CONDITIONS OF HIRE<br />

ASTHMA FOUNDATION WA – TRAINING FACILITY<br />

BEHAVIOUR OF PATRONS<br />

The hirer is responsible for the behaviour of all patrons using the booked rooms.<br />

Antisocial behaviour is not acceptable and patrons displaying antisocial behaviour must vacate<br />

the facility when requested.<br />

The behaviour of children will be the responsibility of the parents or carers concerned and must<br />

be supervised at all times.<br />

BINS<br />

Bins are provided and are to be used with no exceptions. Please ensure waste that is considered<br />

large scale is deposited into the bins located in the basement at the conclusion of the <strong>booking</strong>.<br />

BOOKINGS<br />

All <strong>booking</strong>s must be made in writing, excluding one-off casual <strong>booking</strong>s. The Account Contact<br />

Person must make all <strong>booking</strong>s, amendments and cancellations.<br />

Annual <strong>booking</strong>s are periodically reviewed and are NOT valid until <strong>booking</strong> confirmation has<br />

been sent in writing from Asthma Foundation WA.<br />

For functions/events a <strong>booking</strong> request <strong>form</strong> must be submitted, including details of set up and<br />

clean up time. Charges will apply for room hire, set up time and clean up time. Bookings are<br />

NOT valid until a confirmation has been sent in writing from Asthma Foundation WA.<br />

BONDS & TARIFFS<br />

A damage bond will apply to <strong>booking</strong>s scheduled outside of Asthma Foundation WA operational<br />

hours.<br />

A tariff will also apply to <strong>booking</strong>s scheduled outside of Asthma Foundation WA operational<br />

hours to cover staffing costs.<br />

Bonds will be refunded by cheque, made out to the name and address on the <strong>booking</strong> <strong>form</strong>,<br />

subject to compliance with conditions.<br />

BREAKDOWNS<br />

In the event of a breakdown in services, utilities, equipment, etc. no responsibility will be<br />

accepted by Asthma Foundation WA but every care and precaution will be taken in this regard.<br />

CANCELLATION, REFUNDS AND TENTATIVE BOOKINGS<br />

Asthma Foundation WA reserves the right to cancel any <strong>booking</strong>s and any amounts paid will<br />

be fully refunded.<br />

Cancellation by the Hirer must be received in writing 28 days prior to the <strong>booking</strong> date for<br />

Major and Minor Events. Where a cancellation is less than 28 days prior to the event, a refund<br />

will not be given. All other <strong>booking</strong> cancellations or amendments must be made in writing with<br />

a minimum of 24 hours notice prior to the <strong>booking</strong>.<br />

Bookings are not secure without confirmation in writing.


CLEANING<br />

Rooms and toilets are required to be left in a clean and tidy condition immediately after use,<br />

including the removal of all rubbish and personal belongings. Please note, if the Hirer requires<br />

time for cleaning the room after use, the Hirer must request this in the <strong>booking</strong> request <strong>form</strong><br />

and will be charged accordingly.<br />

CURFEW<br />

The curfew for Asthma Foundation WA is the regular closing hours unless prior arrangements<br />

have been made with management. Please ensure you have vacated the facility at the designated<br />

time as stated on your <strong>booking</strong> confirmation. Failure to do so may jeopardise future <strong>booking</strong><br />

requests, and will result in additional charges. Please adhere strictly to <strong>booking</strong> times as any<br />

usage outside the agreed <strong>booking</strong> time will incur a minimum of one hour room hire fee (specific<br />

to your <strong>booking</strong>), along with staff charges if applicable.<br />

All activities must end at the hire time designated by Asthma Foundation WA.<br />

DAMAGE<br />

The Hirer must in<strong>form</strong> Asthma Foundation WA Staff if any damage is discovered prior to<br />

usage. In the event of damage to the facility resulting from the function the Hirer will be held<br />

responsible and will be charged for any repair costs which will be determined by management.<br />

The driving of nails and screws etc. into any part of the building fixtures and fittings is forbidden.<br />

DECORATIONS<br />

and fittings is forbidden. All decorations must be removed after completion of function. All<br />

decorations must be approved by management prior to their placement.<br />

ELECTRICAL EQUIPMENT<br />

If a hirer provides any <strong>form</strong> of electrical equipment it is required to be tagged in accordance with<br />

Australian Standard AS/NZS 3760:2001 - In Service Safety Inspection and Testing of Electrical<br />

Equipment requires that all electrical equipment such as urns, kettles, stereo systems, extension<br />

cords and heaters be inspected, tested and tagged on a regular basis. This equipment is to be<br />

identified in the application.<br />

EQUIPMENT<br />

Tables and chairs are supplied for the duration of your <strong>booking</strong>. Please ensure these are returned<br />

to the correct area of storage. Chairs must be clean and stacked in groups of no more than 4.<br />

Tables must be cleaned, folded and stored in an upright position.<br />

FEES & CHARGES<br />

Customers will be quoted on current fees and charges, however charges are subject to change<br />

due to the adoption of new fees and charges.<br />

HIRED AREA<br />

People attending functions/<strong>booking</strong>s are to remain within the area hired. Consideration of<br />

others is appreciated.<br />

HIRER'S POSSESSIONS<br />

Articles and goods left during the hire of the centre's facilities are left at the hirer's/owner's<br />

own risk. Asthma Foundation WA accepts no responsibility for any items left on the premises.


LAW AND ORDER<br />

The Hirer of any facility shall comply with the provisions of the Health Act, Liquor Act, Police Act<br />

and Criminal Code, or any other Act in force.<br />

LIABILITY<br />

Hirer’s of Asthma Foundation WA Facilities have some cover for their public liability depending<br />

on the activities being undertaken. Regular hirers and groups/organisations and the like are<br />

encouraged to arrange their own public liability insurance.<br />

LIQUOR<br />

Prior written consent from Asthma Foundation WA must be obtained if alcohol is proposed to<br />

be consumed at the event or function. Alcohol on a "BYO basis" for guests to an event will not<br />

be permitted. Permission from management must also be obtained when selling liquor prior to<br />

applying for an Occasional License<br />

Occasional License: Where a charge for entry or payment for alcohol is involved an Occasional<br />

License must be obtained from the Liquor Licensing Division, Hyatt Centre, 1st Floor, 87 Adelaide<br />

Terrace, Perth, fourteen (14) days prior to the event. A copy of this Licence must be forwarded to<br />

Asthma Foundation WA before the event and must be shown on inspection by any authorised<br />

person within the licensed premises.<br />

Asthma Foundation WA are not required to hold or obtain liquor licenses for events or functions<br />

which are not run or organised by Asthma Foundation WA.<br />

NOISE<br />

The Hirer must ensure that no undue noise whether in the centre or by leaving the venue is<br />

created by patrons. Any complaints received from adjacent residents could jeopardise future<br />

applications. If noise is excessive, it may result in the <strong>booking</strong> being shut down and no refund<br />

for the remainder of the <strong>booking</strong> fee given.<br />

Hirers must consider other centre users in regard to noise. If noise is deemed as excessive the<br />

Hirer must rectify the problem immediately. Please be aware that the partitions are not sound<br />

proof and you may be booked near a group who will be playing music or making noise.<br />

PAYMENTS<br />

An invoice is issued upon confirmation of the <strong>booking</strong>. The Hirer must finalise their account<br />

prior to the event (This includes bond payment).<br />

REFUSAL<br />

Asthma Foundation WA reserves the right to refuse to let the facility or any portion thereof<br />

without assigning any reason for refusal.<br />

SEVERAL APPLICATIONS<br />

In the event of two (2) or more applications being received for the hire of any portion of the<br />

facility at one time and date, Asthma Foundation WA may without considering priority of<br />

application, determine to which applicant the <strong>booking</strong> shall be granted.<br />

SMOKING<br />

Asthma Foundation WA adopted policy prohibits smoking inside any Asthma Foundation WA<br />

owned facility or within 50 metres of Asthma Foundation WA owned facilities.
