1. Why do I need to replace the existing safety Passport<br />

to OGSP<br />

- Because PETRONAS, Shell and Titan Safety<br />

Passport will no longer be offered. When your<br />

existing safety passport expires, you are advised to<br />

attend the OGSP course and pass the assessment.<br />

2. How long is the validity of the card<br />

- The card is valid for 3 years.<br />

3. How long will I get the result and when can the<br />

safety passport be claimed<br />

- Assessment results can be obtained within 3<br />

working days and card can be claimed within<br />

5 working days.<br />

4. If failed the assessment, what do I have to do<br />

- You have to re-sit the assessment without attending<br />

the course.<br />

5. How many times am I allowed to re-sit for the<br />

assessment should I failed for the first time<br />

- You are allowed to re-sit for the assessment 3 times<br />

only and should you failed after that you would<br />

have to attend the course and the assessment again.<br />

6. How much is the cost to re-sit for the assessment<br />

- RM40.00 only.<br />

7. If I failed the assessment will the safety passport be<br />

issued<br />

- No, the safety passport will only be issued<br />

when you pass the assessment.<br />

8. Are participants covered by any insurance plan<br />

- Yes, participants are covered by Group Family<br />

Takaful ( Personal Accident ).<br />

9. What are the passing marks for the assessment<br />

- The passing marks are 24/40 and have to pass<br />

according to these modules:<br />

Module 1 – 8 questions, answer 4 questions correctly<br />

Module 2 – 8 questions ,answer 4 questions correctly<br />

Module 3 –14 questions, answer 7 questions correctly<br />

Module 4 –10 questions, answer 5 questions correctly<br />

What is it<br />

<strong>NIOSH</strong>-Oil and Gas Safety Passport (OGSP) System<br />

is a series of training programme designed for specific<br />

target groups on specific OSH subjects and for a<br />

specific industry. The system provides the appropriate<br />

training and assessment so as to ensure that the target<br />

groups acquire the appropriate and necessary safety<br />

and health knowledge required for their jobs and fulfill<br />

the legislative requirements and the companies’ policies.<br />

The series include Induction For Plant Access.<br />

Induction For Plant Access (IPA)<br />

The programme provides the essential basic training to<br />

contractors’ workers and the appropriate assessment so<br />

as to ensure that these workers have acquired the<br />

appropriate and necessary safety and health knowledge<br />

before they embark on their jobs at the workplace. It also<br />

ensures that contractors fulfill their minimum legal<br />

requirements as well as the company’s internal<br />

requirements.<br />

Objectives<br />

The OGSP - <strong>NIOSH</strong> - Oil and Gas Safety Passport<br />

implemented in Petronas, Shell and Titan plants has two<br />

specific objectives:<br />

· To change the mindset of workers toward<br />

safety-first attitude, which is to elevate safety<br />

and health at the workplace to the same level<br />

as other business functions in the industry, and<br />

to practice it at the highest standard possible.<br />

· To provide the basic training for contractor’s<br />

workers on the recognition of the hazards and<br />

risks present in the oil and gas industry, and the<br />

precautionary and control measures necessary<br />

to ensure safe and healthy working conditions<br />

and minimise accidents at the workplace.The<br />

training also provides the necessary knowledge<br />

on laws affecting safety and health at the<br />

workplace, specifically the Occupational<br />

Safety and Health Act 1994 and its<br />

Regulations.<br />

OGSP - IPA Modules<br />

The four generic modules, common to all companies in<br />

the oil and gas industry, form the core of the induction<br />

programme. Passing an assessment on each of the<br />

modules lead to the issuance of the OGSP :<br />

Module 1 : Legal Requirement on OSH<br />

By the end of this session, participants will be able to<br />

explain the applicable legislations pertaining to OSH,<br />

and reinforce the penalties for violations of the legal<br />

provision.<br />

Module 2 : Causation and Prevention of Incidents<br />

By the end of this session, participants will be able to<br />

describe the principle causes of incidents, describe the<br />

unsafe act and unsafe condition, and explain that all<br />

incidents are preventable.<br />

Module 3 : Common Hazards in the Oil and Gas<br />

Industry and Their Control Measures<br />

By the end of this session, participants will be able to<br />

indentify the common hazards in the oil and gas<br />

industry, consequences of exposure to them and<br />

describe the appropriate control measures to eliminate<br />

or reduce the risk.<br />

Module 4 : Common Safety Violations in the<br />

Plant and Lesson Learned<br />

By the end of this session, participants will observe<br />

some of the common safety violations made by many<br />

workers that may cause incidents in the oil and gas<br />

industry and describe preventive measures from<br />

incidents.<br />
