WELCOME TO WIRRAL!
Dear Foundation Trainee
The foundation programme is a two year programme designed to help bridge the
gap between medical school and specialty training. During these two years,
foundation doctors will be regularly assessed against the competences in the
Foundation Curriculum Programme.
During your first year (F1) you will demonstrate progression from the competence
required of a medical student to that required of a medical professional in the
workplace with the aim of achieving full GMC registration at the end of the year.
During your second year (F2) you will receive further exposure to a range of career
placements across a broad spectrum of specialties, including accident & emergency,
anaesthetics, obstetrics & gynaecology, ophthalmology, oncology, and GP practice.
The Foundation team (see page 7) will be very happy to discuss any queries you
may have.
Good luck in your two years with us.
Dr. Rosemary Morgan
Foundation Programme Director.
It can also be accessed at the following address:
http://www.foundationprogramme.nhs.uk/pages/home
There is an Adobe Acrobat Reader file (pdf) which can be opened and printed off (please be
aware it is 80 pages long! For this reason it cannot be printed off on a PC in the McArdle
Library).
The Assessment section (pages 50-56) gives full details of the assessment summary
outlined below.
THE E-PORTFOLIO
https://www.nhseportfolios.org/Anon/Login/Login.aspx
GETTING STARTED
Check Personal Details are correct. These can be found in the PROFILE/PERSONAL
DETAILS area of the e-porfolio.
The e-portfolio is your record of your assessments, achievements and other evidence
demonstrating completed outcomes as specified by the GMC. It is essential that you
populate your e-portfolio with this evidence as it will be used to inform the end of year
reports. It may also be used during interviews later for appointment to specialty training.
YOUR INITIAL MEETING – for each placement
It is your responsibility to arrange the appointment with your Educational Supervisor.
First meeting with educational supervisor: aim to have this meeting within first 2 weeks of the
post beginning. The Educational Supervisor will complete the Initial Meeting Form on the e-
portfolio. This may happen while you are there or at a later date. Form for this meeting is
found in FORMS/EDUCATIONAL SUPERVISION.
PREPARATION FOR MEETING
Declarations and Agreements e.g. Educational Agreement, Probity Forms
Certificates e.g. ALS, ALERT etc these all must be signed on the e-portfolio by you and
(where applicable) by your Educational Supervisor. All these pages may be found in
PROFILE/DECLARATIONS & AGREEMENTS or PROFILE/CERTIFICATES.
Your Educational Supervisor will ask you about your Personal Development Plan i.e. what
you plan to get out of this post, learning opportunities. The PDP can be found in
REFLECTION/PERSONAL DEVELOPMENT PLAN.
MID-POINT MEETING – (OPTIONAL) BUT DESIREABLE
This will take place at the middle of your post. Again it is your responsibility to arrange the
appointment with your Educational Supervisor. The Mid-Point Review Form should be
completed online by your Educational Supervisor. Form for this meeting is found in
FORMS/EDUCATIONAL SUPERVISION.
END OF PLACEMENT REVIEW
At the end of the post you will need to make an appointment to see your Educational
Supervisor again. At this meeting they will comment on your progress. They will complete
the End of Placement Review on the e-portfolio and provide constructive feedback. The
form for this meeting is found in FORMS/EDUCATIONAL SUPERVISION.
8
ASSESSMENT TOOLS
Multi –Source Feedback
Team Assessment of Behaviour (TAB)
This consists of the collated views from a range of co-workers. It will be mapped to a
self-assessment tool with identical domains
MSF should usually take place at least once a year.
1. First do Self Assessment online. This can be found in FORMS/WORK-BASED
ASSESSMENTS.
2. Then you will need to get a minimum of 10 assessors to complete the online
questionnaire (should ask at least 15 people to get good result).
3. Can ask 2-8 medical staff more senior than F2 doctors, including your supervising
Consultant as one of the assessors.
4. Can ask 2-6 nursing staff (Band 5 or above)
5. Can ask 2-4 other Allied Health Professionals e.g. Physio, OT, Pharmacist, Dietician.
6. Can ask 2-4 other team members including ward clerks, secretaries & auxiliary staff
7. Assessor is given code to enter on e-portfolio website to complete the form.
8. As questionnaires come in a graph builds to show survey results against your Self
Assessment.
9. Postgraduate Clinical Tutors will look at this graph as a tool to assess your progress
and will be discussed at your MID-POINT REVIEW (RITA) in February.
IN EARLY DECEMBER, DO YOUR SELF ASSESSMENT ON THE
E-PORTFOLIO
DISCUSS WITH YOUR EDUCATIONAL SUPERVISOR WHO YOU
WILL BE ASKING TO ASSESS YOU ON TAB
LOG INTO THE E-PORTFOLIO. GO TO REQUEST AN
ASSESSMENT ON THE MAIN PAGE. AIM TO DO THIS DURING
DECEMBER
GENERATE A NEW TICKET. YOU WILL NEED THE EMAIL
ADDRESS OF EACH ASSESSOR, OR HAND THE
INFORMATION FORM DIRECTLY TO THEM. THEY WILL
RECEIVE IN THIS INFORMATION A UNIQUE LOG IN CODE.
THEY WILL NEED THIS TO ACCESS THE MINI-PAT FORM
ONLINE.
CHECK THAT FORMS ARE BEING DONE BY GOING TO
FORMS/WORK-BASED ASSESSMENTS. REMIND ASSESSORS IF
THEY ARE NOT DOING THE FORMS.
YOUR TAB SURVEY WILL BE DISCUSSED AT YOUR
MID-POINT RITA IN FEBRUARY. IT IS THEREFORE WISE TO
HAVE A MINIMUM OF 10 FORMS COMPLETED BY THE END
OF JANUARY.
Direct observation of doctor/patient encounter
Two tools can be used to assess these:
• Mini-clinical evaluation exercise (mini-CEX)
• Direct observation of procedural skills (DOPS)
Foundation trainees are expected to complete 9 mini – CEX per year 2 of these
assessments may be done as DOPS.
Log Book (F1 year only)
The GMC requires demonstration of competence in a series of procedures in order for a
provisionally registered doctor with a licence to practise to be eligible for full registration.
These will be recorded and signed off in a LOG BOOK, which is found in the e-portfolio. A
completed log book is also required for successful completion of the Foundation
Programme.
TEACHING AND EDUCATION FOR FOUNDATION DOCTORS
Most learning is achieved in the workplace from colleagues and from patients. In addition,
there is a protected one hour a week tutorial organised for F1 doctors on Wednesday
morning from 8.00 – 9.00am when you are expected to attend.
You are also expected to attend other Educational opportunities taking place throughout the
Trust including weekly Postgraduate Lecture on Tuesday and Thursday Lunchtimes (termtimes)
and departmental teaching sessions.
Also Wednesday lunchtimes you can attend the Medical Clinical Meetings.
F2 teaching will take place on a Friday pm from 2.00 – 5.00pm unless in A+E dept where
teaching will take place on a Wednesday afternoon.
It is essential that junior doctors take part in giving written feedback so that we can all learn
from each teaching session.
The Foundation Curriculum is based upon the following headings:
• Good Clinical Care
• Maintaining Good Medical Practice
• Teaching and Training
• Appraising and Assessing
• Relationships with Patients
• Working with Colleagues
• Probity and Health
Its aim is that all F1 doctors will be able to recognise and manage an acutely unwell patient
and will be able to demonstrate core clinical and procedural skills.
The curriculum is essentially the same for F1 and F2 doctors, but with a different emphasis
reflecting different levels of experience.
For F1 doctors it is essentially about gaining experience of managing acutely unwell
patients and learning from them, recognizing your own contribution and professionalism.
We think that the F2 doctors, only a year ahead will have much to contribute to your teaching
on the wards.
11
PROTECTED TEACHING
Please refer to the Trust’s Bleep Policy for more information on protected teaching. All staff
throughout the Trust should be aware of this policy.
Only trainees who are on-call or carrying the red bleep are allowed to keep the bleep with
them.
All other trainees leave the bleeps with the Foundation Team.
YEAR 1
• F1 TEACHING PROGRAMME (ALL YEAR – EVERY WEDNESDAY MORNING FOR
1 HOUR)
• ALERT (if you do not already have the certificate or Saves & Simulations)
• COMMUNICATION SKILLS
YEAR 2
• F2 TEACHING PROGRAMME (ALL YEAR – EVERY FRIDAY AFTERNOON FOR 3
HOURS)
• SPECIALTY TEACHING – SURGICAL TEACHING, DME TEACHING, PAEDIATRIC
TEACHING, ORTHOPAEDIC TEACHING
• MEDICAL AUDITS (once a month, except August)
***
CAREERS ADVICE AND PLANNING
As you will be aware, the Foundation Programme emphasises the need for good Career
Advice for Foundation Doctors. Our Career Lead is Dr Rosemary Morgan, who will be happy
to meet up with you. We also have a resource file with useful contacts for each specialty so
that we can introduce you to a key person within a specialty who would be willing to give you
any guidance. There will be several sessions in the teaching programme focusing on
careers. We have installed Sci 59 on four computers in the Library, which is a web based
tool which may help with career choice.
***
12
STUDY LEAVE
As an F1 doctor you are not entitled to study leave.
During your F2 year you are entitled to 30 study days in total – 20 days can be used for
taster sessions and in-house teaching. 10 days further Study Leave will need to be applied
for via the Foundation Team. You must complete a Study Leave request form and provide
all information about the leave when requested.
***
ANNUAL LEAVE
All your annual leave will be managed and arranged through the Divisions you work in while
with the Trust. Each Division has a Staffing Co-ordinator who you will meet during the
induction.
You will need to apply for annual leave from the department that you will be working with
when you plan to be off. You will need to fill out an annual leave request form which must be
signed by the consultant and the business manager. It is recommended that you take equal
amounts of leave in each post.
There is a section on the e-portfolio in the Profile section, where you should record this.
SICKNESS ABSENCE
All your sickness absence will be managed through the Divisions you work in while at this
Trust. Again, you will need to contact your divisional Coordinator as soon as you know that
you will not be fit to attend for work.
If you are off work for over 4 weeks you will not be able to complete your Foundation
Year 1 or 2 and you will be asked to re-take the year.
NOTE: all absences should be recorded on your e-portfolio in the Profile section.
***
13
FOUNDATION YEAR 1
INDUCTION
As part of your introduction to the Trust you will required to complete a 6-day induction. This
usually takes place in the week before the start of the academic year (last week of July).
You must complete this induction to be able to work at the Trust as you will be provided with
vital information throughout the week; some of which is mandatory.
Lectures
Week 1 and 2 will start with a day of lectures. These will cover topics which will be
mandatory to your employment at the Trust as well as talks about using different services
within the hospital.
Training Activities
On the following two days you will be split into groups to go to a variety of training activities,
which will include:
CPR & Defibrillation Training
PCIS Training (Patient Care Information System)
You will also be seen by Occupational Health for a Health Check, where your vaccinations
will be checked.
Ward Shadowing
During the induction you will have several opportunities to join the F1 you are shadowing on
the wards. Make the most of these times to get to know the staff in the area where you will
be working.
Clinical Skills
You will also spend a day in the Clinical Skills Lab. The purpose of this is to assess your
ability to carry out certain procedures. By the end of this day you should be competent in the
procedures and will be signed off as such. The second week will start with another day of
lectures. The Tuesday of that week will be your handover day. This means that you will
spend the whole day shadowing the F1 that currently does the post you will be assigned to
first.
***
THE MENTORING SCHEME
At the beginning of your first year you will be assigned a Mentor. The key role of the mentor
is to ensure that the doctor in training is well supported by a senior member of the hospital
staff who is not necessarily a consultant for whom the trainee will work. The trainee will
keep the same mentor for the duration of his/her F1 or F2 post on the Wirral.
ALERT COURSE
During the F1 year you will need to complete the ALERT course. This is regardless of
previous courses you may have attended, for example, Saves & Simulations. The ALERT
will run on 2 dates during the year and you will be required to take leave from your post to
14
attend. You will need to arrange leave from your post with the Medical Staffing Co-ordinator
in the post that the course takes place. This course will be free to you during the F1 year. If
you do not complete it in F1 you may have to pay for it in F2 with a claim for Study Leave.
If for any reason you cannot attend on the day that you have been assigned, you will need to
sign up for an alternate date. This may be done by calling Che Wickham, the course coordinator
in x2717.
***
ASSESSMENT
F1 MID-POINT RITA (FEBRUARY)
You will be invited to meet with one of the Postgraduate Clinical Tutors (PGCTs) during the
month of February to review your progress of the year so far and to bring up any issues you
may have. The PGCT will at this time look at your e-portfolio with you and will also receive a
report on your attendance from the Postgraduate Office. Attendance at Foundation
Teaching should be over 70%. Attendance figures are recorded from your attendance at the
following in the first half of the year:
• F1 Induction Talks (August-September, approximately 12 meetings)
• Safer Prescribing Course (August, 10 meetings)
• Nephrology Course (September, 4 meetings)
• F1 Teaching (August – January, approximately 22 meetings)
• Postgraduate Clinical Teaching (August – January, approximately 23 meetings)
• Medical Clinical Teaching (August – January, approximately 13 meetings)
Please note that the quality of your e-portfolio and your attendance of teaching
programmes at this point will be the two main areas considered when allocating you
to your F2 posts.
F1 FINAL RITA: ATTAINMENT OF F1 COMPETENCY (JUNE)
The Foundation Schools ask us to consider the attainment of F1 competency for each
trainee by the last week in June. It is your responsibility to arrange to meet with your
Educational Supervisor (ES) in good time and review your portfolio. The ES can then
recommend to the Foundation Faculty Group (FFG) that you be signed-off. You must make
sure all of your assessments are complete in line with the check list. Attendance at
Foundation Teaching should be over 70%. (It is essential you use your swipe card to
record your attendance). We will, if necessary write to your Educational Supervisor if we
have any concerns. At the end of the F1 year you will also be expected to apply for full
registration with the GMC. This process will be done online by you, with a specific form
(Certificate of Experience) produced during the application. This form must be given to the
Postgraduate Office when you come for your June F1 RITA. A letter will be sent to you
nearer the time to explain exactly what you will need to do to progress to F2.
15
FOUNDATION YEAR 2
INDUCTION
Towards the end of your F1 year you will be required to attend an induction for your F2 year.
This usually takes place towards the end of June. You will also need to attend one of 2
Clinical Skills days. You will need to apply for leave at least 6 weeks in advance with the
Medical Staffing Co-ordinator for the post you will be in at that time.
If you are going to be in Accident & Emergency on your first triplet in F2 you will receive an
induction from that department. This will take place towards the end of the F1 year as well.
You will need to clear leave for this with your Staffing Co-ordinator.
***
END OF YEAR ASSESSMENT
You must make sure all of your assessments are complete by the end of June. F2 doctors
must have completed an Audit during their F2 Year. Attendance at F2 Foundation Teaching
should be over 70%. (It is essential you use your swipe card to record your
attendance) At your F2 Final RITA interview you will be assessed by a panel made up of
one of our Postgraduate Tutors and an independent External Assessor. If everything is
satisfactory they will sign your Foundation Achievement of Competency Document and
provide you with a copy.
IF YOU NEED HELP
If you do have concerns about your training, it is important that you contact either your
educational or clinical supervisor immediately. This should be done in a formal setting. If
you are still not happy after having spoken to the educational or clinical supervisor, please
contact one of the Postgraduate Clinical Tutors. You may also find it helpful to discuss
issues with your Mentor.
