The Foundation is proud of the strong<br />
partnerships it has developed with nonprofit<br />
organizations in this region. To-date, 60 nonprofits<br />
have participated in Impact Foundation’s Institute for<br />
Innovation and Effectiveness (Institute) fundraising<br />
training, made possible by support from DMF.<br />
Additionally, 32 nonprofits have completed a formal<br />
strategic planning process funded through DMF<br />
grants to the Institute.<br />
In the Institute fundraising training, nonprofits are<br />
learning how to secure major individual donor gifts<br />
to support their mission over the long term. In 2005,<br />
DMF commissioned a study of North Dakota’s<br />
wealth transfer which revealed that from 2001-2055,<br />
North Dakota households will make total charitable<br />
contributions of $67.2 billion. Alarmingly, over this<br />
same time frame, North Dakotans are also projected<br />
to transfer $55 billion to the federal government in<br />
the form of estate taxes. With the help of Institute<br />
training, nonprofits in our region are now positioning<br />
themselves to assist donors and their advisors in<br />
making a greater societal impact through more tax<br />
efficient, effective charitable planning.<br />
The overall impact of the fundraising and strategic<br />
planning programs will take several years to fully<br />
measure, but organizations are already starting to<br />
build a solid base of committed donors and new<br />
major gifts that will sustain their mission for many<br />
years to come.<br />
In July 2007, Impact Foundation launched the<br />
GiveBack website, www.impactgiveback.org, a project<br />
funded by DMF to connect donors of time, talent and<br />
treasure to high-performing nonprofit organizations.<br />
Through the website, donors can learn about nonprofit<br />
organizations in our region, make online<br />
donations and explore volunteer opportunities that<br />
match their charitable interests. Our goal over the<br />
next year is to have 100% of DMF’s 300 nonprofit<br />
grant partners listed on the GiveBack website, as well<br />
as about 1,500 other regional nonprofit organizations.<br />
Significant marketing of the website is also planned<br />
for 2008, including individual campaigns implemented<br />
by each nonprofit partner.<br />
Another tool that leverages our nonprofit partners’<br />
ability to raise funds is the DMF Match Grant<br />
Program. In fiscal year 2007, DMF invested $1.1<br />
million in 23 of our nonprofit partners through the<br />
match initiative. The match program provides<br />
nonprofits with the leverage they need to “kick-start”<br />
their fundraising programs and further develop a<br />
committed base of donors.<br />
The sound adage of “teaching people to fish” is being<br />
successfully demonstrated by our nonprofit partners<br />
who will continue to provide valuable health services<br />
to our communities for the long term. We are proud<br />
to partner with these fine organizations!<br />

THE COURAGE OF AN “ANGEL”<br />
What happens when you’re 28 years old<br />
and you hear the diagnosis…“it’s cancer”? If you<br />
are Carly Nelson, you start dreaming about how<br />
you can help others who are experiencing a similar<br />
health challenge.<br />
At age 18, Carly began struggling with a series of<br />
health issues and was eventually diagnosed with<br />
lupus. Several years later, she faced a cancer diagnosis.<br />
That diagnosis transformed Carly, now 33 and a<br />
two-time cancer survivor, into a woman on a mission!<br />
Her mission is to build a retreat center, Angels of<br />
Courage Retreats, where people facing terminal<br />
illness, chronic disease or other life challenges can<br />
seek peace, guidance and support. Carly has been<br />
blessed with the help of another cancer survivor,<br />
Don Goering, who has donated 11 acres of beautiful<br />
land in the Cormorant Lakes area to be used as the<br />
building site for the center.<br />
A friend’s referral led Carly to DMF and the<br />
Impact Foundation. Last spring, DMF funded and<br />
coordinated the establishment of the organization’s<br />
501(c)(3) status and provided strategic planning and<br />
fundraising training consulting services to ensure<br />
a solid start for the organization. In addition,<br />
$100,000 in DMF match funds are leveraging their<br />
fundraising efforts.<br />
DMF’s support motivated Carly to devote more of<br />
her time to developing the retreat center. Carly’s<br />
employer, Microsoft, supported her efforts by<br />
enabling her to move to a four-day work week and<br />
dedicate the remaining day to Angels of Courage.<br />
At Microsoft’s worldwide Woman’s Leadership and<br />
Development Conference this past fall, Carly was one<br />
of the top three nominees for the Community Service<br />
award which recognizes a woman who has dedicated<br />
time and energy toward improving her community<br />
and has made a significant contribution to the quality<br />
of the community through such actions.<br />
A successful August 2007 fundraising event raised<br />
$67,000 for Angels of Courage Retreats. As a result,<br />
last fall they held their first retreat for adult cancer<br />
survivors at Fair Hills Resort in rural Detroit Lakes,<br />
MN. They are planning a second retreat in spring ‘08.<br />
They are currently seeking financial resources to hire<br />
a full-time coordinator to further develop the<br />
organization.<br />
Although Carly is a humble person who tends<br />
to deflect the spotlight away from herself, we believe<br />
she is a true “angel of courage” who is embarking<br />
on a tremendous journey that will help people better<br />
cope with the life-threatening health challenges they<br />
are facing. We are proud to be her partner!<br />

THE FULL CIRCLE OF HOPE<br />
In 2001, DMF received an emotion-filled,<br />
heartfelt thank-you note from grateful parent, Adair<br />
Grommesh, whose 11-month old son, Garrett, had<br />
just participated in the Hope, Inc. Magic Mobility<br />
Camp sponsored by DMF. “Thank you, DMF, for<br />
letting my son experience ‘walking’ for the first time”,<br />
the note said – with a picture of an adorable smiling<br />
little boy trialing a piece of mobility equipment.<br />
Garrett was born with spina bifida. The Camp<br />
provides assessment and mobility equipment trials<br />
for up to 50 children with physical disabilities each<br />
summer. Access to mobility equipment helps the<br />
children improve their circulation and muscle<br />
development, decrease leg contractures, improve<br />
digestion and heart and lung function and increase<br />
self-esteem and socialization skills.<br />
An amazing result of that initial experience is that,<br />
in 2006, the grateful mom became Hope, Inc.’s<br />
executive director! Adair tells “the rest of the story”:<br />
“In June 2001, our world was permanently<br />
changed for the positive! We were invited by<br />
Garrett’s therapist to attend a mobility camp in<br />
Moorhead. At this camp, a whole new world was<br />
opened to Garrett through the trial of various<br />
mobility equipment that enabled him to stand<br />
and move around on his own. If not for DMF’s<br />
support, this camp may not have existed.<br />
When invited to serve as Hope, Inc.’s executive<br />
director, I felt confident that my enthusiasm,<br />
passion and actual life experiences could enable<br />
Hope, Inc. to do even more. Children with<br />
disabilities need more than just one week to<br />
socialize with other children experiencing similar<br />
issues. So, with the help of others, we started a<br />
weekly sports and monthly recreational program<br />
to allow the kids to be a part of a team and<br />
experience the social interaction and recreation<br />
that able-bodied children enjoy.”<br />
Garrett is now seven years old and has experienced<br />
13 surgeries. He maneuvers his wheelchair like a<br />
Nascar driver on the track! His positive attitude and<br />
radiant smile brighten the days of everyone who<br />
comes in contact with him. The boundaries of his<br />
potential are not a part of his mind or body, but<br />
rather that of society.<br />
“Throughout the years, DMF has helped our<br />
organization with grants and trainings. But<br />
most importantly, the DMF staff and board<br />
believe in what our organization stands for<br />
and the potential it has to impact the lives of so<br />
many. On behalf of all of the children we serve,<br />

allowed our family to come ‘full circle’ and<br />

Children’s Mental Health<br />

uninsured ND children to existing free/low-cost<br />
health coverage programs including Caring for<br />
Children, Healthy Steps and Medicaid.<br />
– The number of ND children connected to health<br />
coverage programs increased by 6% statewide<br />
– The Healthy Steps Program enrolled an additional<br />
1,704 ND children (81% statewide increase)<br />
– Healthy Steps enrollment for American Indian<br />
children increased by 140%<br />
– Joint health coverage application and merged<br />
computerized eligibility system implemented<br />
– 1-877-KIDS NOW help line expanded to form the<br />
basis of an intake center sustained by the ND Dept.<br />

Dakota Communities Access Program<br />
Regional 2004-2007<br />
This program was supported by a $2.6 million grant<br />
awarded to DMF from the US Dept. of Health and<br />
Human Services. The purpose of the initiative was<br />
to improve access to and coordination of health care<br />
services for the uninsured and underinsured in<br />
central and eastern ND through placement of<br />
resource workers in select healthcare facilities.<br />
– 6,107 people received health coverage<br />

– 3,140 people were enrolled in low-income<br />

– $11.8 million reimbursed to ND healthcare facilities<br />

– The Urgent Care Dental Clinic continues to<br />
provide critical dental care services to uninsured/lowincome<br />

– 45 dentists and 8 dental specialists volunteer at<br />
the clinic
– Over 3,200 patients from 142 communities have<br />
accessed dental services over the past six years<br />
Diabetes Prevention<br />
Grand Forks, ND area 2004 - present<br />
Altru Health System’s diabetes prevention pilot seeks<br />
to prevent the onset of diabetes in persons at-risk for<br />
developing the disease.<br />
– $450,000 invested over the past three years<br />
– 1,446 persons screened for pre-diabetes<br />
– 150 persons enrolled in 11 diabetes prevention groups<br />
– Blood pressure goals (130/80 or less) have been<br />
achieved by all patients<br />
– Weight loss goal has been achieved by many<br />
(40%), but several need additional support<br />
– Blue Cross Blue Shield ND is covering group visits<br />
and <strong>medical</strong> nutrition therapy for participants; they<br />
also funded a $106,000 18-month weight<br />
management pilot that includes payment for<br />
exercise therapy for 25 participants with body<br />

$1.16 million 5-year public/private, rural/urban<br />
school nurse pilot project initiated to create an<br />
efficient, cost-effective, replicable, model school nurse<br />
program with proven outcomes.<br />
– Three school systems – Cass County, Fargo Catholic<br />
Schools Network and Grand Forks – participated in<br />
the pilot which involved a total of 40 schools<br />
– All three school systems sustained their expanded<br />
programs and, in some cases, further expanded their<br />
program after DMF’s grant expired in May 2007<br />
– The value of the school nurse program was proven<br />
through a formal data collection process; a five-year<br />
summary evaluation will be completed in spring 2008<br />
– 2007 outcome data:<br />

Nonprofit Organizational Effectiveness<br />

To support Impact Foundation’s Institute for<br />
Innovation and Effectiveness. The Institute provides<br />
organizational assessments, strategic planning and<br />
fundraising consulting services and volunteer<br />
management training resources for health-related<br />
nonprofits to dramatically improve their effectiveness<br />
and efficiency.<br />
– 60 nonprofits have completed fundraising training<br />
– 32 nonprofits have participated in the strategic<br />

Go Red North Dakota<br />
Women’s Heart Health<br />
Regional/Statewide 2006 - present<br />
Up to $1.25 million to implement a statewide<br />
women’s heart health initiative in partnership with<br />
the American Heart Association; includes a statewide<br />
education campaign, health professional education<br />
and targeted heart health interventions in Fargo,<br />
Jamestown and Bismarck.<br />
Year One Results:<br />
– 19 Go Red events held across state; nearly 4,500<br />
women joined the Go Red movement by registering<br />
at the events<br />
– Website: www.americanheart.org/gorednd; over<br />
600 women visited the Go Red check-up on the<br />
website to learn about their personal risk for<br />
heart disease<br />
– A marketing campaign was implemented including<br />
billboards and television/radio/newspapers/<br />

– Over 1,100 North Dakota physicians and<br />
practitioners received women’s heart health<br />

Initiative assists low-income population in obtaining<br />
free or reduced cost prescription drugs from<br />
pharmaceutical companies.<br />

– Over 3,500 persons assisted and 12,000<br />

Worksite Wellness Pilot<br />

DMF has made grants to Impact Foundation<br />
to manage a pilot program designed to create a<br />
model which will measurably improve the health<br />
of the region’s workforce. Impact Foundation<br />
contracts with Altru Health System in Grand Forks,<br />
ND to administer the program.<br />
– Pilot sites include Jamestown College, West Country<br />
Products and Anne Carlsen Center in Jamestown,<br />
ND, and Grand Forks Public Schools<br />
– 400 employees received health screenings and<br />
assessments; 60% of the employees screened<br />
have enough risk factors to put them in the<br />
“high risk” category<br />
– Evidence-based interventions for highest risk<br />
categories are being administered on-site by<br />
employer groups<br />
– Employees will be re-screened to assess the health<br />
improvement impact of the program

lack of an on-site radiologist.<br />

larger healthcare facility over<br />
an hour away in order to have<br />
the x-rays read.<br />

ENSURING THE FUTURE OF RURAL HEALTHCARE<br />
The scenario described on the opposite<br />
page depicts the experience of First Care Health<br />
Center in Park River, ND, a community of 1,400<br />
people in Walsh County about 50 miles from Grand<br />
Forks, ND. First Care Health Center was founded by<br />
the Presentation Sisters 55 years ago. In 2005, they<br />
launched a successful $7 million building and<br />
renovation project which was completed in October<br />
2007. They are positioning themselves to continue to<br />
provide access to high quality healthcare in their<br />
rural region.<br />
To keep pace with rapidly changing healthcare<br />
technology, installation of computed radiography<br />
equipment was critical. First Care Health Center<br />
applied for and received a $20,000 DMF match grant<br />
for the equipment. With additional funding from<br />
Blue Cross Blue Shield of North Dakota’s rural<br />
technology grant program, in July 2007 they<br />
purchased and installed the equipment. This<br />
equipment allows x-rays to be taken on-site,<br />
digitized, and sent via a T-line to an off-site<br />
radiologist. A diagnosis is returned within an<br />
hour, or within 15 minutes in an emergency.<br />
DMF’s match grant encouraged well over 100 local<br />
donors to support the project by matching their<br />
donations, dollar-for-dollar. Not only did this address<br />
the Health Center’s immediate technology needs, the<br />
fundraising effort has galvanized a solid base of<br />
donors who will help sustain the organization for the<br />
long term.<br />

patient outcomes, but decreases the need for<br />

the cost of film, postage and staff time to package and<br />

DMF Donor Advised Funds<br />
Donor Advised Funds allow the donor to<br />
establish a fund or account at the Foundation<br />
and recommend grants from their fund<br />
to qualified charitable organizations. The<br />
recommendations are non-binding; however,<br />
the Foundation gives each recommendation<br />
full consideration, provided that the<br />
suggested recipient is eligible to receive<br />
such a grant. The Foundation oversees<br />
administration of the fund, allowing the<br />
donor to focus on designing and implementing<br />
a charitable strategy that will produce a<br />
more substantial societal impact. A Donor<br />
Advised Fund can be established with a taxdeductible,<br />
irrevocable contribution of<br />
$10,000 or more. At this time, it is the<br />
Foundation’s practice not to charge expenses<br />
to administer these types of funds other than<br />
the direct costs of external investment<br />
management services.<br />

Scholarship Funds are created by individuals,<br />
families, schools, and other organizations to<br />
provide financial assistance to qualified<br />
students in pursuit of higher education in<br />
health-related fields. These funds are often<br />
established in memory or in honor of an<br />
individual to commemorate that person’s<br />
life, achievements, or dedication to a<br />
particular cause. The Foundation generally<br />
works with institutions of higher education<br />
to administer scholarship grant funds. The<br />
donor may select institutions to administer<br />
the scholarship grant funds as well as<br />
develop criteria for the scholarship award.<br />
DMF does not charge a fee to manage<br />
these funds.<br />

The Mack V. Traynor, M.D. Scholarship Fund<br />
was established by Dr. Traynor’s children to<br />
commemorate his lifelong commitment to<br />
delivering high quality healthcare services to<br />

students at the University of North Dakota.<br />

Matching Gift Program<br />
In August, 2005, DMF’s board established<br />
the Helping Hand Matching Gift Program<br />
to encourage members and employees to<br />
partner with DMF in financially supporting<br />
important health-related programs in our<br />
region. Through the Helping Hand Matching<br />
Gift Program, DMF members and employees<br />
may contribute up to $5,000 to DMF and<br />
recommend that their gift, plus a DMF<br />
match, be granted to up to four approved<br />
grant recipients and/or named funds during<br />
the fiscal year (Oct. 1 – Sept. 30). All funds<br />
awarded through the Helping Hand program<br />
become grants that the Foundation actively<br />
manages and monitors to ensure desired<br />

ANNUAL INVESTMENT MARKET REPORT<br />
The Foundation’s cash and investment portfolio<br />
increased its overall market value during the last<br />
twelve months to $118,950,631 from $110,654,271<br />
even with expenditures totaling $5,067,362. The<br />
Foundation’s investment strategy has resulted in a<br />
10.7% return for the year, up from the prior year’s<br />
7.6%. On recommendations by our financial advisors,<br />
the Foundation has implemented a Board-approved<br />
target asset allocation during 2007 that reflects<br />
a more long-term focus and diversification<br />
commensurate with other similar <strong>foundation</strong>s<br />
and public institutional investors. The new target<br />
allocation strategy of 55% equities, 35% fixed<br />
income, and 10% alternative investments will allow<br />
the portfolio to grow over the long-term taking<br />
advantage of future market conditions to improve<br />
the portfolio returns at acceptable risk levels. The<br />
Foundation continues with a manager-of-managers<br />
approach to the management of its investment<br />
portfolio, as recommended by our financial advisors.<br />
This strategy provides the DMF portfolio with a<br />
broad diversification of multiple investment<br />
managers, styles, and securities.<br />
