DOCUMENTATION REQUIREMENTS<br />
Public Notification: The project must inform the public that the proposed project will be located in a<br />
floodplain/wetland area, and that the project will have certain environmental impacts on that<br />
floodplain/wetland area, and must allow public comment. The following actions must be taken:<br />
A. The applicant must publish a notice in the <strong>local</strong> media describing the proposed action in the<br />
floodplain or wetland area and invite the public to provide their views on the proposal. This may<br />
be done by holding a public hearing or making the plans and other pertinent materials available<br />
for review at a public place (i.e., city hall, courthouse, library, etc.). Describe actions taken.<br />
B. The application must contain a copy of the published notice and any public comments received.<br />
If no public comments are received, this must be clearly stated.<br />
Any public hearing or comments received within two years of the application submission will be<br />
accepted.<br />
ADDITIONAL INFORMATION<br />
C. The following environmental information focusing on the floodplain or wetland activity must be<br />
included and attached in this section of the application:<br />
• The extent of the direct and indirect impacts of the project on the floodplain/wetland area.<br />
• Measures to be taken to minimize harm to lives and property and to the natural and<br />
beneficial floodplain/wetland values.<br />
• Alternative actions and locations considered in the event of an adverse impact of the project<br />
on floodplain/wetland values.<br />
• Assurance that all state and <strong>local</strong> floodplain and wetland regulations are being met.<br />
• A map delineating the floodplain/wetland area as it applies to the proposed project (highlight<br />
the project area).<br />
