<strong>The</strong> <strong>Income</strong> & <strong>Expense</strong> <strong>Declaration</strong> (Form FL-150) – also known as the “I & E” --covers fin<strong>an</strong>cial<br />
information required for m<strong>an</strong>y family law proceedings, such as fin<strong>an</strong>cial disclosures for divorce cases<br />
<strong>an</strong>d basic information for calculating child <strong>an</strong>d spousal support.<br />
In most cases, the <strong>Income</strong> & <strong>Expense</strong> <strong>Declaration</strong> is filed along with other forms, such as a Petition<br />
for Dissolution or <strong>an</strong> Order to Show Cause to Modify Child Support. However, there may be a few<br />
occasions when you need to only file the <strong>Income</strong> & <strong>Expense</strong> <strong>Declaration</strong>. Examples: the judge has<br />
asked for <strong>an</strong> updated form to be filed or the Department <strong>of</strong> Child Support Services has asked you to<br />
send this information to your assigned Child Support Case Worker as part <strong>of</strong> <strong>an</strong> informal review.<br />
In order to assist you when only this form is needed, this <strong>Income</strong> & <strong>Expense</strong> form kit was created.<br />
<strong>Caution</strong>: <strong>The</strong> <strong>filing</strong> <strong>of</strong> <strong>just</strong> <strong>an</strong> <strong>Income</strong> & <strong>Expense</strong> <strong>Declaration</strong><br />
(<strong>without</strong> <strong>an</strong>y other paperwork) will NOT get you a court date. If<br />
you are <strong>filing</strong> this form solo, usually it is because the judge has<br />
asked for updated information prior to a hearing which is already<br />
on the court’s calendar.<br />
<strong>The</strong> following documents <strong>an</strong>d information will be necessary to fill out your forms<br />
completely <strong>an</strong>d accurately:<br />
Pro<strong>of</strong> <strong>of</strong> income [attach to your form when you file it]<br />
o Current pay stubs for the last 2 months OR<br />
o If you are self-employed, your Schedule “C” from last income tax<br />
return<br />
Information regarding the custody/visitation <strong>of</strong> each parent<br />
Information regarding the other party’s income, if known<br />
<strong>Expense</strong> information
<strong>The</strong> information that you put into the forms CANNOT be saved. It is very import<strong>an</strong>t that you<br />
print <strong>an</strong>y forms that you need prior to closing the program.<br />
Look for the yellow question marks for expl<strong>an</strong>ations. When you see a yellow question mark (?),<br />
simply roll your cursor over it.<br />
Don’t worry if some areas within the forms c<strong>an</strong>not be filled in. That information shouldn’t be<br />
pertinent to your case. If you feel it is, you most likely have the incorrect form set.<br />
