isk for fistula. The data gathered <strong>in</strong> this study,<br />

however, show that women of all ages are at<br />

risk, <strong>in</strong>clud<strong>in</strong>g those who have had “normal”<br />

deliveries previously. For this reason, community<br />

education efforts must <strong>in</strong>form people that all<br />

women are potentially at risk for obstetric fistula.<br />

Information is essential regard<strong>in</strong>g the danger<br />

signs dur<strong>in</strong>g delivery, as well as the importance<br />

of hav<strong>in</strong>g a plan to get rapid access to a facility<br />

that can perform caesarean section. Key<br />

stakeholders <strong>in</strong> communities, as well as health<br />

care workers at peripheral facilities who can<br />

facilitate referrals, should also receive this<br />

potentially life-sav<strong>in</strong>g <strong>in</strong>formation (see<br />

Recommendation 3 below).<br />

Girls <strong>and</strong> women noted that they were rarely the<br />

s<strong>in</strong>gle decision makers <strong>in</strong> seek<strong>in</strong>g care dur<strong>in</strong>g<br />

delivery; often, other members of the family made<br />

the decision about whether, when, <strong>and</strong> where the<br />

girl or woman should seek help. As such,<br />

educational messages need to consider women’s<br />

<strong>and</strong> girls’ typical level of autonomy (or<br />

dependency). Educational efforts need to reach<br />

others who may be <strong>in</strong> a decision-mak<strong>in</strong>g role, such<br />

as husb<strong>and</strong>s, <strong>in</strong>-laws, <strong>and</strong> community <strong>and</strong><br />

peripheral health care providers.<br />

F<strong>in</strong>d<strong>in</strong>g 2: Antenatal care services, while<br />

widely available <strong>and</strong> used, are <strong>in</strong>consistent<br />

<strong>and</strong> <strong>in</strong>adequate.<br />

A majority of the women <strong>in</strong> the study attended<br />

ANC services—nearly all of them at least twice—but<br />

the services received were <strong>in</strong>consistent,<br />

<strong>in</strong>adequate, <strong>and</strong> differed greatly from M<strong>in</strong>istry of<br />

Health guidel<strong>in</strong>es. No hemoglob<strong>in</strong> tests, ur<strong>in</strong>e<br />

analysis, or blood group<strong>in</strong>g were reported. Women<br />

did report hav<strong>in</strong>g an abdom<strong>in</strong>al exam<strong>in</strong>ation <strong>and</strong><br />

hav<strong>in</strong>g their weight <strong>and</strong> height measured, <strong>and</strong><br />

some were given tablets.<br />

These ANC visits are important “missed<br />

opportunities” to provide women with <strong>in</strong>formation<br />

<strong>and</strong> services, <strong>in</strong>clud<strong>in</strong>g on pregnancy, labor, <strong>and</strong><br />

delivery. All of the women <strong>in</strong> the study who<br />

mentioned any knowledge about pregnancy <strong>and</strong><br />

labor had this knowledge because of past labor<br />

<strong>and</strong> delivery experiences. None of the women<br />

participat<strong>in</strong>g <strong>in</strong> the study had a discussion related<br />

to pregnancy or labor <strong>and</strong>/or delivery dur<strong>in</strong>g ANC<br />

visits, except that <strong>in</strong> a few cases, the woman was<br />

simply told to deliver <strong>in</strong> a facility.<br />

Recommendation 2: Providers need adequate<br />

tra<strong>in</strong><strong>in</strong>g, supplies, <strong>and</strong> equipment, as well as<br />

supportive supervision, to implement high-quality<br />

<strong>and</strong> consistent ANC services.<br />

Evidence-based <strong>and</strong> clear <strong>in</strong>formation on ANC,<br />

labor, delivery, <strong>and</strong> postpartum care are critical for<br />

the adequate tra<strong>in</strong><strong>in</strong>g of health care providers.<br />

Health care workers also require specific tra<strong>in</strong><strong>in</strong>g <strong>in</strong><br />

appropriate, effective, <strong>and</strong> respectful<br />

communication with a variety of clients. This<br />

<strong>in</strong>cludes nondiscrim<strong>in</strong>atory communication to<br />

vulnerable populations such as poor, disabled, <strong>and</strong><br />

other marg<strong>in</strong>alized girls <strong>and</strong> women.<br />
