you by the University of Virginia and<br />
<strong>United</strong> <strong>Concordia</strong>, a dental insurer with<br />
more than 31 years of experience.<br />
<strong>United</strong> <strong>Concordia</strong> offers a complete<br />
range of dental products and delivery<br />
models through contracted providers to<br />
6.5 million Americans. *<br />
With <strong>Concordia</strong> <strong>Flex</strong>, you are free to<br />
use any licensed dentist you wish, but<br />
the plan’s value and convenience is<br />
maximized when you use a <strong>United</strong><br />
<strong>Concordia</strong> participating dentist.<br />
When you receive care from a<br />
participating dentist, there are no claim<br />
forms to fill out. Participating dentists<br />
accept <strong>United</strong> <strong>Concordia</strong>’s allowance as<br />
payment in full for covered services. You<br />
will pay only the deductible and coinsurance<br />
amounts that are part of your<br />
program design and any amounts<br />
exceeding the annual program maximum.<br />
If you receive care from a nonparticipating<br />
dentist, the dentist can bill<br />
you for the difference between <strong>United</strong><br />
<strong>Concordia</strong>’s allowance and the dentist’s<br />
charge. You may also have to submit<br />
claim forms to united <strong>Concordia</strong> to<br />
receive reimbursement for services<br />
provided by a non-participating provider.<br />

Frequently Asked Questions<br />
My dentist doesn’t participate with <strong>United</strong> <strong>Concordia</strong>; can I still see<br />
him/her for care?<br />
You can receive care from any licensed dentist anywhere in the <strong>United</strong> States.<br />
If you choose to see a non-participating dentist, you will be responsible for the<br />
coinsurance amount that is part of your program design, as well as any<br />
charges over and above <strong>United</strong> <strong>Concordia</strong>’s allowance for covered services.<br />
You may also have to fill out claim forms and accept payment from <strong>United</strong><br />
<strong>Concordia</strong>.<br />
How will my dentist know I’m a member of <strong>Concordia</strong> <strong>Flex</strong>?<br />
Once you have enrolled in the <strong>Concordia</strong> <strong>Flex</strong> dental plan, you can print out an<br />
identification card from our website using My Dental Benefits feature. In<br />
addition, you or your dentist can contact our customer service center to confirm<br />
coverage.<br />
Do I have to fill out a claim form after each visit?<br />
If you receive care from a <strong>United</strong> <strong>Concordia</strong> participating dentist, there are no<br />
claim forms for you to complete. You may have to submit claim forms to <strong>United</strong><br />
<strong>Concordia</strong> to receive reimbursement for services rendered by a nonparticipating<br />
provider.<br />
Does <strong>United</strong> <strong>Concordia</strong> require predetermination of benefits?<br />
While predeterminations are not required, you should consider asking your<br />
dentist to provide a predetermination, or pre-treatment estimate, before<br />
undergoing any type of extensive treatment, That way, you’ll know if the service<br />
is covered an exactly what your financial responsibility will be.<br />
How do I sign up for <strong>Concordia</strong> <strong>Flex</strong>?<br />
