Dear Colleagues <strong>and</strong> Friends,<br />

The health status <strong>of</strong> <strong>the</strong> people <strong>of</strong> <strong>Malaysia</strong> has improved significantly since <strong>the</strong><br />

nation achieved its independence in 1957. Today, <strong>the</strong> country’s health system is<br />

one <strong>of</strong> <strong>the</strong> best in <strong>the</strong> South East Asia region. <strong>International</strong>ly recognised measures<br />

<strong>of</strong> <strong>the</strong> quality <strong>and</strong> efficiency <strong>of</strong> a health system show that infant mortality rates<br />

in <strong>Malaysia</strong> had fallen by 2005 to a level comparable with <strong>the</strong> USA <strong>and</strong> Western<br />

Europe, whilst life expectancy rates have risen to 74 years.<br />

Despite <strong>the</strong>se successes, which have been underpinned by a firm political<br />

commitment to universal healthcare coverage, <strong>the</strong> key to <strong>the</strong> effective <strong>and</strong> efficient<br />

delivery <strong>of</strong> <strong>Malaysia</strong>’s health infrastructure objectives in <strong>the</strong> future is to build its<br />

knowledge <strong>and</strong> underst<strong>and</strong>ing <strong>of</strong> <strong>the</strong> planning, design, finance, construction <strong>and</strong><br />

facility management process through greater exposure to <strong>the</strong> lessons learned from<br />

comparative international health systems, <strong>and</strong> <strong>the</strong> use <strong>and</strong> application <strong>of</strong> research<br />

<strong>and</strong> evidence.<br />

In a globalised <strong>and</strong> increasingly interdependent world, <strong>the</strong> continuous<br />

improvement <strong>of</strong> <strong>the</strong> quality <strong>of</strong> life <strong>and</strong> wellbeing <strong>of</strong> <strong>the</strong> citizens <strong>of</strong> <strong>Malaysia</strong> will be<br />

founded on <strong>the</strong> recognition that a healthy population is <strong>the</strong> foundation for social<br />

development <strong>and</strong> economic growth.<br />

Today’s rapid movement <strong>of</strong> individuals <strong>and</strong> populations living in ever larger,<br />

multi-cultural, urban environments, combined with <strong>the</strong> equally rapid evolution<br />

<strong>of</strong> <strong>the</strong> knowledge society means that <strong>the</strong> world we live in today is one with few<br />

boundaries. In <strong>the</strong> 21st Century, <strong>the</strong> health <strong>of</strong> nations is a global issue, <strong>and</strong><br />

<strong>Malaysia</strong>’s improving health status is dependent on recognition <strong>of</strong> its international<br />

co-existence.<br />

This international symposium <strong>and</strong> workshop organised by <strong>the</strong> <strong>Malaysia</strong>n<br />

Ministry <strong>of</strong> <strong>Health</strong> in partnership with <strong>the</strong> <strong>International</strong> <strong>Academy</strong> for <strong>Design</strong> &<br />

<strong>Health</strong> will explore ideas <strong>and</strong> opportunities to transfer knowledge, exchange new<br />

Introduction<br />

A ‘salutogenic approach’ to health infrastructure development embedded at <strong>the</strong> core<br />

<strong>of</strong> a preventative care strategy will help to change <strong>the</strong> focus from risk factors <strong>and</strong> <strong>the</strong><br />

treatment <strong>of</strong> disease to a more holistic underst<strong>and</strong>ing <strong>of</strong> a healthy society in <strong>Malaysia</strong><br />

In 1997, <strong>the</strong> World <strong>Health</strong> Organization<br />

identified that <strong>the</strong> health “arena”, including<br />

priority settings <strong>and</strong> frequently used spaces<br />

such as <strong>the</strong> workplace, schools, hospitals,<br />

correctional institutions, commercial <strong>of</strong>fices,<br />

public spaces within our towns <strong>and</strong> cities, <strong>and</strong><br />

indeed our homes, should be at <strong>the</strong> centre <strong>of</strong><br />

health promotion activities in <strong>the</strong> 21st century.<br />

Using <strong>the</strong> environment as a strategic tool is<br />

one <strong>of</strong> <strong>the</strong> most cost-effective <strong>and</strong> enduring<br />

approaches to improving public health, but<br />

it is one that requires new perspectives<br />

that consider wellness factors to encourage<br />

innovative design solutions.<br />

<strong>Design</strong> objectives for enhancing human<br />

health must facilitate an active lifestyle, enable<br />

<strong>the</strong> successful management <strong>of</strong> physical,<br />

psychological <strong>and</strong> emotional stress <strong>and</strong> support<br />

mental <strong>and</strong> cognitive processing <strong>of</strong> information<br />

by stimuli in a variety <strong>of</strong> designed environments.<br />

Central to this cause is <strong>the</strong> development <strong>of</strong><br />

a scientific research base, which illustrates<br />

<strong>and</strong> explores <strong>the</strong> relationship between<br />

human health <strong>and</strong> <strong>the</strong> environment, <strong>and</strong> even<br />

more vitally, creates a case for <strong>the</strong> rigorous<br />

application <strong>of</strong> this knowledge in pr<strong>of</strong>essional<br />

practice.<br />

Well-designed healthcare facilities around<br />

<strong>the</strong> world are making a critical contribution<br />

to <strong>the</strong> process <strong>of</strong> healing <strong>and</strong> rehabilitation.<br />

<strong>Malaysia</strong>’s Prince Court Hospital is a privately-owned five star facility<br />

<strong>Design</strong> excellence at <strong>the</strong> Prince Court Hospital in Kuala Lumpur<br />

However, a broader, more holistic perspective<br />

demonstrates that health processes are<br />

continuously operating in all types <strong>of</strong> built <strong>and</strong><br />

physical environments where individuals spend<br />

<strong>the</strong>ir daily lives.<br />

<strong>International</strong> research studies demonstrate<br />

how <strong>the</strong> quality <strong>of</strong> our public <strong>and</strong> private spaces<br />

is closely linked to levels <strong>of</strong> health, crime,<br />

poverty <strong>and</strong> our quality <strong>of</strong> life. The level <strong>of</strong><br />

design quality in <strong>the</strong> built environment is also<br />

critical to <strong>the</strong> sustainable development <strong>of</strong> our<br />

neighborhoods, towns <strong>and</strong> cities.<br />

Architects <strong>and</strong> developers have a special<br />

responsibility to ensure environmentallysupportive<br />

design principles are to <strong>the</strong> fore in<br />

addressing one <strong>of</strong> <strong>the</strong> keynote issues <strong>of</strong> our<br />

time within <strong>the</strong> local cultural context. Good<br />

design in our educational institutions also<br />

enhances our children’s emotional, intellectual<br />

<strong>and</strong> social development, laying <strong>the</strong> foundation<br />

for a well-balanced <strong>and</strong> productive future<br />

society.<br />

The <strong>International</strong> <strong>Academy</strong> for <strong>Design</strong><br />

<strong>and</strong> <strong>Health</strong> believes that human health<br />

is significantly related to <strong>the</strong> designed<br />

environment. Our mission is to spread<br />

awareness <strong>of</strong> this important message <strong>and</strong> its<br />

value as a foundation for improving human<br />

health, well-being <strong>and</strong> quality <strong>of</strong> life.<br />

Objectives <strong>of</strong> <strong>the</strong> symposium <strong>and</strong> workshop<br />

<strong>Design</strong> & <strong>Health</strong> <strong>Malaysia</strong> <strong>2010</strong> will explore global ‘salutogenic’ perspectives on <strong>the</strong><br />

planning, procurement, finance, design, construction <strong>and</strong> operation <strong>of</strong> health facilities<br />

within <strong>the</strong> local context <strong>of</strong> infrastructure development in <strong>the</strong> region. The <strong>Academy</strong> will<br />

provide <strong>the</strong> most recent knowledge by engaging some <strong>of</strong> <strong>the</strong> most highly qualified<br />

experts in <strong>the</strong> world from USA, Canada, Sc<strong>and</strong>inavia <strong>and</strong> <strong>the</strong> UK. The format <strong>of</strong> <strong>the</strong> two<br />

day event will include a two day symposium <strong>and</strong> one day workshop, developed with <strong>the</strong><br />

support <strong>of</strong> <strong>the</strong> Ministry <strong>of</strong> <strong>Health</strong>.<br />

Pr<strong>of</strong>essor Alan Dilani is a founder <strong>and</strong> general director <strong>of</strong> <strong>the</strong> <strong>International</strong> <strong>Academy</strong><br />

for <strong>Design</strong> <strong>and</strong> <strong>Health</strong> (IADH). He is co-founder <strong>of</strong> <strong>the</strong> journal World <strong>Health</strong> <strong>Design</strong>.<br />

Dr Dilani has been engaged worldwide in several universities in <strong>the</strong> field <strong>of</strong> <strong>Design</strong><br />

<strong>and</strong> <strong>Health</strong> developing psychosocially supportive design programs, both in medical<br />

<strong>and</strong> design institutions. He holds a Ph.D. in <strong>Health</strong> Facility <strong>Design</strong> from <strong>the</strong> Royal<br />

Institute <strong>of</strong> Technology, Stockholm <strong>and</strong> a Masters <strong>of</strong> Architecture in Environmental<br />

<strong>Design</strong> from <strong>the</strong> Polytechnic <strong>of</strong> Turin, Italy. His research at <strong>the</strong> Karolinska Institute,<br />

Medical University, developed a multidisciplinary research approach, leads to<br />

<strong>the</strong> new definition <strong>of</strong> design that not only fosters functional efficiency, but also<br />

improves health processes. He has designed all types <strong>of</strong> healthcare facilities <strong>and</strong><br />

<strong>Design</strong> <strong>and</strong> <strong>Health</strong>.<br />

Mike Nightingale<br />

Founder, Nightingale Associates<br />

For three <strong>and</strong> a half decades, Mike Nightingale has dedicated his career to<br />

elevating <strong>the</strong> design <strong>of</strong> healthcare buildings to new levels <strong>of</strong> excellence. His 36<br />

years <strong>of</strong> experience have seen him spearhead new design approaches, promote<br />

<strong>the</strong> importance <strong>of</strong> healthcare design <strong>and</strong> create guidance documents for <strong>the</strong> NHS.<br />

Following his qualification at <strong>the</strong> Oxford School <strong>of</strong> Architecture, Mike’s first job saw<br />

him working on Cirencester Community Hospital, for <strong>the</strong> Oxford Hospital Board,<br />

beginning a lifetime <strong>of</strong> work in healthcare design. After joining Percy Thomas as<br />

project architect for Colchester DGH, Mike pioneered several new design approaches<br />

<strong>and</strong> went on to lead a 40 person team creating a health masterplan for Oman,<br />

leading to <strong>the</strong> designing <strong>of</strong> three new hospitals including <strong>the</strong> Royal Hospital. In 1989<br />

Mike founded Nightingale Associates to enable him to push <strong>the</strong> design <strong>of</strong> health<br />

buildings to ever greater heights. In 18 years <strong>the</strong> practice has grown from two people<br />

based in Oxford, to 300 staff working across 12 <strong>of</strong>fices in <strong>the</strong> UK <strong>and</strong> South Africa on<br />

some <strong>of</strong> <strong>the</strong> biggest healthcare projects ever undertaken.<br />

Dr Ray Pentecost III, DrPH, FAIA, FACHA, LEED AP<br />

President, American Institute <strong>of</strong> Architects. <strong>Academy</strong><br />

Dr Ray Pentecost is an experienced healthcare pr<strong>of</strong>essional, a licensed architect<br />

<strong>and</strong> fellow in <strong>the</strong> American Institute <strong>of</strong> Architects. He is board certified in <strong>the</strong><br />

healthcare architecture specialty <strong>and</strong> a fellow in <strong>the</strong> American College <strong>of</strong><br />

<strong>Health</strong>care Architects. He is also a LEED Accredited Pr<strong>of</strong>essional. Ray is currently<br />

<strong>the</strong> director <strong>of</strong> healthcare architecture for Clark Nexsen Architecture & Engineering,<br />

<strong>of</strong> Norfolk, Virginia. In 2009, he served as <strong>the</strong> national President <strong>of</strong> <strong>the</strong> American<br />

Institute <strong>of</strong> Architects - <strong>Academy</strong> <strong>of</strong> Architecture <strong>of</strong> <strong>Health</strong>, <strong>and</strong> for <strong>2010</strong> he is again<br />

serving as <strong>the</strong> national president <strong>of</strong> that organization. Ray is interested in research<br />

in <strong>the</strong> modern architectural practice. He recently authored <strong>the</strong> chapter entitled<br />

“The Practitioner as Researcher” for <strong>the</strong> 2009 book from Wiley & Sons entitled<br />

Evidence-Based <strong>Design</strong> for Multiple Building Types, by Hamilton <strong>and</strong> Watkins,<br />

<strong>and</strong> co-authored with Quinn, Fisher <strong>and</strong> Hughes <strong>the</strong> chapter entitled ‘<strong>Health</strong>y<br />

<strong>Health</strong>care <strong>Design</strong>’ in <strong>the</strong> 2009 NIOSH DHHS publication entitled State <strong>of</strong> <strong>the</strong><br />

Sector: <strong>Health</strong>care <strong>and</strong> Social Assistance, Identification <strong>of</strong> Research Opportunities<br />

for <strong>the</strong> Next Decade <strong>of</strong> NORA.<br />

Mungo Smith<br />

Director, MAAP Architects<br />

With international experience as a specialist consultant in <strong>the</strong> planning <strong>and</strong> design<br />

<strong>of</strong> healthcare buildings, Mungo has 30 years <strong>of</strong> experience in medical architecture.<br />

In <strong>the</strong> 1980’s he was project architect for <strong>the</strong> pioneering Lambeth Community Care<br />

Centre in <strong>the</strong> UK with Edward Cullinan Architects. After a period in teaching, he<br />

<strong>the</strong>n moved to become head <strong>of</strong> design at <strong>the</strong> Medical Architecture Research Unit<br />

(MARU) at <strong>the</strong> Polytechnic <strong>of</strong> North London, where he was responsible for all design<br />

<strong>and</strong> research projects. In 1991, Mungo became a founding director <strong>of</strong> MAAP. As<br />

design lead within <strong>the</strong> practice his primary responsibility is to improve <strong>the</strong> quality<br />

<strong>of</strong> knowledge, design effectiveness <strong>and</strong> architectural excellence across all projects.<br />

Whilst he continues to be actively involved in research through consultancy with<br />

<strong>the</strong> Department <strong>of</strong> <strong>Health</strong>, <strong>and</strong> is a member <strong>of</strong> <strong>the</strong>ir <strong>Design</strong> Review Panel for major<br />

capital projects, he is also regularly invited to speak on <strong>the</strong> design <strong>of</strong> healthcare<br />

environments in <strong>the</strong> UK, Europe <strong>and</strong> Australasia at conferences <strong>and</strong> seminars.<br />

Recently, he has been involved in developing <strong>the</strong> reference design for <strong>the</strong> New<br />

Royal Adelaide Hospital <strong>and</strong> <strong>the</strong> latest masterplan for <strong>the</strong> Sunshine Coast University<br />

Hospital. MAAP are also in a team designing new acute mental health facilities in<br />

South Australia.<br />

Mikael Paatela<br />

Architect <strong>and</strong> Managing Director, Sweco Paatela Architects, Finl<strong>and</strong><br />

Mikael Paatela has over 30 years <strong>of</strong> experience as an architect in <strong>the</strong> design <strong>of</strong><br />

hospitals, health centres <strong>and</strong> facilities for elderly care both in Finl<strong>and</strong> <strong>and</strong> abroad.<br />

Having worked until 2007 as <strong>the</strong> chief design architect <strong>and</strong> managing director <strong>of</strong><br />

Paatela-Paatela Architects, a firm specialising in hospital <strong>and</strong> healthcare design<br />

for over 80 years, he is now <strong>the</strong> chief architect <strong>and</strong> managing director <strong>of</strong> Sweco<br />

Paatela Architects. Additionally he works as <strong>the</strong> healthcare architecture advisor<br />

for Sweco Architects Ab, a Swedish architectural firm with over 400 employees. He<br />

has designed several university hospitals, both new buildings <strong>and</strong> renovations <strong>and</strong><br />

elderly care facilities for public <strong>and</strong> private clients. He has also designed hospitals<br />

<strong>and</strong> o<strong>the</strong>r health <strong>and</strong> elderly care projects overseas in Russia, China, Turkmenistan,<br />

Armenia, Somalia, Namibia, Sri Lanka, Iraq, Vanuatu <strong>and</strong> Japan. He has also given<br />

lectures in several international seminars <strong>and</strong> worked as a consultant for <strong>the</strong><br />

Ministry <strong>of</strong> <strong>Health</strong>, China <strong>and</strong> <strong>the</strong> World <strong>Health</strong> Organization WHO.<br />

Nick Boulter BSc FIOA<br />

Senior Associate, Arup, Singapore<br />

Nick Boulter is an acoustic consultant with Arup. He joined <strong>the</strong> firm in 1987 to run<br />

<strong>the</strong> Hong Kong <strong>of</strong>fice <strong>and</strong> has since worked extensively in <strong>the</strong> UK <strong>and</strong> Europe <strong>and</strong><br />

he now leads a large team <strong>of</strong> acoustic specialists based in Singapore.<br />

He is responsible for a wide range <strong>of</strong> acoustic design projects, including many<br />

healthcare buildings, education facilities <strong>and</strong> performing arts venues. Nick has<br />

a particular interest in <strong>the</strong> acoustic conditions in healthcare premises <strong>and</strong> <strong>the</strong><br />

contribution this can make to wellbeing <strong>of</strong> patients, staff <strong>and</strong> visitors.<br />

Nick has acted as project manager <strong>and</strong> project director for many key healthcare<br />

projects in Arup. This has included a leading role in major hospital developments<br />

at Derby, Mansfield, Coventry, Walsall, Scarborough, Knowsley,Whiston, Broomfield<br />

<strong>and</strong> Walsall in <strong>the</strong> UK. These projects required an assessment <strong>of</strong> noise from traffic,<br />

rail <strong>and</strong> helicopters along with a detailed advice on partition sound insulation <strong>and</strong><br />

privacy issues, room acoustics control, services noise <strong>and</strong> vibration control for<br />

comfort <strong>and</strong> for <strong>the</strong> protection <strong>of</strong> vibration-sensitive diagnostic equipment.<br />

Nick had a leading role in reviewing <strong>the</strong> UK government guidance on Acoustics<br />

in <strong>Health</strong>care <strong>and</strong> was one <strong>of</strong> <strong>the</strong> authors <strong>of</strong> <strong>the</strong> recently issued HTM 08-01.<br />

Cliff Harvey<br />

Senior Architect, <strong>Health</strong> Capital Investment Branch, Ministry <strong>of</strong><br />

<strong>Health</strong> <strong>and</strong> Long Term Care, Ontario, Canada<br />

Cliff Harvey is an architect with a specialization in operational planning, design,<br />

<strong>and</strong> construction <strong>of</strong> health care facilities. A graduate <strong>of</strong> <strong>the</strong> University <strong>of</strong> Toronto in<br />

architecture <strong>and</strong> Ottawa’s Carleton University in architectural history, Cliff has been<br />

involved in <strong>the</strong> industry for over 20 years, <strong>of</strong> which <strong>the</strong> last seven years has been<br />

as <strong>the</strong> Architect for <strong>the</strong> Ontario Ministry <strong>of</strong> <strong>Health</strong> <strong>and</strong> Long-Term Care. In this role,<br />

Cliff has been responsible for advising <strong>the</strong> Ministry on planning <strong>and</strong> design issues,<br />

establishing planning <strong>and</strong> design guidelines <strong>and</strong> overseeing <strong>the</strong> review <strong>of</strong> over<br />

1,500 technical submissions to <strong>the</strong> Ministry. Building on his studies in architectural<br />

history, Cliff has applied research <strong>and</strong> analysis <strong>of</strong> past trends in healthcare<br />

architecture to <strong>the</strong> development <strong>of</strong> planning <strong>and</strong> design guidance documents to<br />

address healthcare issues such as: flexibility <strong>and</strong> adaptability for future service<br />

needs; issues <strong>of</strong> <strong>the</strong> ageing workforce, <strong>and</strong> <strong>the</strong> introduction <strong>of</strong> operational efficiency<br />

<strong>and</strong> effectiveness by design. Cliff is an active member <strong>of</strong> <strong>the</strong> architectural pr<strong>of</strong>ession<br />

<strong>and</strong> healthcare design industry, as a past Ontario Association <strong>of</strong> Architects (OAA)<br />

councillor for <strong>the</strong> City <strong>of</strong> Toronto <strong>and</strong> past chair <strong>of</strong> <strong>the</strong> Toronto Society <strong>of</strong> Architects<br />

(TSA). He is a member <strong>of</strong> <strong>the</strong> Royal Architectural Institute <strong>of</strong> Canada (RAIC), an<br />

Tye Farrow is a senior partner at Farrow Partnership Architects. His work is viewed<br />

internationally as setting a new st<strong>and</strong>ard in healthcare design. Farrow received <strong>the</strong><br />

first annual Architect Award from <strong>the</strong> <strong>International</strong> <strong>Academy</strong> for <strong>Design</strong> <strong>and</strong> <strong>Health</strong><br />

for making “a significant contribution to health <strong>and</strong> humanity”. He also received<br />

<strong>the</strong> <strong>Health</strong>care Facilities Symposium Individual Architect Award in <strong>the</strong> US <strong>and</strong> <strong>the</strong><br />

Best <strong>International</strong> <strong>Design</strong> award from <strong>the</strong> Commission for Architecture <strong>and</strong> <strong>the</strong> Built<br />
