programs provide experienced

Functional Rehabilitation and
Habilitation) Lymphedema and

Macomb Hospital-Warren program
include a completely renovated unit
and patient rooms. More than 50
percent of patient rooms are private,
with placement based on patient
need and availability.

About the Admission Process
Private insurance, Medicaid, and
managed care benefits will be
verified prior to admission and preauthorization
for admission will be
obtained by hospital staff.
Medicare and other insurers have
established specific criteria for admission
to inpatient rehabilitation
programs. This criteria includes
a medical condition (beyond the
condition that resulted in the need
for rehabilitation) that necessitates
care in an inpatient rehabilitation
program, along with the ability to
participate in at least five days or
15 hours of therapy per week.
Physicians and nurses specializing
in rehabilitation will determine
whether patients meet the criteria
for admission to the program.
Henry Ford Macomb rehabilitation
experts will serve as a resource
for suggestions for alternative
placement when patients referred
do not qualify for inpatient rehabilitation.
Our programs do not
currently serve patients who are
ventilator-dependent, comatose, or
have complete spinal cord injuries.
Patients must be 18 years or older
for admission.

The Inpatient Rehabilitation
Programs at Henry Ford Macomb
Hospitals provide the medical,
nursing, therapy and support
services required to comply
with local, state and federal
regulations. In addition, our
Warren Campus program is
accredited by the Commission on
Accreditation of Rehabilitation
Facilities (CARF) for both basic
and stroke rehabilitation. Our
Clinton Township Campus program
anticipates CARF accreditation in
late 2010.
Our multidisciplinary team is
highly qualified, and includes
board certified medical directors
and medical specialists, and
therapists with advanced
clinical training in a variety of
subspecialty areas. We carefully
monitor quality data at both of our
hospitals to help ensure the best
possible outcomes. In 2009, about
80 percent of our rehabilitation
patients were discharged to their
homes, an outcome that exceeds
national benchmarks for this goal.

The rehabilitation program at
