For <strong>the</strong> purposes <strong>of</strong> <strong>the</strong>se guidelines, CAP in <strong>the</strong> <strong>community</strong><br />
has been defined as:<br />
c Symptoms <strong>of</strong> an acute lower respiratory tract illness (cough<br />
and at least one o<strong>the</strong>r lower respiratory tract symptom).<br />
c New focal chest signs on examination.<br />
c At least one systemic feature (ei<strong>the</strong>r a symptom complex <strong>of</strong><br />
sweating, fevers, shivers, aches and pains and/or temperature<br />
<strong>of</strong> 38uC or more).<br />
c No o<strong>the</strong>r explanation <strong>for</strong> <strong>the</strong> illness, which is treated as CAP<br />
with antibiotics.<br />
1.3.1.2 Definition <strong>of</strong> CAP in patients admitted to hospital (when a<br />
chest radiograph is available)<br />
Studies <strong>of</strong> CAP from different countries have used very different<br />
definitions and inclusion criteria; 3–5 most have required a<br />

combination <strong>of</strong> symptoms, signs and radiological features. The<br />
BTS study <strong>of</strong> CAP used a definition which included: an acute<br />
illness with radiographic shadowing which was at least<br />
segmental or present in more than one lobe and was not<br />
known to be previously present or due to o<strong>the</strong>r causes. 6 Like<br />
most studies, cases were excluded if pneumonia occurred distal<br />
to a known carcinoma or <strong>for</strong>eign body.<br />
For <strong>the</strong> purposes <strong>of</strong> <strong>the</strong>se guidelines, CAP in hospital has been<br />
defined as:<br />
c Symptoms and signs consistent with an acute lower<br />
respiratory tract infection associated with new radiographic<br />
shadowing <strong>for</strong> which <strong>the</strong>re is no o<strong>the</strong>r explanation (eg, not<br />
pulmonary oedema or infarction).<br />
c The illness is <strong>the</strong> primary reason <strong>for</strong> hospital admission and<br />
is managed as pneumonia.<br />
