Welcome to Concordia Flex, the fee
for service dental program with a
difference. Concordia Flex is offered to
you by the University of Virginia and
United Concordia, a dental insurer with
more than 31 years of experience.
United Concordia offers a complete
range of dental products and delivery
models through contracted providers to
6.5 million Americans. *
With Concordia Flex, you are free to
use any licensed dentist you wish, but
the plan’s value and convenience is
maximized when you use a United
Concordia participating dentist.
When you receive care from a
participating dentist, there are no claim
forms to fill out. Participating dentists
accept United Concordia’s allowance as
payment in full for covered services. You
will pay only the deductible and coinsurance
amounts that are part of your
program design and any amounts
exceeding the annual program maximum.
If you receive care from a nonparticipating
dentist, the dentist can bill
you for the difference between United
Concordia’s allowance and the dentist’s
charge. You may also have to submit
claim forms to united Concordia to
receive reimbursement for services
provided by a non-participating provider.

Should you choose to enroll, simply
complete the enrollment form application
and return the form to United Concordia
along with a check made payable to United
Concordia Companies, Inc. Base your
payment amount on the contract size

Flex Benefit Summary.

Frequently Asked Questions
My dentist doesn’t participate with United Concordia; can I still see
him/her for care?
You can receive care from any licensed dentist anywhere in the United States.
If you choose to see a non-participating dentist, you will be responsible for the
coinsurance amount that is part of your program design, as well as any
charges over and above United Concordia’s allowance for covered services.
You may also have to fill out claim forms and accept payment from United
Concordia.
How will my dentist know I’m a member of Concordia Flex?
Once you have enrolled in the Concordia Flex dental plan, you can print out an
identification card from our website using My Dental Benefits feature. In
addition, you or your dentist can contact our customer service center to confirm
coverage.
Do I have to fill out a claim form after each visit?
If you receive care from a United Concordia participating dentist, there are no
claim forms for you to complete. You may have to submit claim forms to United
Concordia to receive reimbursement for services rendered by a nonparticipating
provider.
Does United Concordia require predetermination of benefits?
While predeterminations are not required, you should consider asking your
dentist to provide a predetermination, or pre-treatment estimate, before
undergoing any type of extensive treatment, That way, you’ll know if the service
is covered an exactly what your financial responsibility will be.
How do I sign up for Concordia Flex?
Simply complete the enrollment application and send the application along with
a check payable to United Concordia Companies, Inc. to:
