This directory provides a list <strong>of</strong> Medicare <strong>Blue</strong> <strong>and</strong> Medicare <strong>Blue</strong> Plus’ vision-care Plan<br />

providers. To get detailed information about your vision-care coverage, please see your Evidence<br />

<strong>of</strong> Coverage.<br />

The “Plan providers” listed in this directory have agreed to provide you with your vision-care<br />

coverage. You may go to any <strong>of</strong> our vision-care providers listed in this directory. If you have<br />

been going to one Plan provider, you are not required to continue going to that same provider. In<br />

some cases, you may get covered services from non-plan providers.<br />

Your vision-care benefits are available to you through our partnership with EyeMed Vision Care,<br />

an independent company that we have chosen to <strong>of</strong>fer our vision-care benefits, which has 567<br />

local providers — including ophthalmologists, optometrists <strong>and</strong> opticians — ready to serve you.<br />

To receive your benefits, please always show your Medicare <strong>Blue</strong> or Medicare <strong>Blue</strong> Plus ID card<br />

when you visit a provider for services.<br />

If you have questions about Medicare <strong>Blue</strong> or Medicare <strong>Blue</strong> Plus vision-care benefits, please<br />

call our Customer Service department at 1-888-645-6025. From November 15, 2010 to March<br />

1, <strong>2011</strong>, our hours <strong>of</strong> operation are 8:00 a.m. to 8:00 p.m. Eastern Time, seven days a week.<br />

Beginning March 2, <strong>2011</strong>, calls are h<strong>and</strong>led by our automated phone system after 8:00 p.m. <strong>and</strong><br />

on Saturdays, Sundays <strong>and</strong> holidays. TTY/TDD users should call 1-888-645-6023. You can also<br />

visit our Web site at www.scmedadvantage.com/medb11.<br />

Our service area is the state <strong>of</strong> South Carolina.<br />

How do you find Medicare <strong>Blue</strong> <strong>and</strong> Medicare <strong>Blue</strong> Plus providers in<br />

your area<br />

This directory lists EyeMed Vision Care’s providers in the state <strong>of</strong> South Carolina. To select<br />

a provider in this directory, find the county you live in or a county that is convenient for you.<br />

Under the counties are cities; under the cities are the vision-care providers, with their addresses<br />

<strong>and</strong> telephone numbers. The counties, cities <strong>and</strong> providers are arranged alphabetically.<br />

If you choose an out-<strong>of</strong>-network vision-care provider, you must pay for all services at the point<br />

<strong>of</strong> care <strong>and</strong> ask the provider for an itemized receipt. Fill out an out-<strong>of</strong>-network vision claim form<br />

<strong>and</strong> submit it, with paid receipt, to EyeMed for processing. Out-<strong>of</strong>-network reimbursement is<br />

sent directly to you <strong>and</strong> the payment will include an Explanation <strong>of</strong> Benefits (EOB). To obtain an<br />

Out-<strong>of</strong>-Network Claim Form, visit www.eyemedvisioncare.com to download <strong>and</strong> print the form,<br />

or call EyeMed’s Customer Service department at 1-866-723-0513 (TTY/TDD: 1-866-308-5375)<br />
