This booklet provides a list of Medicare Blue and Medicare Blue Plus’ network pharmacies.
All network pharmacies may not be listed in this directory. Pharmacies may have been added or
removed from the list after this directory was printed. The directory is current as of August 4,
2010 and listing in the directory does not guarantee the pharmacy is still in the network. Inclusion
of a pharmacy does not guarantee that the pharmacy is open or is at the same location as listed
in the directory. To get current information about Medicare Blue or Medicare Blue Plus network
pharmacies in your area, please visit our Web site at www.scbluesmedadvantage.com/medb11 or
call our Customer Service at 1-888-645-6025. From November 15, 2010 to March 1, 2011, hours
of operation are 8:00 a.m. to 8:00 p.m. Eastern Time, seven days a week. Beginning March 2, 2011,
calls are handled by our automated phone system after 8:00 p.m. and on Saturdays, Sundays and
holidays. TTY/TDD users should call 1-888-645-6023.
Beneficiaries must use network pharmacies to access their prescription drug benefit. Benefits,
formulary, pharmacy network, premium and/or copayments/coinsurance may change on January 1,
2012.
Medicare Blue and Medicare Blue Plus are Preferred Provider Organization (PPO) plans with a
Medicare contract.
