Pers<strong>on</strong>alisati<strong>on</strong> means putting people first, giving them choice and<br />

c<strong>on</strong>trol and enabling them to live as they wish, whether they need<br />

support from others now, or in the future.<br />

What does care co-ordinati<strong>on</strong> mean for you?<br />

Having staff from health and social care work together in the<br />

community mental health teams means that the service is organised<br />

so that it is easier :<br />

the heart of all that we do and for adult care this means being flexible<br />

and offering every <strong>on</strong>e who is eligible for social care services the<br />

opportunity to direct their own support.<br />

Self directed support empowers people to take c<strong>on</strong>trol and shape<br />

their own lives by identifying their own needs and organising their<br />

own support to meet those needs in a way that makes sense to<br />

them. This can be achieved through pers<strong>on</strong>al choice by either<br />

accessing services the council provide, through approved services<br />

provided by an independent organisati<strong>on</strong> or through the offer of a<br />

direct payment.<br />

What happens when some<strong>on</strong>e first c<strong>on</strong>tacts the community<br />

mental health team?<br />

When you c<strong>on</strong>tact us you will be asked some questi<strong>on</strong>s by a<br />

member of health or social care staff to identify how best we can<br />

help you. They can:<br />

give you the advice and informati<strong>on</strong> you need<br />

redirect you to some<strong>on</strong>e else who can offer more appropriate<br />

help and / or<br />

arrange for you to have an assessment to find out what support<br />

you need.<br />

What is an assessment?<br />

An assessment is carried out with you to identify what your needs<br />

are and to determine what services are suitable to support your<br />

needs.<br />

We will arrange for an assessment to be carried out by the most<br />

appropriate pers<strong>on</strong> for you.
