(80) Postoperative Atrial Fibrillation:<br />

(A) Format: Numeric, length 1<br />

(B) Valid Values:1 = Yes; 2 = No<br />

(C) Category: Complications<br />

(D) Definition/Description: Indicate whether the patient had a new onset of<br />

atrial fibrillation/flutter (AF) requiring treatment. Does not include<br />

recurrence of AF which had been present preoperatively.<br />

(81) Facility Identification Number:<br />

(A) Format: Numeric, length 6<br />

(B) Valid Values: Free Text<br />

(C) Category: CCORP<br />

(D) Definition/Description: The six-digit facility identification number<br />

assigned by the Office, as defined in Section 97170.<br />

(b) If a value for a data element, other than data elements specified in<br />

Subsection (b)(1), is unknown or not applicable, a hospital may submit the<br />

record without a value for that data element.<br />

(1) A valid value must be submitted for the following data elements: Facility<br />

Identification Number, Medical Record Number, Responsible Surgeon<br />

Name, Responsible Surgeon California License Number, Isolated CABG,<br />

Date of Surgery, Date of Discharge, Discharge Status, Sex, Status of the<br />

Procedure, Dialysis, Prior PCI, Reoperation for Bleed/Tamponade,<br />

Reoperation for Graft Occlusion, Deep Sternal Wound Infection,<br />

Postoperative Stroke, Continuous Coma >=24 hours,<br />

Prolonged Ventilation, Postoperative Renal Failure, Postoperative<br />

Dialysis, and Postoperative Atrial Fibrillation.<br />

97176. Reporting periods and due date<br />

(a) During each calendar year there are two reporting periods. The first reporting<br />

period is January 1 through June 30; the second period is July 1 through<br />
