Industrial: Cost Shifting and/or Privatisation The State Government has for some time been seeking to change the manner in which services are provided in public hospitals. This might involve increasing private activity or privatising existing public activity; such changes need to be very carefully considered and, if appropriate, legal advice sought. The Association has previously provided detailed advice in relation to the Ambulatory Surgery Initiative (ASI) and Privately Referred Non In-patients (PRNI), and secured a commitment to improved additional indemnity coverage for such initiatives. Given previous adverse publicity and investigations by external authorities, the Association would urge practitioners to exercise due diligence and seek appropriate advice from the Association and their Medical Defence Organisation in relation to any information provided by hospitals before considering participating in any such initiatives. Medical practitioners should be wary of management which seeks to insist there is an obligation to participate in revenue-raising arrangements in order to maintain access to Arrangement A. Whilst the Association strongly supports clinicians and hospitals legitimately maximising income, great care needs to be taken to avoid replicating problems that have arisen in the past by overenthusiastic or naive management embarking upon schemes which subsequently result in significant adverse publicity, investigation and emotional distress. Once such situation occurred a few years ago in relation to a trust fund enquiry at one of our major hospitals. If what is being proposed is a change in traditional private practice activities for the Department, or if eligible patients may not be given the choice of electing to be treated for free as public patients, or if there is a lack of independent external confirmation of the lawfulness or otherwise of what is being proposed, clinicians are strongly advised to seek independent external advice. Health Service Governing Councils The expertise and leadership of five former presidents of the Australian Medical Association (WA) have been recognised by their appointment by the State Government to the newly established Health Service Governing Councils. Health Minister Kim Hames said he had reached into the medical community to access the wealth and experience held by AMA (WA) former presidents. “Governing Councils will bring together a wealth of knowledge and experience from across the community, clinical, corporate and government sectors,” Dr Hames said. “Each council will play a key role in planning the future for WA’s public health services, and ensure effective community and clinician engagement in local health services planning,” he said. Several AMA (WA) past-presidents have been appointed by the State Government: Professor Bryant Stokes (Chair, South Metropolitan Governing Council); Dr Simon Towler (Member, South Metropolitan); Associate Professor Rosanna Capolingua (Chair, Child and Adolescent Health Service Governing Council); Professor Gary Geelhoed (Member, Child and Adolescent Governing Council); and Professor Geoff Dobb (Chair, Southern Country Health Service Governing Council). A number of other AMA (WA) members were also appointed to the new Governing Councils: Professor Fiona Wood (South Metropolitan); Professor Julie Quinlivan (North Metropolitan); Dr Michael Levitt (North Metropolitan); Dr Michiel Mel (Southern Country); Dr Ian (Val) Lishman AM (Southern Country); Dr Phillip Montgomery (Northern and Remote Country Health Service Governing Council); and Dr Shane Kelly (Child and Adolescent Health Service). The AMA (WA) has called for more information to be provided by the State Government on how Governing Councils will work and how their recommendations will be dealt with.
