This booklet provides a list <strong>of</strong> Medicare <strong>Blue</strong> <strong>and</strong> Medicare <strong>Blue</strong> Plus’ network pharmacies. To<br />

get a complete description <strong>of</strong> your prescription coverage, including how to fill your prescriptions,<br />

please review the Evidence <strong>of</strong> Coverage <strong>and</strong> Medicare <strong>Blue</strong> or Medicare <strong>Blue</strong> Plus’ formulary.<br />

November 15, 2010 to March 1, <strong>2011</strong>, hours <strong>of</strong> operation are 8:00 a.m. to 8:00 p.m. Eastern<br />

Time, seven days a week. Beginning March 2, <strong>2011</strong>, calls are h<strong>and</strong>led by our automated phone<br />

system after 8:00 p.m. <strong>and</strong> on Saturdays, Sundays <strong>and</strong> holidays. TTY/TDD users should call<br />

We call the pharmacies on this list our “network pharmacies” because we have made<br />

arrangements with them to provide prescription drugs to Plan members. A network pharmacy<br />

is a pharmacy where beneficiaries obtain prescription drug benefits provided by Medicare <strong>Blue</strong><br />

or Medicare <strong>Blue</strong> Plus. In most cases, your prescriptions are covered under Medicare <strong>Blue</strong> or<br />

Medicare <strong>Blue</strong> Plus only if they are filled at a network pharmacy or through our mail-order<br />

pharmacy service. Once you go to one, you are not required to continue going to the same<br />

pharmacy to fill your prescription. You can go to any <strong>of</strong> our network pharmacies. We will fill<br />

prescriptions at non-network pharmacies under certain circumstances as described in your<br />

Evidence <strong>of</strong> Coverage.<br />

Preferred pharmacies are pharmacies in our network in which our Plan has negotiated lower costsharing<br />

for its plan members for covered prescription drugs than at non-preferred pharmacies.<br />

However, you will still have access to lower drug prices at non-preferred pharmacies than at out<strong>of</strong>-network<br />

pharmacies. You may go to either <strong>of</strong> these types <strong>of</strong> network pharmacies to receive<br />

your covered prescription drugs. Other than a higher cost share, there are no additional restrictions<br />

imposed on a member who uses a non-preferred pharmacy.<br />

All pharmacies listed in this directory support e-prescribing <strong>and</strong> <strong>of</strong>fer extended supply <strong>of</strong><br />

medications.<br />

The pharmacies in this directory are grouped by type (chain, retail, mail order, home infusion,<br />

long-term-care facility, <strong>and</strong> Indian Health Service/Tribal/Urban Indian Health Program), then by<br />

county. The counties are listed alphabetically under each pharmacy type <strong>and</strong> the pharmacies are<br />

listed alphabetically under each county, with their addresses <strong>and</strong> telephone numbers.<br />
