Jade Check-up Plan 翡 翠 體 檢 計 劃 (Plan Code: MS-M / MS-F)
A health screening to detect major diseases. It includes laboratory investigation for cardiovascular disease, diabetes and
hepatitis B status as well as thyroid function for female and gout screening for male.
本 體 檢 計 劃 協 助 及 早 發 現 各 主 要 器 官 疾 病 , 化 驗 及 測 試 包 括 心 臟 血 管 功 能 、 糖 尿 病 及 乙 型 肝 炎 指 標 、 針 對 女 士 的 甲

A comprehensive plan designed for adult aged 40 and above covering all tests provided under Jade check-up plan, with
additional tests of cancer markers, treadmill stress test for male, mammogram and DEXA for female.

A classic plan suitable for different age groups to check health conditions with follow-up doctor consultation.
標 準 體 檢 計 劃 適 合 不 同 年 齡 組 別 的 人 士 , 了 解 自 己 的 健 康 狀 況 , 並 由 醫 生 解 釋 報 告 及 給 予 專 業 的 建 議 。
Executive Check-up Plan 行 政 人 員 體 檢 計 劃 (Plan Code: B16 / B17)
A comprehensive check-up plan for executive. The check-up incorporates laboratory investigations for anaemia, blood group,
kidney function, liver function, bone metabolism, gout, thyroid function, and hepatitis screening. Common diseases like
diabetes and lipid disorders are also screened. All participants undergo treadmill stress test, electrocardiogram, lung function
test and chest X-ray that present a picture of the heart and lung function. Pap smear (for ladies) and PSA (for gentlemen) will be
performed to identify specific cancer risk.
為 行 政 人 員 而 設 的 體 檢 計 劃 , 包 括 專 為 貧 血 、 血 型 、 腎 功 能 、 肝 功 能 、 骨 質 新 陳 代 謝 、 痛 風 、 甲 狀 腺 作 用 及 肝 炎 測

Supreme Check-up Plan 尊 貴 體 檢 計 劃 (Plan Code: B18 / B19)
A more comprehensive and non-invasive check-up plan for ladies and gentlemen aged 40 and above. It includes laboratory
investigations to the functions of major organs. Common diseases like diabetes and lipid disorders are also screened. Treadmill
stress test and check for cardiac marker are done to give an insight of their coronary arteries status. Pap smear and Mammogram
(for ladies) and specific tumour markers will be performed to identify cancer risk. This program also features stool H.pylori test
to detect peptic ulcer bacteria without using endoscopy or radioactive breath test.
專 為 40 歲 以 上 男 、 女 而 設 、 更 為 全 面 及 非 創 傷 性 的 體 檢 計 劃 。 計 劃 包 括 專 為 檢 查 身 體 主 要 器 官 的 化 驗 及 測 試 , 亦 會

Important Notes before the check up 體 檢 前 重 要 事 項
• No eating or drinking or taking of any drug within 8 hours before the examination. (Except plain water)
檢 查 前 八 小 時 不 可 進 食 、 餐 飲 或 服 用 藥 物 。 ( 清 水 除 外 )
• Pregnant ladies, or any members who are suffering from bleeding disorders or receiving anti-coagulant therapy are advised
to consult his/her medical doctor prior to receiving the services.
孕 婦 或 接 受 抗 血 劑 藥 物 之 人 士 進 行 驗 身 前 應 諮 詢 其 家 庭 醫 生 意 見 。
• Ladies in menstruation is not suitable for urine test.
行 經 中 的 女 士 不 宜 進 行 小 便 測 試 。
• Please bring along sportswear for “Treadmill Stress Test”. Please do not smoke or drink any alcohol and caffeine drinks
before the test.

How to Apply 參 加 辦 法
Please fill out an Application Form, which can be download from our website, and send to Blue Cross with payment either by a
crossed cheque made payable to “Blue Cross (Asia Pacific) Insurance Limited” or credit card information. Upon receiving your
application, a service coupon will be sent to you within one week. With reference to the service coupon, customers can then
make appointment with the appointed centre to have the preventive services done.
客 戶 可 於 網 上 下 載 參 加 表 格 , 填 妥 後 , 請 將 參 加 表 格 連 同 以 抬 頭 「 藍 十 字 ( 亞 太 ) 保 險 有 限 公 司 」 的 支 票 或 將 信 用 咭 資
