Learning Disability and Educational Needs<br />

Initially obtain information from other staff working with the young person currently (both education staff and key<br />

worker/personal officer) and information from records including ASSET prior to the assessment. When interviewing<br />

the young person have access to a magazine and non-digital clock (wrist watch or wall clock in room) and use your<br />

observational skills<br />

• If diagnosis of learning disability already made include in the care plan.<br />

• If not diagnosed but presents with possible learning disability or educational needs discuss<br />

with education team or Mental <strong>Health</strong> team (psychologist/psychiatrist or senior nurse) if<br />

further specialist assessment required (all young people with functional impairment should be<br />

considered)<br />
