Tees, Esk and Wear Valleys NHS Foundation Trust provides a range of mental health, learning disability and substance misuse services for the 1.3 million people living in County Durham, the Tees Valley and the Scarborough, Whitby and Ryedale areas of North Yorkshire. Our service users and their carers are at the heart of everything we do and we are continuously developing our services to better meet their individual needs. We want to make sure that people are able to access the specialist care and treatment they need quickly and as near to their homes as possible. The staff involved in the care of people with mental health needs from health and social care have joined together to form community mental health teams. This includes social workers, community mental health nurses, doctors, occupational therapists, and support workers. The community mental health teams work with people with mental health needs to identify: What do I do in a crisis? Contact the team between 9am and 5pm, Monday to Friday and ask for your worker or the duty worker. We want to make it easier for you to see the right professional at the right time without having to have more than one assessment of your needs. This means that you won’t have to be asked the same questions more than once. This way of arranging the services you need is called care co-ordination.
Moving forward on personalisation Personalisation means putting people first, giving them choice and control and enabling them to live as they wish, whether they need support from others now, or in the future. What does care co-ordination mean for you? Having staff from health and social care work together in the community mental health teams means that the service is organised so that it is easier : We are committed to putting every person who needs our support at the heart of all that we do and for adult care this means being flexible and offering every one who is eligible for social care services the opportunity to direct their own support. to understand to ask for information, advice, help and support to provide more choice in the help available to you Self directed support empowers people to take control and shape their own lives by identifying their own needs and organising their own support to meet those needs in a way that makes sense to them. This can be achieved through personal choice by either accessing services the council provide, through approved services provided by an independent organisation or through the offer of a direct payment. What happens when someone first contacts the community mental health team? When you contact us you will be asked some questions by a member of health or social care staff to identify how best we can help you. They can: give you the advice and information you need redirect you to someone else who can offer more appropriate help and / or arrange for you to have an assessment to find out what support you need. What is an assessment? An assessment is carried out with you to identify what your needs are and to determine what services are suitable to support your needs. We will arrange for an assessment to be carried out by the most appropriate person for you.
