645-6025. From November 15, 2010 to March 1, 2011, our hours of operation are 8:00 a.m. to
8:00 p.m. Eastern Time, seven days a week. Beginning March 2, 2011, calls are handled by our
automated phone system after 8:00 p.m. and on Saturdays, Sundays and holidays. TTY/TDD

Medicare Blue and Medicare Blue Plus are preferred provider organization (PPO) plans with a
Medicare contract.
