How to complete Community End of Placement Feedback Forms electronically: a guide for GP tutors
Up to now, the End of Placement Assessment Forms for Year 3 & 4 students have been completed on
paper and then sent to the relevant administrator by fax or post. This form will now be made available
electronically on the bespoke Medlea system, which will have the following advantages:
• It will provide a permanent record for both the tutor and the student of their Community
placement feedback.
• It will be easier for the tutor to complete: a link taking the tutor directly to the form will be emailed
to the tutor towards the end of the placement.
• There will be no risk of the form being misplaced or lost in the post.
• Comments will be easier to read and understand when typed.
• Already assessments for Year 5 and SSC placements are completed on Medlea, which has facilitated
the assessment process for all involved.
How do I complete the form
When you’re ready to complete the form, simply click on the link sent to you by the base hospital
administrator. The email you receive will look like this:

Body: Dear Dr ****,

What happens if I accidentally delete or lose the email with the link
Simply contact your base hospital administrator to request a new link. Alternatively, you can log in to
Medlea yourself and access the form by following the instructions below:
1. Go to www.medlea.manchester.ac.uk
2. If you have used Medlea before, log in as usual.
3. If you are new to Medlea, click the ‘Sign up’ link.

4. You will then be emailed a password enabling you to log in.
5. Once you have logged in, any outstanding forms will appear under the To Do list on your home
page
