Class Performance/Listening Behaviors<br />

Use this checklist to document class performance/listening behaviors before a student uses an assistive listening device for a minimum three week observation period. If it is determined<br />

that a trial period <strong>of</strong> using the device is needed, complete this form again after the student has used the device for approximately four weeks.<br />
