FOREWORD
SAMOAN PROFILE
The Diversicare Samoan Seniors’ Project 2003-
2004 was a successful attempt at engaging a
positive, helping and reciprocal relationship with the
Samoan people in Greater Brisbane North and South,
Caboolture, Redcliffe, Logan and West Moreton
regions. The Seniors project process is an innovative
and effective way to promote information about Home
and Community Care, health and wellbeing programs,
enhancing social capital.
This Directory is an initiative of Diversicare, the
Community Care Division of Ethnic Communities
Council of Queensland and is the result of an initiative
to profile the Samoan Community. Our thanks go to
Ms Tusi Luafutu and Mr Jeff Ruepena, Samoan Seniors
Project Officers from 2003-2004 and to the Respite
workers: Leah Iumai, Mareta Vasa–Gasu, Sisifo
Faraimo, Elisapete Malielegaoi.
The purpose of the Directory is to:
1. To provide information about the Home and
Community Care (HACC) program.
2. To assist Home and Community Care agencies/
other service agencies to achieve their goals in
meeting the needs of people from culturally and
linguistically diverse backgrounds (CALDB).
3. Provide information about networks, formal and
informal in the Samoan communities.
4. Assist and facilitate generic community services’
access to Samoan appropriate resources assistance.
page 3
5. Assist the Samoan communities in identifying and
utilizing culturally appropriate assistance in the
health, ageing education, employment and welfare
sectors.
We would like to acknowledge the assistance in
compiling this directory:
• Ms Lorraine Sing Cutler, Project Manager
• Mrs Tusiga Luafutu, Project Officer
• Mr Vic Tamati, Pacific Island Communities
Liaison Officer
• Ms Beata Drozdz, Administration Officer for
formatting
... and to all those who have provided comment. We
hope the Directory complies with the Samoan Matai
naming system and reflects the values of Fa’asamoa,
the Samoan way.
Disclaimer: Any errors in cultural protocols in this
Directory are unintended and we apologize in the first
instance.
Thank you
WHAT IS HOME AND
COMMUNITY CARE (HACC)
PROGRAM?
Do you need help but want to
stay in your home?
Please read on and we will
explain how you can be helped?
Most people want to live in
their home, but some frail older
people and people with a
disability can find this difficult
without support. Help can be
provided by carers (family and
friends) and by HACC and
other services.
HACC can provide basic
maintenance and support
services so that frail older
people and people with
disability can continue to live in
their community. These services
provide eligible people with
help at home, support in getting
out into the community, and a
break for carers.
HACC services are offered in
your home or local community
and may be provided by HACC
agency, community health
centre or local council.
WHO IS ELIGIBLE FOR A
HACC SERVICE?
HACC services are for people
who need help to continue
living at home in the community
because:
• They are frail older people
who have difficulty with
everyday tasks, such as
showering, getting dressed
or doing housework, or¸
• They are a younger person
with a moderate to severe
disability, and they are at risk
of having to go into a nursing
home or aged people’s
hostel.
• HACC services are also
available for people who are
caring for a frail older person
or someone with disability.
Additional information
relating to eligibility
The HACC Program is a joint
Commonwealth, State and
Territory initiative. The Program
funds services designed to help
frail older people and people
with a permanent disability to
live independently at home.
page 5
HACC Programs assists eligible
clients who need basic maintenance
and support services to
continue living in the community
with dignity.
Many older people are
supported by carers who are
relatives, friends or neighbours.
Caring for an older person
or someone with a disability
can be extremely demanding
and the carer can become
housebound or in need of a
break.
Carers also need advice and
support to assist them in their
role. HACC recognises the
role of a carer therefore some
services are available to carers.
(ie. respite care).
While the term "older and
frail persons" is used in
the definition of the target
population for HACC services,
it should be noted that
eligibility for services is based
on frailty related to a disability
and that individuals do not
qualify for HACC services
solely on the grounds of
advanced age.
Thus, individuals over any
particular age do not qualify for
HACC services on the basis
of their age alone, but because
they have difficulties in carrying
out tasks and need assistance
due to a moderate, severe or
profound disability.
A person assessed as having
a moderate, severe, profound
or functional disability, which
makes it difficult to perform
the tasks of daily living
without help, or supervision,
may be eligible to receive
services.
Activities of daily living include
dressing, preparing meals,
house cleaning and maintenance,
and using public
transport. Some people may
need only one service (for
example nursing services or
personal care), while others
may need a combination of
services.
It should be noted that for
younger people with a disability
there may be specific purpose
programs (for example, disability
rehabilitation and employment
page 6
support) which may be more
appropriate than the more
generic services offered by
HACC providers.
Both older people and younger
people with disabilities should
be assessed on the basis of
need for basic maintenance and
support services.
To be assisted through
the HACC Program people
must:
• Live in the community, and
• Have difficulty in performing
everyday tasks because of a
disability, which may require
admission into long term
residential care (eg. Nursing
home) earlier than required
without assistance from
HACC services.
The HACC program is intended
to provide basic maintenance
and support services that are
cost effective and meet the
needs of individuals so they can
remain in the community.
Basic maintenance and support
services are defined as those
essential to a person’s well
being, for example nutrition,
community nursing, home help
and personal care.
Others, such as information
and training courses, enable a
greater level of understanding
and expertise to be build up
among service providers and
consumers about the care of
frail aged people and people
with disabilities and the needs
of their carers.
HACC services are provided
to people living in the
community. This includes
people living in:
• Their homes;
• Retirement villages, boarding
houses or self care units;
and
• Temporary accommodation
when they are transient or
homeless.
page 7
HACC services are not
generally provided to
residents of:
• Aged care homes, if other
government funding is
provided for those services;
or
• Retirement villages, if
residents contracts include
the availability for those
services; or
• Hospitals.
HOW DO I GET HACC
SERVICE?
You, your doctor, a family
member or friend can contact:
• Commonwealth Carelink
Centre (1800 052 222) to
discuss your needs, or
• Your local HACC Area
Manager for referral to an
appropriate service:
Assessment
In order to assess your needs,
a person from a HACC agency
speaks to you about how you
are coping with your daily living.
The information you provide is
written in the HACC Ongoing
Needs Identification (ONI)
form. This is used by the HACC
Agency to decide whether you
are eligible for a service and
what type of service you need
to assist you to remain living at
home.
WILL I HAVE TO PAY FOR A
HACC SERVICE?
Each HACC service has its
own policy on fees, but most
services ask for a small contribution,
depending on your
situation. Special consideration
is given to people with limited
finances.
page 8
WHAT TYPES OF HACC
SERVICES ARE AVAILABLE?
HACC services provided in the
community include:
• Centre-based day respite
Care, company and group
activities in the centre, and
may include short trips away
from the centre.
• Transport
Helps people get out and
about to a day care centre,
shopping or appointments.
• Social support
Gives clients a hand by
taking them shopping,
banking or to appointments,
or just providing company for
a chat.
HACC services provided at
home may include:
• Domestic assistance
(home help/ home care)
Household jobs like cleaning,
clothes washing and ironing.
• Personal care
Includes help with bathing
or showering, dressing, hair
care and toileting.
• Home maintenance
Home maintenance is
general repair and care of
a client’s house or yard (for
example, changing light
bulbs, fixing door locks,
mowing).
• Home modification
Services can install safety
like arms, ramps and support
rails in your home.
• Community nursing
Provided in your home by a
qualified nurse (eg. Wound
dressing, continence advise)
Some HACC services can be
provided either at a community
centre or in your home:
• Food services
If you are unable to prepare
meals, these services may
include:
- Providing meals at a
community centre or day
care centre
- Helping with shopping for
food, preparing and
storing it
- Delivering meals to your
home
page 9
• Allied health services
For people having problems
with their feet or speech,
for example, or needing
help with ongoing problems
resulting from an accident
or illness. Services include
physiotherapy, podiatry,
speech pathology, occupational
therapy and advice
from a dietician.
• Respite care
Gives carers a break from
their caring role. A respite
worker looks after the person
needing care so the carer
can enjoy other activities and
interests. This service can be
provided at a day centre or in
your home.
• Support services for
carers
As well as respite care, there
are other support services
for carers (see following
section).
INFORMATION FOR CARERS
Are you caring for someone
at home?
Many frail older people, or
people with disability, need
support so they can live
independently in their own
home.
They may be looked after by
a carer who could be their
spouse, partner, family member,
neighbour or friend.
You may be eligible for a HACC
service if you are caring for
someone who is a frail older
person, or a person of any age
who has a moderate to severe
disability.
Are you carer? A carer is
someone who cares for people
who have a disability, mental
illness, chronic illness or are
older and frail, and are unable
to look after themselves.
Caring for a frail older person
or a person with a disability
can be very hard, and carers
may need a break, advice and
page 10
support to help them cope with
their caring role. If you are a
carer, you may be eligible for
support through HACC and
other services such as:
• Respite care
This service gives carers a
break while a respite worker
looks after the person
needing care. The person
being cared for can receive
respite care in their home
or in a day respite centre. In
most cases respite care is
planned ahead, but it may be
provided as an emergency
service.
• Support and information
The service helps a carer
find longer term respite such
as temporary accommodation
in a residential
aged care facility or at the
local hospital. The service
can help find the most
appropriate respite place
for the person needing care.
This allows carers to have a
longer break from their caring
responsibilities.
Other support services for
carers:
Carers can also get support
from other agencies such as
Carers Queensland. This
service offers carer support
RIGHTS AND
RESPONSIBILITIES
All HACC agencies are
required by the government
to meet National Service
Standards in order to provide
quality care. The Rights and
Responsibilities described in
this booklet are all related to
those Service Standards.
page 12
You Rights as a HACC
Client
As a person using HACC
services, you have a number of
rights. HACC service providers
should recognise your right to:
• Be treated with respect
and courtesy
Service providers must
respect your ideas and are
expected to show courtesy in
their behaviour to you.
• Be informed and consulted
As a client you have the right
to be informed about the
service available to you, and
about your rights as a client.
• Be part of decisions made
about your care
You have the right to be
in control of the care you
receive by being part of
planning and decisions made
about the services provided
to you.
• Receive good quality
services
A service provider needs
to inform you about what
services it can and cannot
provide. You have the right
to receive a planned and
reliable service.
• Privacy and confidentiality,
and to access all personal
information kept about you
by the HACC service
You have the right to privacy
and confidentiality and to
access information about you
on agency files.
• Have another person of
your choice support you
and advocate (speak) on
your behalf
Everyone has basic rights
as Australian citizens and
these include expressing
your views. It can be helpful
to have family or friends
to speak on your behalf,
or agencies whose role
is to advise people about
their rights and responsibilities
when receiving
services. If you wish, one
of those agencies can act
on your behalf with services
providers. Agencies like this
are known as “advocacy”
agencies, and people who
page 13
act on your behalf, with your
permission, are known as
advocates.
As someone using a HACC
service, you have the right
to involve an advocate of
your choice to represent
you at any time. The service
must accept the advocate
you choose. Your advocate
can be anyone you choose
– a spouse, partner, relative,
neighbour, friend or someone
from an advocacy service.
There are independent
agencies such as
Queensland Aged and
Disability Advocacy that
can provide you with an
advocate. Service providers
should tell you about
advocacy services when you
receive a service.
• Have your comments
valued and to make a
confidential complaint if
you are not happy with the
service you receive.
As someone using HACC
services, you have the right
to give honest feedback
about the service you are
receiving without fear of
losing the service, or having
it reduced. HACC National
Service Standards require
services to have clear,
written policies for handling
complaints from service
users. HACC agencies are
required to make sure that
their clients understand how
those policies work.
You have the right to have
an advocate of your choice
to support you in making
a complaint. If you are not
comfortable talking to your
service provider, you can
talk to an advocacy agency.
page 14
Your Responsibilities as a
HACC client
While you have a number of
Rights as a service user, you
also have some Responsibilities
to the people providing care to
you.
• Take responsibility for the
results of any decisions
which you make with staff
and volunteers about your
care.
HACC services ask their
clients to:
• Treat staff and volunteers
with respect and courtesy
– for example, by letting
them know as soon as
possible if you cannot keep
an appointment.
• Provide a safe work
environment for staff and
volunteers, help them to
provide you with services
safely, and inform them if it
is unsafe to visit. A HACC
worker will use his/her
commonsense to judge
whether it is safe to visit your
house at a particular time.
