Physical Evidence Manual<br />

The Laboratory Case #, Submission #, Laboratory Exhibit, and the bottom portion of the<br />

form should be left blank.<br />

If the form is not available, a letter containing the following information should be submitted<br />

with the evidence:<br />

1. The date of the letter<br />

2. The originating agency name<br />

3. The agency case number<br />

4. The county of venue<br />

5. The nature of the offense and the offense date<br />

6. The victim or complainant (an establishment or individual) and the date of birth of<br />

individuals<br />

7. The names of suspects and their dates of birth<br />

8. The evidence submitted (described in detail)<br />

9. The type of examination requested<br />

10. The name of the officer submitting the report or investigating the case, and the<br />

officer's commission and telephone numbers.<br />

11. Notations concerning accidental or intentional changes which the submitting officer<br />

has made in the exhibits<br />

Rushes<br />

If your request is a “Rush”, indicate so in the appropriate location on the Forensic Services<br />

Request (Form 49), along with the date the request needs to be completed by, if known.<br />

Common reasons for “Rushes” are upcoming grand jury and trial dates, and an immediate<br />

need for investigative leads.<br />

Laboratory Reports<br />

The laboratory will issue a report written by the analyst(s) on all examinations<br />

performed. Copies of the report are mailed to the attention of the investigating officer.<br />

Additional copies may be sent to the District Attorney’s Office or other involved<br />

agencies/individuals, if applicable.<br />

Forensic Laboratory Evaluation Form<br />

Periodically, the Forensic Services Division will mail out evaluation forms to its service<br />

agencies. These forms provide valuable feedback on our goal of providing quality and<br />

timely scientific, technical, and investigative support. Should you receive one of these<br />

evaluation forms, please fill it out completely.<br />
