NEW APPLICATIONS FOR SECTION 8 HOUSING CHOICE VOUCHER PROGRAM –<br />
INFORMATION REQUIRED<br />
1. Applicant (person completing <strong>application</strong>):<br />
a. Must have written documentation of current address<br />
b. Documentation must not be more than 2 months old<br />
c. Local preference definition is “lives” or “works” in LCPHA jurisdiction = Lewis, Lincoln,<br />
Macon, Marion, Monroe, Montgomery, Pike, Ralls, Randolph, Shelby or Warren county<br />
d. If applicant does not “live” in Lewis, Lincoln, Macon, Marion, Monroe, Montgomery, Pike,<br />
Ralls, Randolph, Shelby or Warren county but “works” in one of the counties, a paycheck<br />
stub is required. The paycheck stub must have the following information:<br />
i. applicants name and address<br />
ii. employer name and address.<br />
2. Forms of documentation for local preference:<br />
a. Social Security award letter<br />
b. Current bank statement<br />
c. Current phone bill – cell or land line<br />
d. Pharmacy print-out<br />
e. Check stub – must be current and show employer name and address<br />
f. If using a post office box, must have a letter from post master that states the actual address<br />
for the person on the post office box.<br />
g. Cable bill<br />
h. Electric or other utility bill.<br />
i. Letter from courts/child support enforcement/social security office/lawyers.<br />
3. Non-acceptable forms of documentation for local preference:<br />
a. You may not use any documentation from Division of Family Services<br />
b. You may not use your driver’s license for proof of residency.<br />
c. Junk Mail/Personal Letters<br />
d. Handwritten Certifications<br />
ALL DOCUMENTATION MUST BE DATED.<br />
In order to claim a local preference, the documentation must be submitted with your completed<br />
<strong>application</strong> for review. Once your preference has been approved, you will will receive a letter of<br />
eligibility and your <strong>application</strong> will be listed on the waiting list with other applicants that have also<br />
claimed or qualified for a local preference. You will receive a written notification if additional<br />
documentation is needed.<br />
If you move after submitting an <strong>application</strong>, the address change must be submitted in writing one of the<br />
county offices or to the Central Housing Office by using the Find Us. When it is time to issue a <strong>voucher</strong>,<br />
the letter will be sent to address on file. If the letter is returned to this office as undeliverable, the name<br />
will be permanently removed from the waiting list.
The following information is a timeline on what happens after you make an <strong>application</strong> for the LCPHA<br />
HUD Rental Assistance Housing Choice Voucher Program:<br />
1. The applicant information is placed on the computer and a waiting list is generated by date and<br />
time order.<br />
2. The applicant will receive written notification of eligibility or ineligibility.<br />
3. When funding is available, the applicant will receive a letter indicating they are invited to an open<br />
slot meeting. At that time, if the family is eligible a <strong>voucher</strong> will be issued.<br />
4. The following documentation must be brought to the meeting when you are issued a <strong>voucher</strong>:<br />
a. Social Security cards for all household members<br />
b. Birth certificates for all household members<br />
c. Current income for all household members:<br />
i. Consecutive pay stubs for one month.<br />
ii. Child support for 90 days.<br />
iii. Current bank statements<br />
5. The above are some of the examples of income – all income must be reported.
