A CERTIFICATE OF INSURANCE IS MANDATORY FOR<br />
ALL WOMEN TODAY EXHIBITORS<br />
The Certificate of Liability insurance requirements are detailed on your<br />
Women Today contract Official Terms & Conditions under item #11.<br />
Please complete the following three steps:<br />

Provide your insurance company with the paragraph in the box below:<br />
Exhibitor agrees to submit to the <strong>Ventura</strong> <strong>County</strong> <strong>Star</strong> prior to Women Today and no<br />
later than Friday, November 2, 2012 a certificate of insurance evidencing its Workers’<br />
Compensation insurance coverage with statutory limits and its general liability<br />
insurance coverage with an occurrence form and combined single limit of not less<br />
than $1,000,000 per occurrence. Such certificate must name Scripps Media Inc.,<br />
<strong>Ventura</strong> <strong>County</strong> <strong>Star</strong>, <strong>Ventura</strong> <strong>County</strong>, the State of California, the Fairgrounds, Co-<br />
Sponsors, and their directors, officers, employees, management and agents named as<br />
additionally insured under general liability from November 8, 2012 to November 10,<br />
2012. These dates include Exhibitor move-in, general public admittance and Exhibitor<br />
move-out days.<br />
