Continuing Pharmacy Education. Initial terms <strong>of</strong> accreditation are usually <strong>for</strong> two<br />

years with a First Review in one year, fall 2012, and a Second Review in two<br />

years, fall 2013.<br />

Five applications <strong>for</strong> Initial <strong>Accreditation</strong> were reviewed and accredited:<br />

Austin Community College Pharmacy Technician Program; South University<br />

School <strong>of</strong> Pharmacy; Chapter Health System; PCMA; and Medication<br />

Management Center.<br />

One application <strong>for</strong> Initial <strong>Accreditation</strong> was not reviewed because it did not meet<br />

eligibility criteria: The American University <strong>of</strong> Athens-Greece.<br />

Submission <strong>of</strong> First Review by Applicants Initially Accredited in Fall 2010<br />

Four applicants submitted first reviews (next review date is included in<br />

parentheses), including:<br />

Cook County Health & Hospital System (fall 2012); King Faisal Specialist<br />

Hospital & Research Center (fall 2012); National Jewish Health (fall 2012); and<br />

Texas A&M Health Science Center Coastal Bend Health Education Center, (fall<br />

2012).<br />

Submission <strong>of</strong> Second Review by Applicants Initially Accredited in Fall<br />

2009<br />

Three applicants submitted second reviews (next review date is included in<br />

parentheses), including:<br />

Humana, Inc. (fall 2015); Sullivan University College <strong>of</strong> Pharmacy (fall 2015)<br />

The reviews <strong>of</strong> the following Second Review resulted in a shortened term<br />

(Progress <strong>Report</strong> fall 2012):<br />

NSF-DBA, LLC<br />

Consideration <strong>for</strong> Continued <strong>Accreditation</strong> on the Basis <strong>of</strong> Comprehensive<br />

Reviews<br />

The submitted Self-Assessment <strong>Report</strong> undergoes both a Commissioner review,<br />

an internal (ACPE staff) review, an external (Field reviewer) review, a review <strong>of</strong><br />

all records including correspondence, Activity Description Forms (ADFs), if any, a<br />

conference call with the provider and the reviewers, complaints and third party<br />

comments. The Comprehensive Review <strong>for</strong> Continued <strong>Accreditation</strong> provides the<br />

basis <strong>for</strong> the Board’s action to continue the provider’s term <strong>of</strong> accreditation. The<br />

standard term <strong>of</strong> accreditation is six years.<br />

Comprehensive Reviews from ten providers were considered and acted upon.<br />

The next Comprehensive Review <strong>for</strong> the following is fall 2017 and they include:<br />
