The Income & Expense Declaration (Form FL-150) – also known as the “I & E” --covers financial
information required for many family law proceedings, such as financial disclosures for divorce cases
and basic information for calculating child and spousal support.
In most cases, the Income & Expense Declaration is filed along with other forms, such as a Petition
for Dissolution or an Order to Show Cause to Modify Child Support. However, there may be a few
occasions when you need to only file the Income & Expense Declaration. Examples: the judge has
asked for an updated form to be filed or the Department of Child Support Services has asked you to
send this information to your assigned Child Support Case Worker as part of an informal review.
In order to assist you when only this form is needed, this Income & Expense form kit was created.
Caution: The filing of just an Income & Expense Declaration
(without any other paperwork) will NOT get you a court date. If
you are filing this form solo, usually it is because the judge has
asked for updated information prior to a hearing which is already
on the court’s calendar.
The following documents and information will be necessary to fill out your forms
completely and accurately:
Proof of income [attach to your form when you file it]
o Current pay stubs for the last 2 months OR
o If you are self-employed, your Schedule “C” from last income tax
return
Information regarding the custody/visitation of each parent
Information regarding the other party’s income, if known
Expense information

The information that you put into the forms CANNOT be saved. It is very important that you
print any forms that you need prior to closing the program.
Look for the yellow question marks for explanations. When you see a yellow question mark (?),
simply roll your cursor over it.
Don’t worry if some areas within the forms cannot be filled in. That information shouldn’t be
pertinent to your case. If you feel it is, you most likely have the incorrect form set.
After you print, please stop by one of our Resource Centers so we can sit down with you,
review your papers, and answer all of your questions.
