2012-2013 FINANCIAL AGREEMENT
This initial agreement must be completed by all parents with students attending Mount St. Mary Academy (MSM) in the
2012-2013 school year before enrollment can be completed. This form and the Enrollment Fee of $550.00 must be
returned to the school no later than February 24, 2012.

PAYMENT AND FEES:
I agree to pay the sum of ____ $6,268.80 for Participating Catholic or ____ $6,908.40 for All Others as tuition in the
amounts and on the due dates set forth in the Tuition Plan of Payment noted below and by reference made a part hereof.
I further agree to pay all other applicable fees as detailed on the Tuition and Fee Schedule. I acknowledge and agree that
the above named student may be dropped from school enrollment if the tuition fee is not paid on or before the 15 th day
following the due date. I further acknowledge and agree that the above named student may not be permitted to take
examinations nor will the grades of the above named student be released until and unless all fees, tuition, assessments
and other charges have been paid in full.
TUITION PLAN OF PAYMENT:

the following Tuition Plan of Payment, and agree to abide by the terms and conditions therein stated:

____ FACTS Annual Payment Plan. Automatic Bank Payment Plan through FACTS on July 1, 2012. Payment can be made
by credit card with a convenience fee added. Payments are not made to MSM.
____ FACTS Semi Annual Payment Plan. Automatic Bank Payment Plan through FACTS on July 1, 2012 and January 1,
2013. Payments can be made by credit card with a convenience fee added. Payments are not made to MSM.
____ FACTS 12 Month Payment Plan. Automatic Bank Payment Plan through FACTS budgeted over 12 months, July 2012
through June 2013. Payments can be made by credit card with a convenience fee added. Payments are not made to
MSM.

2012-2013 TUITION COLLECTION POLICY
The Mount St. Mary Academy Board of Directors has the responsibility, in connection with the MSM Finance Director,
for policies concerning the amount of tuition, the manner of payment and the collection of tuition to ensure the school’s
mission.
Mount St. Mary Academy (MSM) will use the services of FACTS Tuition Management Company (FACTS) for collection of
all tuition payments. Installment payments will be debited (withdrawn) electronically from either a checking or savings
account.
All families must complete, sign and return the Financial Agreement and $550 Enrollment Fee to the Mount St. Mary
Academy Admissions Office to be considered for enrollment by February 24, 2012.
LATE ENROLLMENT
Families enrolling after July 1, 2012 will be expected to fulfill their tuition obligation according to the tuition payment
policy stated above. Tuition for students enrolling on or after the first day of school will be pro-rated over the number of
school days they will be in attendance according to a formula.
LATE PAYMENT
It will be the responsibility of each family to keep the MSM Assistant Finance Director informed of any need they may
have to request changes to the method or amount of tuition payment they schedule. Delinquent accounts will be
assessed a $15 late fee when tuition payments are not received or received late.
RETURNED PAYMENT
Tuition payments returned unpaid will be assessed a $30 returned item fee. Returned items will be resubmitted by
FACTS; however, if this process is unsuccessful, it is the responsibility of the family to submit payment as soon as
possible to FACTS, as well as continue regularly scheduled payments.
NON-ADMISSION OF STUDENTS DUE TO TUITION DELINQUINCY
Families failing to pay tuition according to the agreement made with Mount St. Mary Academy or those families
unwilling to make suitable alternative payment arrangements may be informed that their daughter may not be enrolled.
All families must be current in their payment of tuition at all times. If suitable payment arrangements are not made, the
delinquent accounts will be turned over to a debt collection company.
CO-CURRICULAR AND EXTRA-CURRICULAR ACTIVITIES
AAA requires that students participating in AAA sanctioned events (All Sports, Cheer and Rockettes) must not be
delinquent on any financial arrangements made with the school for more than 60 days.
TUITION REFUNDS
Families withdrawing students prior to the first day of school will be refunded the entire amount of tuition that has been
paid for the current school year excluding the Enrollment Fee. In the event a student is withdrawn or is dismissed from
Mount St. Mary Academy, the parent or legal guardian may apply for a partial refund. Partial refunds may be approved
at the discretion of Mount St. Mary Academy and will be pro-rated if approved.
