FOREWORD<br />

SAMOAN PROFILE<br />

The <strong>Diversicare</strong> Samoan Seniors’ Project 2003-<br />

2004 was a successful attempt at engaging a<br />

positive, helping and reciprocal relationship with the<br />

Samoan people in <strong>Greater</strong> Brisbane North and South,<br />

Caboolture, Redcliffe, Logan and West Moreton<br />

regions. The Seniors project process is an innovative<br />

and effective way to promote information about Home<br />

and Community Care, health and wellbeing programs,<br />

enhancing social capital.<br />

This Directory is an initiative of <strong>Diversicare</strong>, the<br />

Community Care Division of Ethnic Communities<br />

Council of Queensland and is the result of an initiative<br />

to profile the Samoan Community. Our thanks go to<br />

Ms Tusi Luafutu and Mr Jeff Ruepena, Samoan Seniors<br />

Project Officers from 2003-2004 and to the Respite<br />

workers: Leah Iumai, Mareta Vasa–Gasu, Sisifo<br />

Faraimo, Elisapete Malielegaoi.<br />

The purpose of the Directory is to:<br />

1. To provide information about the Home and<br />

Community Care (HACC) program.<br />

2. To assist Home and Community Care agencies/<br />

other service agencies to achieve their goals in<br />

meeting the needs of people from culturally and<br />

linguistically diverse backgrounds (CALDB).<br />

3. Provide information about networks, formal and<br />

informal in the Samoan communities.<br />

4. Assist and facilitate generic community services’<br />

access to Samoan appropriate resources assistance.<br />

5. Assist the Samoan communities in identifying and<br />

utilizing culturally appropriate assistance in the<br />

health, ageing education, employment and welfare<br />

WHAT IS HOME AND<br />

COMMUNITY CARE (HACC)<br />

PROGRAM?<br />

Do you need help but want to<br />

stay in your home?<br />

Please read on and we will<br />

explain how you can be helped?<br />

Most people want to live in<br />

their home, but some frail older<br />

people and people with a<br />

disability can find this difficult<br />

without support. Help can be<br />

provided by carers (family and<br />

friends) and by HACC and<br />

other services.<br />

HACC can provide basic<br />

maintenance and support<br />

services so that frail older<br />

people and people with<br />

disability can continue to live in<br />

their community. These services<br />

provide eligible people with<br />

help at home, support in getting<br />

out into the community, and a<br />

break for carers.<br />

HACC services are offered in<br />

your home or local community<br />

and may be provided by HACC<br />

agency, community health<br />

centre or local council.<br />

WHO IS ELIGIBLE FOR A<br />

HACC SERVICE?<br />

HACC services are for people<br />

who need help to continue<br />

living at home in the community<br />

because:<br />

• They are frail older people<br />

who have difficulty with<br />

everyday tasks, such as<br />

showering, getting dressed<br />

or doing housework, or¸<br />

• They are a younger person<br />

with a moderate to severe<br />

disability, and they are at risk<br />

of having to go into a nursing<br />

home or aged people’s<br />

hostel.<br />

• HACC services are also<br />

available for people who are<br />

caring for a frail older person<br />

or someone with disability.<br />

relating to eligibility<br />

The HACC Program is a joint<br />

Commonwealth, State and<br />

Territory initiative. The Program<br />

funds services designed to help<br />

frail older people and people<br />

with a permanent disability to<br />

live independently at home.<br />

HACC Programs assists eligible<br />

clients who need basic maintenance<br />

and support services to<br />

continue living in the community<br />

with dignity.<br />

Many older people are<br />

supported by carers who are<br />

relatives, friends or neighbours.<br />

Caring for an older person<br />

or someone with a disability<br />

can be extremely demanding<br />

and the carer can become<br />

housebound or in need of a<br />

break.<br />

Carers also need advice and<br />

support to assist them in their<br />

role. HACC recognises the<br />

role of a carer therefore some<br />

services are available to carers.<br />

(ie. respite care).<br />

While the term "older and<br />

frail persons" is used in<br />

the definition of the target<br />

population for HACC services,<br />

it should be noted that<br />

eligibility for services is based<br />

on frailty related to a disability<br />

and that individuals do not<br />

qualify for HACC services<br />

solely on the grounds of<br />

advanced age.<br />

Thus, individuals over any<br />

particular age do not qualify for<br />

HACC services on the basis<br />

of their age alone, but because<br />

they have difficulties in carrying<br />

out tasks and need assistance<br />

due to a moderate, severe or<br />

profound disability.<br />

A person assessed as having<br />

a moderate, severe, profound<br />

or functional disability, which<br />

makes it difficult to perform<br />

the tasks of daily living<br />

without help, or supervision,<br />

may be eligible to receive<br />

services.<br />

Activities of daily living include<br />

dressing, preparing meals,<br />

house cleaning and maintenance,<br />

and using public<br />

transport. Some people may<br />

need only one service (for<br />

example nursing services or<br />

personal care), while others<br />

may need a combination of<br />

services.<br />

It should be noted that for<br />

younger people with a disability<br />

there may be specific purpose<br />

programs (for example, disability<br />

rehabilitation and employment<br />

support) which may be more<br />

appropriate than the more<br />

generic services offered by<br />

HACC providers.<br />

Both older people and younger<br />

people with disabilities should<br />

be assessed on the basis of<br />

need for basic maintenance and<br />

support services.<br />

To be assisted through<br />

the HACC Program people<br />

must:<br />

• Live in the community, and<br />

• Have difficulty in performing<br />

everyday tasks because of a<br />

disability, which may require<br />

admission into long term<br />

residential care (eg. Nursing<br />

home) earlier than required<br />

without assistance from<br />

HACC services.<br />

The HACC program is intended<br />

to provide basic maintenance<br />

and support services that are<br />

cost effective and meet the<br />

needs of individuals so they can<br />

remain in the community.<br />

Basic maintenance and support<br />

services are defined as those<br />

essential to a person’s well<br />

being, for example nutrition,<br />

community nursing, home help<br />

and personal care.<br />

Others, such as information<br />

and training courses, enable a<br />

greater level of understanding<br />

and expertise to be build up<br />

among service providers and<br />

consumers about the care of<br />

frail aged people and people<br />

with disabilities and the needs<br />

of their carers.<br />

HACC services are provided<br />

to people living in the<br />

community. This includes<br />

people living in:<br />

• Their homes;<br />

• Retirement villages, boarding<br />

houses or self care units;<br />

and<br />

• Temporary accommodation<br />

when they are transient or<br />

homeless.<br />

HACC services are not<br />

generally provided to<br />

residents of:<br />

• Aged care homes, if other<br />

government funding is<br />

provided for those services;<br />

or<br />

• Retirement villages, if<br />

residents contracts include<br />

the availability for those<br />

services; or<br />

• Hospitals.<br />

Assessment<br />

In order to assess your needs,<br />

a person from a HACC agency<br />

speaks to you about how you<br />

are coping with your daily living.<br />

The information you provide is<br />

written in the HACC Ongoing<br />

Needs Identification (ONI)<br />

form. This is used by the HACC<br />

Agency to decide whether you<br />

are eligible for a service and<br />

what type of service you need<br />

to assist you to remain living at<br />

home.<br />

WILL I HAVE TO PAY FOR A<br />

HACC SERVICE?<br />

Each HACC service has its<br />

own policy on fees, but most<br />

services ask for a small contribution,<br />

depending on your<br />

situation. Special consideration<br />

is given to people with limited<br />

finances.<br />

WHAT TYPES OF HACC<br />

SERVICES ARE AVAILABLE?<br />

HACC services provided in the<br />

community include:<br />

• Centre-based day respite<br />

Care, company and group<br />

activities in the centre, and<br />

may include short trips away<br />

from the centre.<br />

• Transport<br />

Helps people get out and<br />

about to a day care centre,<br />

shopping or appointments.<br />

• Social support<br />

Gives clients a hand by<br />

taking them shopping,<br />

banking or to appointments,<br />

or just providing company for<br />

a chat.<br />

HACC services provided at<br />

home may include:<br />

• Domestic assistance<br />

(home help/ home care)<br />

Household jobs like cleaning,<br />

clothes washing and ironing.<br />

• Personal care<br />

Includes help with bathing<br />

or showering, dressing, hair<br />

care and toileting.<br />

• Home maintenance<br />

Home maintenance is<br />

general repair and care of<br />

a client’s house or yard (for<br />

example, changing light<br />

bulbs, fixing door locks,<br />

mowing).<br />

• Home modification<br />

Services can install safety<br />

like arms, ramps and support<br />

rails in your home.<br />

• Community nursing<br />

Provided in your home by a<br />

qualified nurse (eg. Wound<br />

dressing, continence advise)<br />

Some HACC services can be<br />

provided either at a community<br />

centre or in your home:<br />

• Food services<br />

If you are unable to prepare<br />

meals, these services may<br />

include:<br />

- Providing meals at a<br />

community centre or day<br />

care centre<br />

- Helping with shopping for<br />

food, preparing and<br />

storing it<br />

- Delivering meals to your<br />

home<br />

• Allied health services<br />

For people having problems<br />

with their feet or speech,<br />

for example, or needing<br />

help with ongoing problems<br />

resulting from an accident<br />

or illness. Services include<br />

physiotherapy, podiatry,<br />

speech pathology, occupational<br />

therapy and advice<br />

from a dietician.<br />

• Respite care<br />

Gives carers a break from<br />

their caring role. A respite<br />

worker looks after the person<br />

needing care so the carer<br />

can enjoy other activities and<br />

interests. This service can be<br />

provided at a day centre or in<br />

your home.<br />

• Support services for<br />

carers<br />

As well as respite care, there<br />

are other support services<br />

for carers (see following<br />

section).<br />

INFORMATION FOR CARERS<br />

Are you caring for someone<br />

at home?<br />

Many frail older people, or<br />

people with disability, need<br />

support so they can live<br />

independently in their own<br />

home.<br />

They may be looked after by<br />

a carer who could be their<br />

spouse, partner, family member,<br />

neighbour or friend.<br />

You may be eligible for a HACC<br />

service if you are caring for<br />

someone who is a frail older<br />

person, or a person of any age<br />

who has a moderate to severe<br />

disability.<br />

Are you carer? A carer is<br />

someone who cares for people<br />

who have a disability, mental<br />

illness, chronic illness or are<br />

older and frail, and are unable<br />

to look after themselves.<br />

Caring for a frail older person<br />

or a person with a disability<br />

can be very hard, and carers<br />

may need a break, advice and<br />

support to help them cope with<br />

their caring role. If you are a<br />

carer, you may be eligible for<br />

support through HACC and<br />

other services such as:<br />

• Respite care<br />

This service gives carers a<br />

break while a respite worker<br />

looks after the person<br />

needing care. The person<br />

being cared for can receive<br />

respite care in their home<br />

or in a day respite centre. In<br />

most cases respite care is<br />

planned ahead, but it may be<br />

provided as an emergency<br />

service.<br />

• Support and information<br />

The service helps a carer<br />

find longer term respite such<br />

as temporary accommodation<br />

in a residential<br />

aged care facility or at the<br />

local hospital. The service<br />

can help find the most<br />

appropriate respite place<br />

for the person needing care.<br />

This allows carers to have a<br />

longer break from their caring<br />

responsibilities.<br />

Other support services for<br />

carers:<br />

Carers can also get support<br />

from other agencies such as<br />

Carers Queensland. This<br />

service offers carer support<br />

groups, care counselling,<br />

free resources, training and<br />

information. Contact the<br />

advice about services and other<br />

forms of help and support.<br />

RESPONSIBILITIES<br />

All HACC agencies are<br />

required by the government<br />

to meet National Service<br />

Standards in order to provide<br />

quality care. The Rights and<br />

Responsibilities described in<br />

this booklet are all related to<br />

those Service Standards.<br />

You Rights as a HACC<br />

Client<br />

As a person using HACC<br />

services, you have a number of<br />

rights. HACC service providers<br />

should recognise your right to:<br />

• Be treated with respect<br />

and courtesy<br />

Service providers must<br />

respect your ideas and are<br />

expected to show courtesy in<br />

their behaviour to you.<br />

• Be informed and consulted<br />

As a client you have the right<br />

to be informed about the<br />

service available to you, and<br />

about your rights as a client.<br />

• Be part of decisions made<br />

about your care<br />

You have the right to be<br />

in control of the care you<br />

receive by being part of<br />

planning and decisions made<br />

about the services provided<br />

to you.<br />

• Receive good quality<br />

services<br />

A service provider needs<br />

to inform you about what<br />

services it can and cannot<br />

provide. You have the right<br />

to receive a planned and<br />

reliable service.<br />

• Privacy and confidentiality,<br />

and to access all personal<br />

information kept about you<br />

by the HACC service<br />

You have the right to privacy<br />

and confidentiality and to<br />

access information about you<br />

on agency files.<br />

• Have another person of<br />

your choice support you<br />

and advocate (speak) on<br />

your behalf<br />

Everyone has basic rights<br />

as Australian citizens and<br />

these include expressing<br />

your views. It can be helpful<br />

to have family or friends<br />

to speak on your behalf,<br />

or agencies whose role<br />

is to advise people about<br />

their rights and responsibilities<br />

when receiving<br />

services. If you wish, one<br />

of those agencies can act<br />

on your behalf with services<br />

providers. Agencies like this<br />

are known as “advocacy”<br />

agencies, and people who<br />

act on your behalf, with your<br />

permission, are known as<br />

advocates.<br />

As someone using a HACC<br />

service, you have the right<br />

to involve an advocate of<br />

your choice to represent<br />

you at any time. The service<br />

must accept the advocate<br />

you choose. Your advocate<br />

can be anyone you choose<br />

– a spouse, partner, relative,<br />

neighbour, friend or someone<br />

from an advocacy service.<br />

There are independent<br />

agencies such as<br />

Queensland Aged and<br />

Disability Advocacy that<br />

can provide you with an<br />

advocate. Service providers<br />

should tell you about<br />

advocacy services when you<br />

receive a service.<br />

• Have your comments<br />

valued and to make a<br />

confidential complaint if<br />

you are not happy with the<br />

service you receive.<br />

As someone using HACC<br />

services, you have the right<br />

to give honest feedback<br />

about the service you are<br />

receiving without fear of<br />

losing the service, or having<br />

it reduced. HACC National<br />

Service Standards require<br />

services to have clear,<br />

written policies for handling<br />

complaints from service<br />

users. HACC agencies are<br />

required to make sure that<br />

their clients understand how<br />

those policies work.<br />

You have the right to have<br />

an advocate of your choice<br />

to support you in making<br />

a complaint. If you are not<br />

comfortable talking to your<br />

service provider, you can<br />

talk to an advocacy agency.<br />

Your Responsibilities as a<br />

HACC client<br />

While you have a number of<br />

Rights as a service user, you<br />

also have some Responsibilities<br />

to the people providing care to<br />

you.<br />

• Take responsibility for the<br />

results of any decisions<br />

which you make with staff<br />

and volunteers about your<br />

care.<br />

HACC services ask their<br />

clients to:<br />

• Treat staff and volunteers<br />

with respect and courtesy<br />

– for example, by letting<br />

them know as soon as<br />

possible if you cannot keep<br />

an appointment.<br />

• Provide a safe work<br />

environment for staff and<br />

volunteers, help them to<br />

provide you with services<br />

safely, and inform them if it<br />

is unsafe to visit. A HACC<br />

worker will use his/her<br />

commonsense to judge<br />

whether it is safe to visit your<br />

house at a particular time.<br />
