<strong>in</strong> <strong>Breast</strong> <strong>Cancer</strong> <strong>Research</strong><br />

Individuals from underrepresented racial and ethnic groups as well as <strong>in</strong>dividuals<br />

with disabilities are always encouraged to apply for support.<br />

<strong>Research</strong> application must be relevant to breast-cancer research.<br />

Amount: One <strong>Career</strong> <strong>Award</strong> will be awarded up to $50,000 for one year.<br />

Required documents: All documents are required to be on the PHS 398 forms unless<br />

otherwise noted. All NIH forms can be found at: http://grants1.nih.gov/grants/forms.htm<br />

1. Face page and Abstract (use SCC Face Page).This document is created as you<br />

enter the system.<br />

2. Budget (398 budget pages 4 attached) and Budget Justification (narrative)<br />

3. NIH Biosketches (<strong>in</strong>clude all sections) for any personnel relevant to the project and/or<br />

<strong>in</strong>cluded <strong>in</strong> the budget<br />

4. Other Support Page (<strong>in</strong>clud<strong>in</strong>g effort)<br />

5. <strong>Research</strong> Plan (up to 5 pages)<br />

6. Literature Cited<br />

7. Letter of Support from Mentor<br />

8. Letter(s) of Support from other relevant sources<br />

9. Additional Information Page- such as description of Human Subjects status, etc.<br />

D. Instructions<br />

A web-based onl<strong>in</strong>e system will be used. These are <strong>in</strong>ternal awards and are NOT<br />

submitted through the Grants & Contracts Office. You must apply onl<strong>in</strong>e at:<br />

For “type” of grant, choose “<strong>2013</strong> <strong>Career</strong> <strong>Award</strong> <strong>in</strong> BC”<br />

You will upload documents (listed below) as either .pdf or Word documents. Please note<br />

the face page is created <strong>in</strong> the system – please use lay language for the Abstract.<br />

Budget form page are separate documents that you can l<strong>in</strong>k to below, then upload with<br />

your narrative and other support<strong>in</strong>g documents.<br />

Please put your name (last name, first <strong>in</strong>itial) at the top or <strong>in</strong> the header of each<br />

page. Use Arial 11 po<strong>in</strong>t or larger for all text.<br />
