Reimbursement policies may be amended at <strong>BMC</strong> <strong>HealthNet</strong> <strong>Plan</strong>’s discretion. <strong>BMC</strong> <strong>HealthNet</strong> <strong>Plan</strong> will always use the<br />

most recent CPT and HCPCS coding guidelines.<br />
