This cover note forms part of the existing Indemnity Agreement between the<br />
Insured agency above <strong>and</strong> the <strong>ACT</strong> Insurance Authority, <strong>and</strong> the Summary in<br />
Schedule One of that Agreement. This Indemnity does not extend to indemnify<br />
those risks, perils, assets, or matters detailed as Exclusions in the Agreement.<br />

Subject to the terms <strong>and</strong> conditions of the Indemnity Agreement. This Cover Note is issued as a matter of<br />
information only <strong>and</strong> confers no rights upon the holder. This Note does not amend, extend or alter the<br />
coverage afforded by the Agreement.<br />
