<strong>Annual</strong> <strong>Report</strong><br />

to the Community<br />

Summary <strong>of</strong> Accomplishments<br />

Together the administration, management teams, staff, volunteers <strong>and</strong> community agencies have worked<br />

throughout the year to create a brighter future, inspire possibilities, accomplish more <strong>and</strong> ensure that everybody wins.<br />

Agency Highlights<br />

Recreation <strong>and</strong> Special Olympics provided 475 individuals 5,084 leisure<br />

opportunities <strong>and</strong> 460 athletes 22 different athletic activities.<br />

Food Service came in 5% under budget with meal counts <strong>of</strong> 96,166 meals in the<br />

2 residential <strong>and</strong> 1 respite programs. Food Service also provided food for over<br />

22 special functions throughout the agency.<br />

Transportation logged in over 400, 000 accident free miles this year <strong>and</strong> reduced<br />

number <strong>of</strong> UIRs 50% by maintaining “CPI” certification.<br />

Operations Department completed many major projects this year including<br />

Restroom renovation at ARC, HVAC cleaning, concrete replacement, ID/Card<br />

Access systems installed. The Maintenance department processed 1,634 work<br />

orders in addition to routine preventative maintenance duties.<br />

Human resources hired <strong>and</strong> provided training to 56 new staff. These staff were<br />

chosen from over 350 applications <strong>and</strong> through 128 interviews.<br />

Their Wellness committee continues to grow by: Hosting a blood drive every 3-4<br />

months, 241 employees participating in Wellness events, Healthy Potlucks, <strong>and</strong><br />

Walk your way to Wellness.<br />

Volunteer Services celebrated many ‘Wow’ events this year. Youth Leadership<br />

<strong>Lake</strong> <strong>County</strong>, Building Bonds Day, Impact Day, United Way Day <strong>of</strong> Caring,<br />

Carnivals, Dr. Seuss Cat in the Hat Day, Book Club, Book Cart, Rocket<br />

construction <strong>and</strong> launching, Friendship Connection, Pet Therapy <strong>and</strong> Quilters.<br />

21 new volunteers were recruited in addition to the many who participated in the<br />

events listed.<br />

Outreach Events complementing the Wow events include: March DD Awareness<br />

month including presentations <strong>and</strong> a billboard, <strong>Annual</strong> <strong>Report</strong> in the News<br />

Herald, Art show at TriPoint, Family Fun Fest at Fine Arts Association <strong>and</strong><br />

Athlete recognition night at the Captains Stadium.<br />

Service <strong>and</strong> Support Highlights<br />

The SSA Department completed 113 new intake/eligibility files <strong>and</strong> completed<br />

118 eligibility redeterminations for a total <strong>of</strong> 231 eligibility assessments.<br />

Family Resource Services had 428 families enrolled at the end <strong>of</strong> the 3rd quarter.<br />

Enrollments to the IO <strong>and</strong> Level 1 Waivers have provided opportunities for more<br />

families to participate in FRS.<br />

SSA department enrolled 5 individuals onto IO Waivers. 36 onto Level 1<br />

waivers. Many <strong>of</strong> these waivers were ‘refinanced’ from Supported Living funds<br />

<strong>and</strong> recovering approximately 60% <strong>of</strong> their Adult Service cost.<br />

Adult Services Highlights<br />

WWS had a 5.5% increase in their sales from this time<br />

last year. DI employees at WWS enjoy continuity <strong>of</strong><br />

pay. 309 community outings in addition to ceramics,<br />

art, crafts <strong>and</strong> other fun projects. <strong>Annual</strong> special events<br />

continued including 3rd annual chili cook <strong>of</strong>f, donation<br />

to Ronald McDonald House, Eikfest <strong>and</strong> Selman &<br />

Associates held an outdoor carnival.<br />

VGC increased the average number <strong>of</strong> hours worked<br />

by DI employees by 43% for 2010. Even though they<br />

were busy working VGC held the 3rd annual ‘Idol,’<br />

raised money <strong>and</strong> collected donations for the disaster in<br />

Haiti, weekly trivia games <strong>and</strong> Script Ohio prior to the<br />

OSU vs. Michigan game. Habilitation groups had a 17%<br />

increase in participation in activities.<br />

CES secured 2 new job sites <strong>and</strong> a temporary job<br />

while maintaining all jobs for individuals through the<br />

economic downturn.<br />

CES implemented a successful transportation contract<br />

with <strong>Lake</strong>tran.<br />

Children Services highlights<br />

Broadmoor continues to house a variety <strong>of</strong> collaborative<br />

agencies, including the <strong>Lake</strong> <strong>County</strong> Educational<br />

Service Center’ Preschool Program, <strong>Lake</strong> <strong>County</strong><br />

General Health District Help Me Grow, Camp Sue<br />

Osborn, YMCA, Ridge Pointe Childcare, Rising Star<br />

Learning Center, New Avenues to Independence’s Swim,<br />

Gym <strong>and</strong> More Program <strong>and</strong> Summer Program <strong>and</strong> the<br />

Deepwood Foundation. Besides being an excellent,<br />

quality educational program for the LCBDD/Deepwood,<br />

these collaboration provide tremendous resources to the<br />

<strong>Lake</strong> <strong>County</strong> community.<br />

The Early Intervention Program restructured its services<br />

according to the State <strong>of</strong> Ohio Department <strong>of</strong> Developmental<br />

Disabilities emphasis on providing services in<br />

natural environments.<br />

The Preschool Program continues to utilize (IMPACT)<br />

Integrated Monitoring Process <strong>and</strong> Continuous Improvement<br />

Tool in order to monitor our preschool program’s<br />

compliance requirements <strong>and</strong> guidelines, required by<br />

the Ohio Department <strong>of</strong> Education, Early Childhood <strong>and</strong><br />

School Readiness.<br />

Broadmoor continues to stay cutting edge by enhancing<br />

technology for instructional purposes <strong>and</strong> increasing its<br />

videoconferencing for students in all program areas<br />

from Early Intervention through the high school LEEP<br />

Program.<br />

The School Age/LEEP Program implemented the second<br />

year <strong>of</strong> the UNIQUE Curriculum with is a modified<br />

curriculum aligned with the Ohio Academic Content<br />

St<strong>and</strong>ards for usse with students disabilities.<br />

ARC/JCDC Highlights<br />

With the aging <strong>and</strong> continually changing needs <strong>of</strong> the<br />

population at ARC, 8 out <strong>of</strong> 17 residents on the second<br />

floor required assistance during emergency drills. A<br />

Room Change Committee was formed to assess, plan<br />

<strong>and</strong> complete the required moves. 42 <strong>of</strong> 68 residents<br />

moved to new rooms <strong>and</strong> /or living units. The committee<br />

worked diligently to maintain the integrity <strong>of</strong> the units,<br />

keeping current roommates <strong>and</strong> friends in tact, <strong>and</strong><br />

minimizing the need for moves in the near future.<br />

ARC secured a 1 –year License <strong>and</strong> JDCD secured a<br />

2-year license. In the Medicaid survey ARC had only<br />

2 citations <strong>and</strong> JCDC had 5. This represents a decrease<br />

<strong>of</strong> 7 total from the previous year.<br />

ARC Satisfaction rate was 98% <strong>and</strong> JCDC 100%.<br />

Residential program increased community outings from<br />

1,177 to 1,404.<br />

Respite increased its efficiency in billing <strong>and</strong> generated<br />

supplemental funds through the waiver program.<br />

Respite received generous donations from the<br />

community such as TVs, Hospital bed, a new brass bed,<br />

new clothing <strong>and</strong> more. Respite served 36 individuals, 3<br />

were admitted as a crisis <strong>and</strong> all discharged to residential<br />

placements within 30 days. 8 individuals were new to<br />

respite.<br />
