Please Note: You must file a separate signed entry form for each dog. No more than one jump<br />
height in each Program is permitted per entry. If entering tournaments or tournament and test<br />
classes at different heights in the same program, submit separate entry forms. A dog may be entered<br />
in either Grand Prix or Performance Grand Prix SM qualifiers and either Steeplechase or Performance<br />
Speed Jumping at this event.<br />
Closing Date:<br />
All entries must be received by 7:00 PM on Monday, June 7, 2010 No entries may be accepted,<br />
changed, altered or withdrawn for any reason after the closing date except for handlers receiving a<br />
title after the closing date may be "advanced" to the next higher level by providing written<br />
notification to the Test Secretary by 7:00 PM on Monday, June 14, 2010 via phone (518-732-2180),<br />

Registration of Dogs:<br />
All entries must include a dog’s <strong>USDAA</strong> registration number. To receive a registration number<br />
immediately and a height card prior to the show, register online at www.usdaa.com and click on<br />
General Information, Dog Registration. Registration may also be made concurrently with your entry<br />
on the form included in this test schedule.<br />
Rules and Regulations:<br />
Rules and regulations promulgated by the United States Dog <strong>Agility</strong> Association, Inc. are to be<br />
strictly applied at this event. Competitors are expected to be familiar with the rules and regulations.<br />
Copies of the rules may be viewed online at www.usdaa.com. Please note that if you have<br />
purchased access to the Subscriber Services area on the <strong>USDAA</strong> website, you may print a complete<br />
copy of the regulations as included in the Subscriber Services Forms & Documents Library at no<br />
charge.<br />
Video Taping and Broadcasting:<br />
Competitors through entry at this event waive any and all rights relative to video broadcast of this<br />
event. Competitors shall have the right to videotape portions of this event for their personal use only.<br />
No portion of this event may be videotaped for commercial purposes. All video and broadcast<br />
rights for this event are the exclusive property of the United States Dog <strong>Agility</strong> Association, Inc.<br />
Portions of this event may be taped for television.
TOURNAMENT DESCRIPTIONS: (for complete rules go to www.usdaa.com)<br />
2010 Grand Prix of Dog <strong>Agility</strong> ® World Championships and Performance Grand Prix SM<br />
Championships Local Qualifier Class Description:<br />
These classes are open to all competitors regardless of titles earned or skill level - a single-round<br />
tournament featuring the sport in its basic form. A dog may not be entered in both Grand Prix and<br />
Performance Grand Prix at this event.<br />
Qualification and Advancement for GP/PGP Local Qualifiers: Competitors that earn zero (0)<br />
faults shall earn a qualifying score and become eligible to enter the preliminary round of a Regional<br />
Championship. In addition, the first place qualifying competitor in each height class shall earn a<br />
“bye” voucher, permitting them to directly advance to the final round of a (Type 1) Regional<br />
Championship; a competitor utilizing a “bye” voucher may elect to run in the preliminary round<br />
without affecting their qualification for the final round. A competitor may earn more than one<br />
voucher, and the voucher is non-transferable, can be used only once and must be redeemed with<br />
entry to a (Type 1) Regional Championship along with fees due for the class. See complete rules &<br />
regulations for Regional Championships, which are hereby incorporated by reference. The rules<br />
may be found on the <strong>USDAA</strong> web site-General Information-Forms and Documents Library, or may<br />
be requested in writing by US mail from the <strong>USDAA</strong> office. A competitor must have two<br />
qualifications from local qualifiers, one of which may be from the first round of a Regional<br />
Championship, in order to qualify for entry to the National/World Championship quarterfinals;<br />
however, where a competitor may choose to compete in both Grand Prix and Performance Grand<br />
Prix qualifiers during the current tournament season, the competitor is only eligible to compete in<br />
the championships of the tournament in which they have earned the most qualifications. Further,<br />
2010 tournament rules stipulate that a competitor must compete all in Championship or all in<br />
Performance tournaments at a Regional and National/World Championship events.<br />
SPECIAL NOTES: When a local qualifier is held in conjunction with an agility test where all<br />
levels of titling are NOT being offered, ‘bye’ vouchers will not be awarded (e.g. Starters/Advanced<br />
only, Advanced/Masters only, Masters only – Tournament Only shows do not fall into this<br />
category). Competitors at a Regional Championships with a qualifying score in the top 50% in<br />
Round 2 in each height class shall earn a bye into the semifinals at the Cynosport World Games. If at<br />
a regional championship a bye is earned by a competitor/dog team who has already earned a bye in<br />
that jump height, the bye is forfeited.<br />
2010 DOG AGILITY STEEPLECHASE ® and PERFORMANCE SPEED JUMPING SM LOCAL QUALIFIER:<br />
These classes are two-round tournaments with emphasis on speed. Each round will be conducted on<br />
a jumpers-style course that shall include at least two spread hurdles (in Steeplechase only), a long<br />
jump and an A-frame and a minimum of 10 weave poles. Either the A-frame or weave poles shall be<br />
required to be performed twice. Top competitors as set forth below shall advance to Round 2 to<br />
compete for prize money and Tournament Top Ten points. Qualifiers and placement winners<br />
(minimum of four, there is no maximum) in the height class are eligible to advance to<br />
Round 2 at a local qualifying event in order to compete for Top Ten points and prize money,<br />
provided they have finished the round with a score. Non-qualifying competitors running in Round 2<br />
shall not be credited with a qualifying score regardless of their final standing and at the Regional<br />
level are not eligible to earn a bye based upon their score in Round 2. A finalist slot vacated by a<br />
competitor who has earned the right to compete in the final round shall be forfeited and no<br />
substitutions are allowed. Qualifiers as defined herein earn the right to enter the respective<br />
Championships to be held in the autumn of 2010. The tournaments are open to all competitors<br />
regardless of titles earned. A dog may not be entered in both Steeplechase ® and Performance Speed<br />
Jumping SM at this event, The tournaments shall be conducted pursuant to <strong>USDAA</strong> Championship<br />
Program Starters/PI level rules, except that scoring in each round shall be on a time plus faults basis<br />
with a five-point penalty added to a competitor's performance time for each occurrence of a fallen<br />
hurdle or missed contact; running the wrong course shall be scored as an elimination as defined<br />
under Masters rules.<br />
Qualification shall be determined based upon the Round 1 scores of the top three dogs in each jump<br />
height provided there are at least 5 scores each in the 12” and 16” height classes or 7 scores each in<br />
the 22” and 26” height classes. The top three scores shall be averaged and multiplied by a factor of<br />
1.25 to determine the maximum qualifying score for each jump height. Only competitors with scores<br />
equal to or less than the maximum qualifying score in round 1 qualify. For purposes of qualification<br />
calculations, dogs scoring an elimination shall be included in the count of competitors “actually<br />
competing” in the qualifying score computations.<br />

CLASS DESCRIPTIONS<br />
Two certification programs will be offered at this event - Championship & Performance. Competitors may<br />
enter both Championship and Performance Program classes for which they are eligible;<br />
however, under NO circumstance may a dog be entered in the same Championship and<br />
Performance class at this agility test (ie. standard at any level in both programs). Classes include<br />
standard agility, gamblers, jumpers, pairs relay and snooker agility as set forth in Chapters 3-7 of<br />
<strong>USDAA</strong> Rules and Regulations, each of which are conducted at three competitive levels.<br />
CHAMPIONSHIP PROGRAM CLASSES<br />
These classes are held at the Starters, Advanced and Masters levels. See Chapters 5, 4 and 3<br />
respectively of the <strong>USDAA</strong> Rules & Regulations for details.<br />
PERFORMANCE PROGRAM CLASSES<br />
Generally, Performance Level I, Level II and Level III are run on courses used for Starters, Advanced<br />
and Masters classes in the Championship Program, with the following exceptions: the A-frame is set at<br />
5’6”, jump heights are one height class lower as set forth in Notice to Competitors, no spread hurdles are<br />
utilized and time standards in the standard, jumpers classes are extended by 3 seconds beyond that<br />
utilized in the Championship Program. See Chapter 7 of the <strong>USDAA</strong> Rules & Regulations for details.<br />
NONSTANDARD CLASSES<br />
The following classes shall be scored pursuant to Chapter 6 & 7 of <strong>USDAA</strong> Rules and Regulations:<br />
•Gamblers: The competition consists of a course with no pre-determined route and each obstacle is<br />
assigned a point value. In the 'opening' period the handler seeks to accumulate points by performing<br />
the obstacles of his/her choice in the time allotted. At the conclusion of the 'opening' the handler<br />
attempts the 'gamble', a sequence designated by the judge to be handled from a distance, for<br />
additional points.<br />
•Jumpers: This class consists of a course of jumps and may include a tire and tunnels.<br />
•Pairs Relay: This class consists of a standard course with one dog and handler running a portion of<br />
the course, followed by a baton exchange to a second handler and dog who run the second portion of<br />
the course. Partners must be from the same level and height division, mini (Ch 12/16, Perf 8/12) or<br />
open (Ch 22/26 or Perf 16/22). In the event one dog is entered in a program/level, un-entered dogs<br />
or dogs from another program/level may NOT be recruited to form a qualifying pair.<br />
•Snooker: This class has an 'opening' sequence of point collection similar to the Gamblers, followed<br />
by a 'closing' sequence of a set pattern. It roughly parallels the billiards game of ‘Snooker’,<br />
requiring the dog to successfully complete a 'red' jump prior to attempting an obstacle designated as<br />
any other color in the 'opening' sequence.<br />
PRIZES AND AWARDS (must be claimed on the day they are awarded- none will be mailed)<br />
Grand Prix of Dog <strong>Agility</strong> ® and Performance Grand Prix SM Local Qualifying Events<br />
Rosettes in each height class will be awarded to first through third place. First place qualifying<br />
winners in each height class shall be awarded a ‘bye voucher’, permitting them to advance to the<br />
final round of a regional championship. A double overlay maroon and white ribbon shall be awarded<br />
to all Grand Prix qualifiers; maroon and pink to all Performance qualifiers.<br />
Dog <strong>Agility</strong> Steeplechase ® and Performance Speed Jumping SM Local Qualifiers:<br />
Prize money shall be computed based upon 30% of the total entry fees to be allocated in accordance<br />
with the prize allocation tables below. Allocable amounts to finalist positions that are not filled for<br />
any reason or allocable prize amounts less than $3.00 shall not be paid.<br />

All <strong>Agility</strong> Test Classes<br />
In all individual classes first through fourth placement rosettes will be awarded in each height class,<br />
except in pairs relay classes, which will be awarded in mini and open divisions. Maroon ribbons will<br />
be awarded to all qualifying scores.<br />
Titling rosettes will be awarded to those completing specific titles at the trial.
NOTICE TO COMPETITORS<br />
Competitors through submission of entry acknowledge that they are knowledgeable of <strong>USDAA</strong>’s<br />
rules and regulations including, but not limited to, the following rules regarding entry:<br />
•This test is open to all dogs at least 18 months of age, including non-purebred and purebred, except<br />
bitches in season, dogs suffering from any deformity, injury or illness that may affect the dog’s ability<br />
to perform safely, or dogs exhibiting signs of aggression.<br />
•No leads, collars, food, toys, or other aids or devices shall be permitted on the course at any time.<br />
• Safety shall be of foremost consideration at all times. Handlers through entry at this event accept full<br />
responsibility for themselves and the actions of their dogs and/or children.<br />

•<strong>Agility</strong> obstacles shall meet the requirements of <strong>USDAA</strong> rules and regulations.<br />
•The organizing committee may refuse any entry of any competitor or dog for just cause should the<br />
committee view that participation by competitor or dog would be prejudicial to the sport. However,<br />
any committee doing so must notify the competitor in writing (with a copy to the <strong>USDAA</strong> office) in<br />
anticipation of or upon receipt of entry. The notice must include the reason for refusing the entry and<br />
a brief explanation of the basis for their decision. The burden of responsibility of appropriateness of<br />
such action rests solely with the host group and its Organizing Committee.<br />
•Unsportsmanlike conduct exhibited towards others and/or dogs will not be tolerated.<br />
•No entry fee will be refunded if the Test cannot open or be completed by reason of riots, civil<br />
disturbances, fire, weather, Act of God, public emergency, act of a public enemy, strike or any other<br />
cause beyond the control of the organizers or in the event a dog and/or handler are dismissed from<br />
competition, regardless of reason for such dismissal.<br />
•Returned checks do not constitute a valid entry. There will be a $25 service charge for returned checks<br />
and $10 fee for foreign checks not drawn on a U.S. based bank or via Traveler's Cheques or<br />
International Money Orders. All funds are payable in U.S. dollars. Payment of entry fees and service<br />
charges shall be made within 30 days of postmark of notice of returned check. Any fees not received<br />
within 30 days of notice result in cancellation of event results for all classes for the registration<br />
number(s) of delinquent entry/entries. No reinstatement of results is possible, and all awards must be<br />
returned within 10 days of notification, or the dog's registration may be suspended pending restitution.<br />
Any unpaid fees or shortfalls must be paid prior to the start of the event or entry is cancelled without<br />
recourse by owner/handler.<br />
•All telephone calls will be returned collect.<br />
•Separate confirmation letters will not be sent for advancements in level.<br />
•All dogs must be kept on leash while on the show grounds except when competing in the ring. No dogs<br />
may be left unattended except when properly kenneled in designated areas.<br />
•This event will be held outdoors on a dirt/grass surface. Bring your own chair and shade.<br />
•Fault limits may be applied in any classes for which faults are assessed. The need for such limits shall<br />
be announced during the judge’s briefing.<br />
•Entries “For Exhibition Only” (F.E.O.) shall not be allowed except if by the judge’s measurement<br />
it is required to jump a higher height for which it is unprepared. No qualifying scores or awards<br />
may be earned and no change in program in check-in is permitted. Dogs that measure up at this<br />
event will run first in the running order at their higher height.<br />
•Mismatched or incomplete pairs entries shall be accepted as draw entries only.<br />
•No corporate logos other than those of <strong>USDAA</strong> ® , its sponsors, or <strong>USDAA</strong> ® member groups are<br />
permitted in the ring at any time.<br />
•Competitor setup after 5:00 PM on Thursday, June 17. Before 5:00 PM, your help with ring setup<br />
is welcome. Setup must be confined to designated areas only.<br />
•All dogs must be measured unless the dog is jumping 26”Ch/22” Perf, a copy of the permanent<br />
card was sent with entry, the handler presents a permanent registration card, temporary card<br />
with 3 signatures awaiting a CMJ or check-in letter issued by <strong>USDAA</strong>/ print out of dog info<br />
page from subscriber account indicating the dog is permanently carded. Only those who send a<br />
copy of their permanent (yellow) registration cards for each dog may be eligible for an expedited<br />
check-in if it is so indicated on their confirmation letters.<br />
•Check-in and Measurement will only be conducted once a day between 8:00 -9:00 AM on Friday<br />
and 7:00 AM - 7:45 AM on Saturday and Sunday. Dogs must be checked-in and measured on the<br />
first day they are entered prior to competing, and will not be measured on subsequent days.<br />

Cairo to Route 145. Follow for 4.5 miles, then turn left onto Sunside Road.<br />
From the Taconic Parkway- Cross the Rip Van Winkle Bridge, then follow Route 23 West to<br />
Route 145 and proceed as above.<br />
From the Massachusetts Turnpike- Take the New York State Thruway South to Exit 21<br />
and proceed as above.<br />

Your support to keep the trial running smoothly is needed. Working in and around the ring is a great<br />
way to increase your knowledge and understanding of the sport, and it gives you a wonderful sense<br />
of fellowship. If you, your guest, or significant other would be willing to work at the trial, we'd be<br />

PLEASE OBSERVE THE FOLLOWING RULES TO MAINTAIN GOOD RELATIONSHIPS<br />

1. Always clean up after your dog and dispose of the waste in an appropriate receptacle.<br />
2. Never leave your dog unattended in a room (even crated).<br />
3. If you allow your dog on furniture, bring protective coverings as a barrier to any hair,<br />
dirt, mud, or moisture.<br />
4. Do not allow dog(s) to sleep in bed.<br />
5. If you groom, do it outside and pick up the hair.<br />
6. Report any and all damage (including stains) to your host and arrange to compensate<br />
for repairs, cleaning, etc.<br />
7. Keep dogs on leash when outside.<br />

GENERAL AGREEMENT<br />
I (we) agree that Y <strong>Agility</strong> has the right to refuse this entry for cause, which the organizing committee alone shall deem to be<br />
sufficient. In consideration of the acceptance of this entry, of the holding of the event and of the opportunity to have the dog<br />
participate and/or be judged, I (we) agree to hold the organizing committee, Y <strong>Agility</strong>, United States Dog <strong>Agility</strong><br />
Association, Inc., including their members, officers, directors, agents and employees, sponsors of the event, and owners of the<br />
premises upon which the event is held and their employees harmless from any claim for loss or injury that may be alleged to<br />
have been caused directly or indirectly to any person or thing by the act of this dog while in or upon the event premises or<br />
grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim. I (we)<br />
further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, death or<br />
otherwise, and from any claim for damage or injury to the dog, whether such loss, disappearance, appearance, theft, damage<br />
or injury, be cause or alleged to be caused by the negligence of the parties aforementioned, or by the negligence of any other<br />
person, or any other cause or causes. I (we) hereby assume the sole responsibility for and agree to indemnify and save the<br />
aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by<br />
law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting<br />
therefrom, sustained by any person or persons, including myself (ourselves) or on account of damage to property arising out<br />

United States Dog <strong>Agility</strong> Association, Inc. and the provisions for entry as set forth in the <strong>Agility</strong> Test Schedule of which this<br />
entry form is a part. I (we) acknowledge and represent that the information supplied by me (us) herein is correct to the best of<br />
my (our) knowledge; if any portion of this information is found to be invalid or insufficient to establish the validity of my<br />
(our) entry, such finding shall render my (our) entry invalid in its entirety, for which there shall be no refund of fees paid.<br />
Further, I (we) represent that I (we) am the owner of the dog listed hereon, or that I (we) have full authority as agent for dog's<br />

persona, or other indica of identity for broadcast, telecast, cablecast, transmission or distribution in any format or media now<br />
known or hereafter to become known. I (we) hereby release <strong>USDAA</strong>, its representatives, agents and assigns from any claim<br />
or cause of action for invasion of the rights of privacy, right of personality, or any similar right.<br />

Junior handlers (those 18 years of age and younger) and their dogs must be separately registered for<br />

assigned for use in entering UJHP events. A dog competing in both programs (regular classes and UJHP) must<br />
