Nomination Form - Sri Shirdi Saibaba Temple

Dear Members,
SSSS, MI invites nominations for Executive Committee (EC) and One Board of Trustee
(BOT) position. Term of EC position is for ONE year (Aug 2010 to July 2011) and for BOT
three years (Aug 2010- July 2013). Please see the attached nomination form. If you would like
to go for nomination for any position, please read the following article regarding the election
process.
The duly filled nomination form must be received by July 15 th 2010 and should be dropped in
designated drop off box located in the Main Prayer Hall at SRI SHIRDI SAIBABA
TEMPLE, 28875, W. Seven Mile Rd. Livonia, MI 48152.
General guidelines for Elections:
i) Contesting member should be MI resident
ii) No person shall contest for more than one position.
iii) One nomination for each member family.
iv) All filled in nominations should be dropped at Temple designated drop box on or
before Thursday, July 15 th 2010 before 8.00 PM.
v) Nominations will be scrutinized and informed via email to respective candidates by
July 16 th 2010.
vi) Last day for withdrawal of nomination by 5.00 PM, Sunday, July 19 th 2010 by email
to election_committee@saibabami.org
vii) Election committee will determine go/no-go for elections (by ballot) on 22 nd July 2010
for any given position(s).
viii) Incase of more than one nomination for same position, elections will be conducted by
election committee.
ix) Ballots will be mailed by Jul 26 th 2010 and completed ballots should be post marked
by Aug 2 nd 2010.
x) Ballots will be counted in case of elections on Aug 6 th 2010 and results will be
communicated to the Chairman.
xi) Make sure following are completely filled in and enclosed with the nomination forma)

c) Election expenses donation should be paid along with the nomination. Election
expenses donation along with nominations: President-elect $100, Board of
Trustee from general body members $100 and all other Executive Committee
positions $25. Make the check payable to “Shirdi Sai MI”.
d) Photo copy (main page) of your current utility bill or phone bill, as an evidence
of your current residential address.
