WELCOME TO WIRRAL!<br />

Dear <strong>Foundation</strong> Trainee<br />

The foundation programme is a two year programme designed to help bridge the<br />

gap between medical school and specialty training. During these two years,<br />

foundation doctors will be regularly assessed against the competences in the<br />

<strong>Foundation</strong> Curriculum Programme.<br />

During your first year (F1) you will demonstrate progression from the competence<br />

required of a medical student to that required of a medical professional in the<br />

workplace with the aim of achieving full GMC registration at the end of the year.<br />

During your second year (F2) you will receive further exposure to a range of career<br />

placements across a broad spectrum of specialties, including accident & emergency,<br />

anaesthetics, obstetrics & gynaecology, ophthalmology, oncology, and GP practice.<br />

The <strong>Foundation</strong> team (see page 7) will be very happy to discuss any queries you<br />

may have.<br />

KEY STAFF<br />

DIRECTOR OF MEDICAL EDUCATION –<br />

DR DAVID GALVANI<br />

The Director of Medical Education is a Consultant in the Trust who is responsible for<br />

organising the Education throughout the Trust. The Director is based in the Education<br />

Centre and is there to help you deal with any problems which concern your career. Dr<br />

Galvani is also a consultant based in Haematology. Contact by email or through the<br />

Education Centre.<br />

POSTGRADUATE CLINICAL TUTORS –<br />

DR ROSEMARY MORGAN & MR SIMON PLATT<br />

Dr Morgan is responsible for the <strong>Foundation</strong> <strong>Teaching</strong> Programmes for F1 and F2 doctors.<br />

In addition she supports other grades of junior doctors. She is a Consultant in Medicine for<br />

the Elderly.<br />

Mr Platt, like Dr Morgan is also responsible for all grades of junior doctors. Mr Platt is a<br />

consultant in Orthopaedic Surgery.<br />

Appointments to see either Tutor can be made through Jane Roach, Lorraine Kendrick or<br />

Michelle Bennett.<br />

CLINICAL/EDUCATIONAL SUPERVISORS<br />

The Clinical Supervisor is the Consultant you will be working for in each attachment and is<br />

responsible for your teaching and supervision while you are there. They will also be your<br />

Educational Supervisor. They will agree your personal development plan, discuss career<br />

progression and make recommendations to the Faculty with regard to sign-off. Your<br />

Educational Supervisor may change with each attachment. It is your responsibility to<br />

make contact and arrange a first meeting with your Educational Supervisor when you<br />

start in each new attachment.<br />

MEDICAL EDUCATION & CENTRE MANAGER –<br />

MS KAREN GRIFFITHS<br />

DEPUTY MEDICAL EDUCATION & CENTRE MANAGER – MS CHERYL MALE.<br />

Karen and Cheryl manage the Education Centre at APH and also <strong>Wirral</strong> Postgraduate<br />

Medical Centre, Clatterbridge. They oversee Postgraduate Medical Education for the Trust.<br />

Their office is on the 2 nd floor of the Centre. They are available during the Centre’s opening<br />

hours, 5 days a week.<br />

COMPLIANCE MANAGER FOR DOCTORS IN TRAINING –<br />

MISS CHARMIAN WILTSHIRE<br />

Charmian will be overseeing your mandatory training during your time on the <strong>Foundation</strong><br />

Programme. She will provide you with all the necessary information to help you complete<br />

your mandatory training. You will receive some mandatory training topics through the<br />

induction week. There will be other training modules which you will need to complete via an<br />

online learning management system (the Dept of Health’s e-Learning for Health package).<br />

It is absolutely essential that you complete all mandatory training so that you are<br />

compliant with the Trust’s Mandatory Training Policy. Each year the Trust is<br />

assessed for compliance by the NHS Litigation Authority to ensure that all staff<br />

receive mandatory training. The Trust faces financial penalties if found to be noncompliant.<br />

aware it is 80 pages long! For this reason it cannot be printed off on a PC in the McArdle<br />

THE E-PORTFOLIO<br />

GETTING STARTED<br />

Check Personal Details are correct. These can be found in the PROFILE/PERSONAL<br />

DETAILS area of the e-porfolio.<br />

The e-portfolio is your record of your assessments, achievements and other evidence<br />

demonstrating completed outcomes as specified by the GMC. It is essential that you<br />

populate your e-portfolio with this evidence as it will be used to inform the end of year<br />

reports. It may also be used during interviews later for appointment to specialty training.<br />

YOUR INITIAL MEETING – for each placement<br />

It is your responsibility to arrange the appointment with your Educational Supervisor.<br />

First meeting with educational supervisor: aim to have this meeting within first 2 weeks of the<br />

post beginning. The Educational Supervisor will complete the Initial Meeting Form on the e-<br />

portfolio. This may happen while you are there or at a later date. Form for this meeting is<br />

found in FORMS/EDUCATIONAL SUPERVISION.<br />

PREPARATION FOR MEETING<br />

Declarations and Agreements e.g. Educational Agreement, Probity Forms<br />

Certificates e.g. ALS, ALERT etc these all must be signed on the e-portfolio by you and<br />

(where applicable) by your Educational Supervisor. All these pages may be found in<br />

PROFILE/DECLARATIONS & AGREEMENTS or PROFILE/CERTIFICATES.<br />

Your Educational Supervisor will ask you about your Personal Development Plan i.e. what<br />

you plan to get out of this post, learning opportunities. The PDP can be found in<br />

REFLECTION/PERSONAL DEVELOPMENT PLAN.<br />

MID-POINT MEETING – (OPTIONAL) BUT DESIREABLE<br />

This will take place at the middle of your post. Again it is your responsibility to arrange the<br />

appointment with your Educational Supervisor. The Mid-Point Review Form should be<br />

completed online by your Educational Supervisor. Form for this meeting is found in<br />

FORMS/EDUCATIONAL SUPERVISION.<br />

END OF PLACEMENT REVIEW<br />

At the end of the post you will need to make an appointment to see your Educational<br />

Supervisor again. At this meeting they will comment on your progress. They will complete<br />

the End of Placement Review on the e-portfolio and provide constructive feedback. The<br />

form for this meeting is found in FORMS/EDUCATIONAL SUPERVISION.<br />

ASSESSMENT TOOLS<br />

Multi –Source Feedback<br />

Team Assessment of Behaviour (TAB)<br />

This consists of the collated views from a range of co-workers. It will be mapped to a<br />

self-assessment tool with identical domains<br />

MSF should usually take place at least once a year.<br />

1. First do Self Assessment online. This can be found in FORMS/WORK-BASED<br />

ASSESSMENTS.<br />

2. Then you will need to get a minimum of 10 assessors to complete the online<br />

questionnaire (should ask at least 15 people to get good result).<br />

3. Can ask 2-8 medical staff more senior than F2 doctors, including your supervising<br />

Consultant as one of the assessors.<br />

4. Can ask 2-6 nursing staff (Band 5 or above)<br />

5. Can ask 2-4 other Allied Health Professionals e.g. Physio, OT, Pharmacist, Dietician.<br />

6. Can ask 2-4 other team members including ward clerks, secretaries & auxiliary staff<br />

7. Assessor is given code to enter on e-portfolio website to complete the form.<br />

8. As questionnaires come in a graph builds to show survey results against your Self<br />

Assessment.<br />

9. Postgraduate Clinical Tutors will look at this graph as a tool to assess your progress<br />

and will be discussed at your MID-POINT REVIEW (RITA) in February.<br />

IN EARLY DECEMBER, DO YOUR SELF ASSESSMENT ON THE<br />

E-PORTFOLIO<br />

DISCUSS WITH YOUR EDUCATIONAL SUPERVISOR WHO YOU<br />

WILL BE ASKING TO ASSESS YOU ON TAB<br />

LOG INTO THE E-PORTFOLIO. GO TO REQUEST AN<br />

ASSESSMENT ON THE MAIN PAGE. AIM TO DO THIS DURING<br />

DECEMBER<br />

GENERATE A NEW TICKET. YOU WILL NEED THE EMAIL<br />

ADDRESS OF EACH ASSESSOR, OR HAND THE<br />

INFORMATION FORM DIRECTLY TO THEM. THEY WILL<br />

RECEIVE IN THIS INFORMATION A UNIQUE LOG IN CODE.<br />

THEY WILL NEED THIS TO ACCESS THE MINI-PAT FORM<br />

ONLINE.<br />

CHECK THAT FORMS ARE BEING DONE BY GOING TO<br />

FORMS/WORK-BASED ASSESSMENTS. REMIND ASSESSORS IF<br />

THEY ARE NOT DOING THE FORMS.<br />

YOUR TAB SURVEY WILL BE DISCUSSED AT YOUR<br />

MID-POINT RITA IN FEBRUARY. IT IS THEREFORE WISE TO<br />

Direct observation of doctor/patient encounter<br />

Two tools can be used to assess these:<br />

• Mini-clinical evaluation exercise (mini-CEX)<br />

• Direct observation of procedural skills (DOPS)<br />

<strong>Foundation</strong> trainees are expected to complete 9 mini – CEX per year 2 of these<br />

assessments may be done as DOPS.<br />

Log Book (F1 year only)<br />

The GMC requires demonstration of competence in a series of procedures in order for a<br />

provisionally registered doctor with a licence to practise to be eligible for full registration.<br />

These will be recorded and signed off in a LOG BOOK, which is found in the e-portfolio. A<br />

completed log book is also required for successful completion of the <strong>Foundation</strong><br />

Programme.<br />

Developing the clinical teacher<br />

This is a form to aid the assessment of a foundation doctor’s skill in teaching and/or making<br />

a presentation.<br />

Case based discussion (CBD)<br />

This is a structured discussion of clinical cases managed by the foundation doctor.<br />

A minimum of 6 CBD’s should be completed with at least two undertaken in any four month<br />

period.<br />

TEACHING AND EDUCATION FOR FOUNDATION DOCTORS<br />

Most learning is achieved in the workplace from colleagues and from patients. In addition,<br />

there is a protected one hour a week tutorial organised for F1 doctors on Wednesday<br />

morning from 8.00 – 9.00am when you are expected to attend.<br />

You are also expected to attend other Educational opportunities taking place throughout the<br />

Trust including weekly Postgraduate Lecture on Tuesday and Thursday Lunchtimes (termtimes)<br />

and departmental teaching sessions.<br />

Also Wednesday lunchtimes you can attend the Medical Clinical Meetings.<br />

F2 teaching will take place on a Friday pm from 2.00 – 5.00pm unless in A+E dept where<br />

teaching will take place on a Wednesday afternoon.<br />

It is essential that junior doctors take part in giving written feedback so that we can all learn<br />

from each teaching session.<br />

The <strong>Foundation</strong> Curriculum is based upon the following headings:<br />

• Good Clinical Care<br />

• Maintaining Good Medical Practice<br />

• <strong>Teaching</strong> and Training<br />

• Appraising and Assessing<br />

• Relationships with Patients<br />

• Working with Colleagues<br />

• Probity and Health<br />

Its aim is that all F1 doctors will be able to recognise and manage an acutely unwell patient<br />

and will be able to demonstrate core clinical and procedural skills.<br />

The curriculum is essentially the same for F1 and F2 doctors, but with a different emphasis<br />

reflecting different levels of experience.<br />

For F1 doctors it is essentially about gaining experience of managing acutely unwell<br />

patients and learning from them, recognizing your own contribution and professionalism.<br />

We think that the F2 doctors, only a year ahead will have much to contribute to your teaching<br />

on the wards.<br />

PROTECTED TEACHING<br />

Please refer to the Trust’s Bleep Policy for more information on protected teaching. All staff<br />

throughout the Trust should be aware of this policy.<br />

Only trainees who are on-call or carrying the red bleep are allowed to keep the bleep with<br />

them.<br />

All other trainees leave the bleeps with the <strong>Foundation</strong> Team.<br />

CAREERS ADVICE AND PLANNING<br />

As you will be aware, the <strong>Foundation</strong> Programme emphasises the need for good Career<br />

Advice for <strong>Foundation</strong> Doctors. Our Career Lead is Dr Rosemary Morgan, who will be happy<br />

to meet up with you. We also have a resource file with useful contacts for each specialty so<br />

that we can introduce you to a key person within a specialty who would be willing to give you<br />

any guidance. There will be several sessions in the teaching programme focusing on<br />

careers. We have installed Sci 59 on four computers in the Library, which is a web based<br />

tool which may help with career choice.<br />

STUDY LEAVE<br />

As an F1 doctor you are not entitled to study leave.<br />

During your F2 year you are entitled to 30 study days in total – 20 days can be used for<br />

taster sessions and in-house teaching. 10 days further Study Leave will need to be applied<br />

for via the <strong>Foundation</strong> Team. You must complete a Study Leave request form and provide<br />

all information about the leave when requested.<br />

ANNUAL LEAVE<br />

All your annual leave will be managed and arranged through the Divisions you work in while<br />

with the Trust. Each Division has a Staffing Co-ordinator who you will meet during the<br />

induction.<br />

You will need to apply for annual leave from the department that you will be working with<br />

when you plan to be off. You will need to fill out an annual leave request form which must be<br />

signed by the consultant and the business manager. It is recommended that you take equal<br />

amounts of leave in each post.<br />

There is a section on the e-portfolio in the Profile section, where you should record this.<br />

SICKNESS ABSENCE<br />

All your sickness absence will be managed through the Divisions you work in while at this<br />

Trust. Again, you will need to contact your divisional Coordinator as soon as you know that<br />

you will not be fit to attend for work.<br />

If you are off work for over 4 weeks you will not be able to complete your <strong>Foundation</strong><br />

Year 1 or 2 and you will be asked to re-take the year.<br />

FOUNDATION YEAR 1<br />

INDUCTION<br />

As part of your introduction to the Trust you will required to complete a 6-day induction. This<br />

usually takes place in the week before the start of the academic year (last week of July).<br />

You must complete this induction to be able to work at the Trust as you will be provided with<br />

vital information throughout the week; some of which is mandatory.<br />

Lectures<br />

Week 1 and 2 will start with a day of lectures. These will cover topics which will be<br />

mandatory to your employment at the Trust as well as talks about using different services<br />

within the hospital.<br />

Training Activities<br />

On the following two days you will be split into groups to go to a variety of training activities,<br />

which will include:<br />

CPR & Defibrillation Training<br />

PCIS Training (Patient Care Information System)<br />

You will also be seen by Occupational Health for a Health Check, where your vaccinations<br />

will be checked.<br />

Ward Shadowing<br />

During the induction you will have several opportunities to join the F1 you are shadowing on<br />

the wards. Make the most of these times to get to know the staff in the area where you will<br />

be working.<br />

Clinical Skills<br />

You will also spend a day in the Clinical Skills Lab. The purpose of this is to assess your<br />

ability to carry out certain procedures. By the end of this day you should be competent in the<br />

procedures and will be signed off as such. The second week will start with another day of<br />

lectures. The Tuesday of that week will be your handover day. This means that you will<br />

spend the whole day shadowing the F1 that currently does the post you will be assigned to<br />

first.<br />

THE MENTORING SCHEME<br />

At the beginning of your first year you will be assigned a Mentor. The key role of the mentor<br />

is to ensure that the doctor in training is well supported by a senior member of the hospital<br />

staff who is not necessarily a consultant for whom the trainee will work. The trainee will<br />

keep the same mentor for the duration of his/her F1 or F2 post on the <strong>Wirral</strong>.<br />

ALERT COURSE<br />

During the F1 year you will need to complete the ALERT course. This is regardless of<br />

previous courses you may have attended, for example, Saves & Simulations. The ALERT<br />

will run on 2 dates during the year and you will be required to take leave from your post to<br />

attend. You will need to arrange leave from your post with the Medical Staffing Co-ordinator<br />

in the post that the course takes place. This course will be free to you during the F1 year. If<br />

If for any reason you cannot attend on the day that you have been assigned, you will need to<br />

sign up for an alternate date. This may be done by calling Che Wickham, the course coordinator<br />

ASSESSMENT<br />

F1 MID-POINT RITA (FEBRUARY)<br />

You will be invited to meet with one of the Postgraduate Clinical Tutors (PGCTs) during the<br />

month of February to review your progress of the year so far and to bring up any issues you<br />

may have. The PGCT will at this time look at your e-portfolio with you and will also receive a<br />

report on your attendance from the Postgraduate Office. Attendance at <strong>Foundation</strong><br />

<strong>Teaching</strong> should be over 70%. Attendance figures are recorded from your attendance at the<br />

following in the first half of the year:<br />

• F1 Induction Talks (August-September, approximately 12 meetings)<br />

• Safer Prescribing Course (August, 10 meetings)<br />

• Nephrology Course (September, 4 meetings)<br />

• F1 <strong>Teaching</strong> (August – January, approximately 22 meetings)<br />

• Postgraduate Clinical <strong>Teaching</strong> (August – January, approximately 23 meetings)<br />

• Medical Clinical <strong>Teaching</strong> (August – January, approximately 13 meetings)<br />

Please note that the quality of your e-portfolio and your attendance of teaching<br />

programmes at this point will be the two main areas considered when allocating you<br />

to your F2 posts.<br />

F1 FINAL RITA: ATTAINMENT OF F1 COMPETENCY (JUNE)<br />

The <strong>Foundation</strong> Schools ask us to consider the attainment of F1 competency for each<br />

trainee by the last week in June. It is your responsibility to arrange to meet with your<br />

Educational Supervisor (ES) in good time and review your portfolio. The ES can then<br />

recommend to the <strong>Foundation</strong> Faculty Group (FFG) that you be signed-off. You must make<br />

sure all of your assessments are complete in line with the check list. Attendance at<br />

<strong>Foundation</strong> <strong>Teaching</strong> should be over 70%. (It is essential you use your swipe card to<br />

record your attendance). We will, if necessary write to your Educational Supervisor if we<br />

have any concerns. At the end of the F1 year you will also be expected to apply for full<br />

registration with the GMC. This process will be done online by you, with a specific form<br />

(Certificate of Experience) produced during the application. This form must be given to the<br />

Postgraduate Office when you come for your June F1 RITA. A letter will be sent to you<br />

nearer the time to explain exactly what you will need to do to progress to F2.<br />

FOUNDATION YEAR 2<br />

INDUCTION<br />

Towards the end of your F1 year you will be required to attend an induction for your F2 year.<br />

This usually takes place towards the end of June. You will also need to attend one of 2<br />

Clinical Skills days. You will need to apply for leave at least 6 weeks in advance with the<br />

Medical Staffing Co-ordinator for the post you will be in at that time.<br />

If you are going to be in Accident & Emergency on your first triplet in F2 you will receive an<br />

induction from that department. This will take place towards the end of the F1 year as well.<br />

You will need to clear leave for this with your Staffing Co-ordinator.<br />

END OF YEAR ASSESSMENT<br />

You must make sure all of your assessments are complete by the end of June. F2 doctors<br />

must have completed an Audit during their F2 Year. Attendance at F2 <strong>Foundation</strong> <strong>Teaching</strong><br />

should be over 70%. (It is essential you use your swipe card to record your<br />

attendance) At your F2 Final RITA interview you will be assessed by a panel made up of<br />

one of our Postgraduate Tutors and an independent External Assessor. If everything is<br />

satisfactory they will sign your <strong>Foundation</strong> Achievement of Competency Document and<br />

provide you with a copy.<br />
