Medical workforce Out of the frying pan and into the fire 1. The following should inform the debate on increasing medical student numbers in WA: a. In 2005 107 students graduated from the then WA Medical School b. In 2012 321 students graduated from WA’s two medical schools – 281 of these students were Australian residents who remained in WA for their internship c. Based on already established student places, from 2015 there will be above 320 graduates per year d. For the number of intern positions to increase, WA will have to find and pay for additional placements and supervision e. There is increased competition for training positions at PGY3 level and above; and this will only become more competitive with the bulge of doctors currently progressing through the system 2. Based on current figures, from 2015, graduate numbers will be triple that of 2005 – most of these graduates will be seeking training posts. 3. By 2020 the impact of this tripling effect will have flowed through to independent practice. This means WA will be producing more than triple the number of fully trained GPs and other specialists than in 2005. 4. The tripling of medical student numbers nationally will leave some States struggling to provide good intern and training positions. If graduate numbers escalate again, further difficulties will arise. 5. A new graduate is only halfway through their training pathway, with their training post-graduation being predominantly financed by the State Budget. The rapid increases in medical graduate numbers over recent years has placed pressure on a system that is already struggling to provide the level of teaching and training required to ensure the next generation of doctors are efficient, cost effective and provide quality care. The increase in student numbers (refer Fig. 1) has been unprecedented and designed to address the caps on low medical student numbers of previous years, excessive reliance on importing practitioners from overseas, and provides more Australian students an opportunity to practice medicine and meet the needs of our growing population. However, not surprisingly, there have been significant challenges in keeping up with these increases. The output of medical schools, since the University of Notre Dame’s (UND) initial 74 graduate output in 2009, has continued to increase. The University of Western Australia (UWA) and UND have accumulatively increased graduate numbers by over 200%. Strong lobbying of the State Government has been needed to ensure all domestic graduates are guaranteed intern positions, and seek to secure adequate funding and resources for pre-vocational and vocational training. However challenges still remain and as the system struggles to catch up with the growth in student numbers, there is a hotly debated push to increase these figures. Curtin University has been lobbying hard to establish a new undergraduate medical school. The advocacy of an undergraduate course is understandable, given the UWA recently moved away from this model and the UND has only ever offered a postgraduate degree. However, many people in the health sector are concerned about the proposed numbers and timeframe. Due to this the AMA (WA), over the last two years, has repeatedly requested detailed justification for Curtin’s proposal. In particular, the Association has, in discussion with various Ministers, the Director General and Curtin, advised the need to: 1. Ascertain the flow-through effect of the current increase in student numbers of more than 200%, with due consideration being given to the entire educational vocational continuum. 2. Provide a sound evidence-based justification for the numbers proposed. 3. Provide a properly integrated, funded and resourced package that provides the requisite staff, resources and infrastructure in order to: a. Support the education of additional medical students b. Support the pre-vocational and vocational education of these additional graduates c. Increase the capacity for employment of medical graduates and junior doctors.
