through a<br />
comprehensive curriculum and a logical<br />
sequence of enjoyable repertoire. School-age<br />
students may perform in recitals, take master<br />
classes, compose and perform their own music<br />
and participate in a piano festival.<br />
VIOLIN<br />
Ages 3 years to adult<br />
A challenging program for all levels and ages.<br />
Students progress at their own pace and develop<br />
an ability to play with expression, sensitivity and<br />
a beautiful tone. Older students and adults at<br />
advanced levels are welcome. Students may<br />
participate in recitals and special events.<br />
VOICE<br />
Ages 13 years to adult<br />
Private voice lessons may begin in the seventh<br />
grade or with permission from the choral<br />
coordinator. Emphasis is placed on developing<br />
proper vocal technique, intonation, tone quality,<br />
breathing, articulation and phrasing. Voice<br />
students may study privately or in a group.<br />
Students younger than age 13 may take a<br />
combination of musicianship or sight singing and<br />
voice lessons. (See below.)<br />
MUSICIANSHIP<br />
An exciting experience that integrates listening,<br />
music reading, learning basic concepts and music<br />
from around the world. Lessons are designed to<br />
stimulate the minds of young musicians through<br />
listening to historical pieces, learning about<br />
famous composers and instruments from around<br />
the world, and integrating music from each<br />
student’s experience into his/her understanding<br />
of music. Various forms of expression will be<br />
encouraged, including student composition,<br />
writing and talking about music, learning dances<br />
from other cultures and even drawing. Students<br />
from all musical backgrounds are welcome.<br />
Ability to read music is not required.<br />
INSTRUMENTAL, VOCAL, SIGHT<br />
SINGING AND MUSICIANSHIP<br />
LESSONS<br />
Summer is a great time to start or continue<br />
lessons because there is more time for practice.<br />
Lessons are scheduled based on the availability<br />
of the student and instructor. Note: Individual<br />
faculty might vary the teaching weeks and<br />
have asked students to discuss their vacation<br />
schedules as they arrange lesson times. There<br />
will be one makeup lesson allowed.<br />
Some faculty teach additional lessons in the<br />
summer. Students who wish to take more lessons<br />
than the basic package below may talk with their<br />
instructor. The basic package should be paid for<br />
at the time the first eight lessons are scheduled.<br />
A separate form for additional lessons may be<br />
obtained from the <strong>Carroll</strong> <strong>Academy</strong> office or your<br />
instructor at your first lesson.<br />

Braille is a mystery language, a system of letters<br />
or numbers symbolized by raised dots (or bumps)<br />
that enables people who are blind or visually<br />
impaired to read and write. You might have<br />
seen Braille on elevators, room signs, menus<br />
at restaurants and at your local public library.<br />
Have you ever wondered what the different dot<br />
combinations mean? In this introductory class,<br />
you will learn how to decipher each of the dot<br />
combinations that are part of the Braille system<br />
and make your own Braille using a Braille writer<br />
and a slate and stylus. You will use the Braille<br />
alphabet to read children’s books and other<br />
materials, and write a short story that others can<br />
decode. Learning Braille is fun, but it might be a<br />
bumpy ride.<br />

NOTICE TO ALL PARENT(S)/LEGAL GUARDIAN(S) OF STUDENT PARTICIPANTS UNDER<br />
THE AGE OF 18 IN THE CARROLL ACADEMY PROGRAM<br />
The <strong>Carroll</strong> <strong>Academy</strong> program is sponsored by <strong>Carroll</strong> <strong>University</strong> (“<strong>Carroll</strong> <strong>Academy</strong>”) for the purpose of offering community outreach enrichment<br />
classes for children and adults. As part of the learning experience, student Participants in <strong>Carroll</strong> <strong>Academy</strong> will, from time to time,<br />
engage in field trips (both on and off the <strong>Carroll</strong> <strong>University</strong> grounds), sports and other indoor and outdoor recreational activities. Many of<br />
these activities involve the risk of bodily injury, property damage, and other dangers associated with participation therein. Dangers may<br />
include, but are not limited to, the following: broken bones, strains, sprains, bruises, concussions, heart attack, heat exhaustion and, in some<br />
cases, even death. Each student Participant in the <strong>Carroll</strong> <strong>Academy</strong> program, and the parent(s)/legal guardian(s) of each student Participant,<br />
must acknowledge these risks and dangers, which are often inherent in both the activities themselves, as well as the travel to and from the<br />
site of the activities.<br />
Each student Participant, and the parent(s)/legal guardian(s) of each student Participant, who decides to participate in activities associated<br />
with the <strong>Carroll</strong> <strong>Academy</strong> program is doing so voluntary. It is the responsibility of each student Participant, and the parent(s)/legal<br />
guardian(s) of the student Participant, to make sure the student Participant participates only in those activities for which s/he has the requisite<br />
skills, qualifications, preparations and training. <strong>Carroll</strong> <strong>Academy</strong> does not warrant or guarantee in any respect the competency or mental<br />
or physical condition of any student Participant in any <strong>Carroll</strong> <strong>Academy</strong> activity.<br />
As a condition of participation in <strong>Carroll</strong> <strong>Academy</strong> activities, each student Participant, and the parent(s)/legal guardian(s) of each student<br />
Participant, is required to:<br />
(1) carefully review and acknowledge receipt of this Notice and the corresponding Release and Waiver of Liability, and ask any<br />
questions of <strong>Carroll</strong> <strong>Academy</strong> personnel if the student Participant, and/or the parent(s)/legal guardian(s) of the student<br />
Participant, are in any way uncertain about this document’s intent and/or meaning; and<br />
(2) carefully review and abide by the principles of conduct set forth in the <strong>Carroll</strong> <strong>Academy</strong> Catalog (additional copies of which<br />
can be obtained at the <strong>Carroll</strong> <strong>Academy</strong> Office).<br />
RELEASE AND WAIVER OF LIABILITY<br />
I, _______________________________________ (name/please print) am the parent/legal guardian of _______________________________________<br />
(name/please print), who is a student Participant in the <strong>Carroll</strong> <strong>Academy</strong> program. I hereby give approval for the student Participant’s enrollment<br />
and participation in <strong>Carroll</strong> <strong>Academy</strong> and all of the activities involved therein. The student Participant’s participation in <strong>Carroll</strong> <strong>Academy</strong><br />
and all of its activities is by choice, and is completely voluntary.<br />
Accordingly, in my capacity as parent/legal guardian for the student Participant, I agree on behalf of myself, the above-named student Participant,<br />
my spouse, my heirs, assigns, related individuals and related entities, to hereby release, waive, absolve, discharge and agree to hold<br />
harmless <strong>Carroll</strong> <strong>Academy</strong>, its trustees, directors, officers, employees, agents and insurers (collectively, the “<strong>Carroll</strong> <strong>Academy</strong> Released<br />
Parties”) from and against any and all rights, claims, demands, causes of action, obligations, suits, liens, damages or liabilities of any kind<br />
and character whatsoever, whether known or unknown, suspected or claimed, which we shall or may have in the future against the <strong>Carroll</strong><br />
<strong>Academy</strong> Released Parties arising out of, based on, related to or connected with the student Participant’s enrollment and participation in the<br />
<strong>Carroll</strong> <strong>Academy</strong> program and activities. We also agree to indemnify and hold the <strong>Carroll</strong> <strong>Academy</strong> Released Parties harmless from the payment<br />
of any and all judgments, settlements, costs, disbursements and attorney fees that are associated with the <strong>Carroll</strong> <strong>Academy</strong> Released<br />
Parties having to defend or investigate any claim, action or proceeding of any type whatsoever arising out of the student Participant’s enrollment<br />
or participation in the <strong>Carroll</strong> <strong>Academy</strong> program and activities including, but not limited to, claims for breach of contract, negligence<br />
(whether caused by the sole negligence of <strong>Carroll</strong> <strong>Academy</strong>, the concurrent negligence of <strong>Carroll</strong> <strong>Academy</strong> with another, or the negligence<br />
of any other person), strict liability or otherwise. This indemnification obligation and this Release and Waiver of Liability does not, however,<br />
absolve the <strong>Carroll</strong> <strong>Academy</strong> Released Parties from any liability, damages, costs, disbursements and attorney fees incurred due to their intentional<br />
or reckless conduct.<br />
I fully understand that if any fact with respect to which this Release and Waiver of Liability is executed is found hereafter to be other than or<br />
different from the fact in that connection now believed by me to be true, I expressly accept and assume the risk of such possible difference in<br />
fact and agree that this Release and Waiver of Liability shall be and remain effective notwithstanding such difference in facts.
COVENANT NOT TO SUE OR OTHERWISE TAKE ACTION<br />
The student Participant, and the parent(s)/legal guardian(s) of the student Participant, covenant and agree that they will forever refrain from instituting,<br />
prosecuting, maintaining, proceeding on, assisting with or advising to be commenced a suit which arises out of, or may be, in whole or<br />
in part, based upon, related to or connected with the released matters herein or any part of them.<br />
CONSENT TO MEDICAL CARE AND TREATMENT<br />
In the event the student Participant is injured during a <strong>Carroll</strong> <strong>Academy</strong> activity, the student Participant and the parent(s)/legal guardian(s)<br />
of the student Participant, consent to treatment of the student Participant’s injury by a professional employed by or otherwise under contract<br />
with <strong>Carroll</strong> <strong>Academy</strong>, subject to the provisions of the above Release and Waiver of Liability. The student Participant and the parent(s)/legal<br />
guardian(s) of the student Participant, authorize <strong>Carroll</strong> <strong>Academy</strong>, at its discretion, to arrange for the student Participant’s transport to a hospital<br />
or other medical facility for further medical attention. The student Participant and the parent(s)/legal guardian(s) of the student Participant,<br />
understand and agree that <strong>Carroll</strong> <strong>Academy</strong> is not responsible for transporting the student Participant to a hospital or medical facility that participates<br />
in any particular insurance/managed care plan.<br />
PHOTOGRAPHIC CONSENT AND RELEASE<br />
I hereby authorize <strong>Carroll</strong> <strong>University</strong>, Inc. (the “university”) and those acting pursuant to its authority to:<br />
a. Record my likeness and voice on a photographic, video, audio, digital, electronic or any other medium.<br />
b. Use my name in connection with these recordings.<br />
c. Use, reproduce, exhibit or distribute in any medium (ex. print publications, CD, DVD, Internet) these recordings for any purpose that<br />
the university, and those acting pursuant to its authority, deem appropriate, including promotional or advertising efforts.<br />
I release the university and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in<br />
connection with such use. I understand that all such recordings, in whatever medium, shall remain the property of the university. I have read and<br />
fully understand the terms of this release.<br />
REPRESENTATIONS AND WARRANTIES<br />
I represent and warrant that I have the full power, capacity and authority to execute this document as the parent or legal guardian of the student<br />
Participant. I further represent and warrant that I understand the terms and provisions of this document and its nature and effect.<br />
APPLICABLE LAW<br />
This document is made, executed and entered into and shall be governed by the laws of the state of Wisconsin. I expressly consent to the venue<br />
and jurisdiction of the Wisconsin courts with respect to any dispute arising out of the student Participant’s enrollment and participation in the<br />
above-referenced <strong>Carroll</strong> <strong>Academy</strong> program and associated activities.<br />
I HAVE READ THE FOREGOING PROVISIONS OF THIS DOCUMENT CAREFULLY AND FULLY UNDERSTAND THEM.<br />
