P R E S I D E N T ’ S M E S S A G E<br />

This has been an exciting and<br />

enlightening year for me. My<br />

advice to you is to get involved.<br />

We have had an exciting<br />

year so far. Several<br />

<strong>of</strong> your academy <strong>of</strong>ficers<br />

and Sonora went<br />

to the Southeast Forum<br />

and learned all kinds <strong>of</strong><br />

information about<br />

other states and problems<br />

that they are having.<br />

The one thing that<br />

I learned is that I am<br />

glad that I live in <strong>Louisiana</strong>.<br />

It is incredible<br />

to hear how hard it is<br />

for other states to get<br />

malpractice insurance<br />

and with no cap getting<br />

sued for incredibly high<br />

settlements. We also<br />

learned that other<br />

states are having CME<br />

on cruise ships and<br />

things <strong>of</strong> this nature<br />

and it will be something<br />

that we look into<br />

for the future.<br />

In September, we had a<br />

meeting at Beaufort<br />

Plantation and started<br />

getting ready for the<br />

Winter Conference.<br />

Please join us for some<br />

fun on the slopes. The<br />

operations committee<br />

met and approved the<br />

budget and gave<br />

Sonora a well deserving<br />

raise. The legislative<br />

committee met and had<br />

several areas <strong>of</strong> concern.<br />

One was lowering<br />

the malpractice cap to<br />

250,000 dollars. I really<br />

don't believe that will<br />

happen, but it would<br />

not hurt for you to<br />

write your legislators<br />

and tell them what a<br />

great idea this would<br />

be.<br />

In October, several <strong>of</strong><br />

us went to San Diego.<br />

The most exciting thing<br />

that happened there<br />

was that one <strong>of</strong> our<br />

own, Dr. Michael Fleming<br />

was elected President-elect<br />

<strong>of</strong> the American<br />

<strong>Academy</strong> <strong>of</strong> F<strong>amily</strong><br />

<strong>Physicians</strong> and will be<br />

the President next year.<br />

We congratulate Mike<br />

on a job well done. It<br />

was fun and exciting to<br />

be part <strong>of</strong> that.<br />

Things are going well<br />

with Sonora as our executive<br />

vice president.<br />

My compliments to her<br />

and her staff. If you<br />

have any needs, just let<br />

her know.<br />

Dr. Zoorob is in charge<br />

<strong>of</strong> research for the<br />

<strong>Academy</strong>, so if any <strong>of</strong><br />

you have interest in<br />

that, please let him<br />

Meningococcal Meningitis:<br />

How Can We Help Prevent It<br />

Every winter and spring we hear <strong>of</strong><br />

an epidemic <strong>of</strong> meningitis at one <strong>of</strong><br />

our colleges. Last year it was at Tulane.<br />

We may not have one this year<br />

in <strong>Louisiana</strong>, but an epidemic will<br />

occur somewhere on a US campus.<br />

That is why I have chosen to address<br />

this topic. F<strong>amily</strong> physicians see and<br />

deal with adolescents more than any<br />

other primary care specialist, and I<br />

am sure you get questions about this<br />

disease from parents and college students.<br />

Meningococcal disease is a lifethreatening<br />

infection, which is spread<br />

from person to person through direct<br />

exchange <strong>of</strong> respiratory secretions<br />

via kissing and other close contact.<br />

Common symptoms include: fever,<br />

headache, neck stiffness, lethargy,<br />

vomiting, and rash.<br />

In the US, 50% <strong>of</strong> cases occur in patients<br />

15 and older. In the 1990s,<br />

outbreaks increased on colleges campuses<br />

with most patients in the 15- to<br />

24-year-old age range. Risk factors<br />

for college students include freshman<br />

status, living in a dormitory, being<br />

Caucasian, smoking, and frequenting<br />

bars. The American <strong>Academy</strong> <strong>of</strong><br />

Pediatrics issued recommendations<br />

for healthcare providers and universities<br />

to educate freshman students<br />

about the increased risk <strong>of</strong> meningitis<br />

and the benefits <strong>of</strong> vaccination.<br />

Our academy recommends vaccination<br />

to patients with compliment deficiencies<br />

and asplenia. It also recommends<br />

vaccinating travelers to<br />

epidemic countries. Freshman students<br />

in college need to be counseled,<br />

especially if they live in dormitories,<br />

so they can make informed<br />

decisions about vaccination. Although<br />

the AAFP recommends that<br />

colleges take the lead in providing<br />

such education, meningitis is still a<br />

burden on f<strong>amily</strong> physicians. We<br />

must be able to respond and address<br />

patient and parent concerns.<br />

The US vaccination and prevention<br />

strategies for meningitis are as follows:<br />

1. Antimicrobial prophylaxis for<br />

close contacts with patients.<br />

2. Routine vaccination <strong>of</strong> higher<br />

risk populations, such as military<br />

recruits.<br />

3. Mass vaccination in response to<br />

community outbreaks defined as<br />

two to three cases within three<br />

months or an attack rate <strong>of</strong> 10<br />

per 100,000 individuals.<br />

4. Information for freshman students<br />

and parents about increased<br />

risk and availability <strong>of</strong><br />

the vaccine.<br />

I hope I have given you a brief summary<br />

<strong>of</strong> meningococcal disease and<br />

methods <strong>of</strong> prevention and vaccination.<br />

\It is worth mentioning that new<br />

sereotype combination conjugated<br />

vaccines are in the pipeline and have<br />

already started to being administered<br />

in Europe. They will be used in the<br />

US in the future.<br />

With the Annual Assembly and all <strong>of</strong><br />

the post-meeting activities behind us, I<br />

would like to take this opportunity to<br />

thank the Board <strong>of</strong> Directors and YOU,<br />

the members <strong>of</strong> the <strong>Louisiana</strong> <strong>Academy</strong><br />

<strong>of</strong> F<strong>amily</strong> <strong>Physicians</strong>, for giving<br />

me the opportunity to serve you as the<br />

EVP. It has been a pleasure serving you<br />

in the various capacities I’ve served in<br />

for over 6 years. I’ve had the opportunity<br />

to work in every capacity within<br />

the LAFP <strong>of</strong>fice.<br />

I am very appreciative <strong>of</strong> the support<br />

the Board <strong>of</strong> Directors, and Executive<br />

Committee members, in particular,<br />

have given me in this time <strong>of</strong> transition.<br />

Their vast experience and expertise has<br />

been very beneficial to the smooth operation<br />

<strong>of</strong> the headquarters <strong>of</strong>fice and<br />

to the success <strong>of</strong> our 2002 CME meetings.<br />

For that, I’m very grateful.<br />

I humbly accept this immense responsibility<br />

<strong>of</strong> serving as the administrative<br />

leader <strong>of</strong> this great organization. I am<br />

also very appreciative to you the members<br />

<strong>of</strong> the LAFP. After all, if it were<br />

not for you there would not be a <strong>Louisiana</strong><br />

AFP.<br />

My immediate goals for the organization<br />

are to maintain it’s fiscal health<br />

and, most importantly serve you, our<br />

membership.<br />

I, as well as the physician leadership <strong>of</strong><br />

the organization, am very concerned<br />

about the needs <strong>of</strong> the membership. It<br />

is our goal to provide services that<br />

would be beneficial to every member.<br />

However, as with any organization, our<br />

ability to meet your needs is dependant<br />

upon meeting our financial obligations.<br />

Thus, the LAFP Board is working on<br />

ways to better connect the leadership to<br />

the membership and its needs.<br />

The LAFP, as many other AAFP Chapters,<br />

is struggling with declining attendance<br />

at our CME programs. This decline<br />

in attendance has a direct bearing<br />

on the amount <strong>of</strong> financial support we<br />

are able to recruit from our supporters,<br />

thus possibly affecting our overall financial<br />

stability. Pharmaceutical companies<br />

are reluctant to support meetings<br />

with less than 100 physicians in attendance.<br />

We realize that you have many competing<br />

factors in making a choice <strong>of</strong> how<br />

to spend your CME dollars. Which is<br />

why we will make an effort to <strong>of</strong>fer a<br />

variety <strong>of</strong> CME activities on relevant<br />

topics.<br />

This is why we need your help to ensure<br />

the stability <strong>of</strong> your organization.<br />

So, You may ask, “How can I help”<br />

Well, here’s how…<br />

1. Send me your comments & suggestions<br />

on how to improve our<br />

CME Offerings (which I'll forward<br />

to the Education Committee).<br />

2. Encourage pharmaceutical representatives<br />

to support our CME<br />

meetings. The more support we<br />

get, the more we can <strong>of</strong>fer, in<br />

terms <strong>of</strong> high-quality CME with<br />

nationally renowned speakers, and<br />

exciting fun-filled social activities,<br />

etc.<br />

3. Make plans to attend at least one<br />

LAFP meeting every three years.<br />

Increased physician attendance,<br />

would encourage pharmaceutical<br />

companies to support meetings<br />

that they perceive as valuable to<br />

“their docs”.<br />

4. Send us your suggestions <strong>of</strong> speakers,<br />

locations, and other useful information.<br />

5. Let us know how we can be <strong>of</strong> assistance<br />

to you.<br />

6. Let us know how you can be <strong>of</strong><br />

assistance to us (maybe you’re a<br />

speaker with an expertise in a particular<br />

area, etc.).<br />

Plans are well underway for our 2003<br />

CME meetings. Won’t you make plans<br />

to join us in Breckenridge, March 1-4 th ,<br />

for some “fun on the slopes.” The Education<br />

Committee has worked diligently<br />

on topic selection and program<br />

design. So this meeting promises to be<br />

one you won’t want to miss! I am anxiously<br />

awaiting your registrations. You<br />

should have received a meeting brochure<br />

by this time. If not, please let me<br />

know.<br />

Also make plans to join us for our 56th<br />

Annual Assembly in Lafayette, July<br />

17-20th. There you’ll be given the opportunity<br />

to earn at least 20 hours <strong>of</strong><br />

CME credit, as well as the opportunity<br />

Sonora S. Norman<br />

to participate in the LAFP’s policymaking<br />

process during our General Assembly.<br />

I would also ask that you take a moment<br />

and/or suggestions <strong>of</strong> information<br />

you would like to see on the site.<br />

A Letter to the Board <strong>of</strong> Directors and Members and<br />

Staff <strong>of</strong> the <strong>Louisiana</strong> <strong>Academy</strong> <strong>of</strong> F<strong>amily</strong> <strong>Physicians</strong><br />

Dear colleagues,<br />

Dr. Robert Jarvik, the inventor <strong>of</strong> the Jarvik-7 artificial heart, once<br />

said, “Leaders are visionaries with a poorly developed sense <strong>of</strong> fear<br />

and no concept <strong>of</strong> the odds against them.” The history <strong>of</strong> leadership<br />

<strong>of</strong> the LAPF is extraordinary. We are the progeny <strong>of</strong> visionaries like<br />

Rafael Sanchez, Gerry Gehringer, Deke Sorkow, Jerry Keller and<br />

others who have led our state organization and become leaders on a<br />

national scale. None could have succeeded without the support <strong>of</strong><br />

the members <strong>of</strong> the Boards <strong>of</strong> Directors from around the state who<br />

represent our members, and the members themselves who have<br />

given encouragement to those leaders.<br />

I remember my first AAFP Congress <strong>of</strong> Delegates, where I watched<br />

as Gerry Gehringer was elected as President-Elect <strong>of</strong> the AAFP. I<br />

wanted to one day be like him and the others I saw that day. It is<br />

humbling to follow such leaders, the mentors that steeled me with the passion to lead our organization.<br />

Last month Sally and I were recipients <strong>of</strong> your outpouring <strong>of</strong> support. The <strong>Louisiana</strong> hospitality room in San Diego featured a<br />

restaurant theme, “Café Lagniappe”, serving up a Shreveport-developed specialty, crawfish and grits, to all comers to the Congress<br />

<strong>of</strong> Delegates. Our delegation welcomed over 700 to taste the spirit <strong>of</strong> <strong>Louisiana</strong>, and to sample our hospitality. LAFP<br />

President Cissy Picou, along with her faithful companion (is this the Cajun royalty thing, President-consort!) Brian, and a remarkable<br />

contingent <strong>of</strong> LAFP members and staff dressed in chef’s jackets and toques to proclaim that we in <strong>Louisiana</strong> <strong>of</strong>fered<br />

“Something Special in 2002” for our <strong>Academy</strong>. Even Jerry Keller donned the toque, telling AAFP Delegate Ed Martin that he<br />

wouldn’t have done it for anyone else!<br />

To all <strong>of</strong> you, those who came and enjoyed beautiful San Diego and those who stayed home and did what we do best – care for<br />

our patients – Sally, my f<strong>amily</strong> and I thank you from the bottom <strong>of</strong> our hearts.<br />

The <strong>Louisiana</strong> <strong>Academy</strong> <strong>of</strong> F<strong>amily</strong> <strong>Physicians</strong> Foundation (LAFP-Foundation) is a largely membership-driven organization.<br />

Room Deposit Policy<br />

A deposit <strong>of</strong> two nights per unit is required<br />

within 10 days from date <strong>of</strong> booking the<br />

reservation. The remaining balance is required<br />

45 days prior to arrival. Beaver Run reserves<br />

the right to cancel your reservation without<br />

additional notice if full payment is not received<br />

45 days prior to arrival.<br />

Room Cancellation Policy<br />

To cancel a reservation you must contact<br />

Beaver Run at least 45 days prior to arrival to<br />

receive a refund less $75 cancellation/<br />

handling fee. If cancellation occurs less than<br />

Accreditation<br />

This program has been reviewed and is acceptable<br />

for 16 Prescribed hours by the American<br />

Prescribed credit is accepted by the AMA as<br />

equivalent to AMA PRA Category 1 credit for<br />

the AMA <strong>Physicians</strong>’ Recognition Award.<br />

Objectives<br />

This activity is intended for f<strong>amily</strong> physicians<br />

and is appropriate for other primary care providers.<br />

It is designed to introduce f<strong>amily</strong><br />

physicians to the latest information, techniques<br />

and technology applicable to <strong>of</strong>fice-based<br />

patient care through didactic lectures and<br />

interactive discussions. Upon completion <strong>of</strong><br />

this activity, participants should have a<br />

working and applicable comprehension <strong>of</strong><br />

these topics. Specific objectives for each topic<br />

will be included in the participant syllabus.<br />

In 1987, the <strong>Louisiana</strong> <strong>Academy</strong> <strong>of</strong> F<strong>amily</strong> <strong>Physicians</strong> Foundation was established to promote and support innovation<br />

and improvement <strong>of</strong> the specialty <strong>of</strong> F<strong>amily</strong> Practice. Through our combined efforts, our 2002 Foundation accomplishments<br />

include:<br />

• Funding for student attendance at the National Conference <strong>of</strong> Students and Residents<br />

F<strong>amily</strong> Practice Residency<br />

Robin Dale, MD, PGY-II<br />

What an exciting time for us here in<br />

Alexandria! We are in the midst <strong>of</strong> recruiting<br />

season once again. Even more<br />

exciting are the newest members <strong>of</strong> our<br />

faculty, Dr. Tuncay Ertan and Dr. Brian<br />

Jobe.<br />

Dr. Ertan, pulmonologist, joined our<br />

staff in October. He originally hails from<br />

Turkey where he completed medical<br />

school. He completed his residency and<br />

fellowship in pulmonology at Tulane. He<br />

was Assistant Chief <strong>of</strong> Medicine and Director<br />

<strong>of</strong> Medical Education for the VA<br />

Hospital and Huey P. Long Medical Center<br />

from 1969-1971. He had a thriving<br />

private practice at Rapides Regional<br />

Medical Center from 1971-1996, where<br />

he began the respiratory care unit. In<br />

1996, he semi-retired to Colorado where<br />

he started another cardiopulmonary unit at<br />

Routt Hospital in Steamboat Springs. To<br />

our good fortune, he could not stay away<br />

from his love <strong>of</strong> medicine and teaching.<br />

As such, he agreed to join our faculty and<br />

has already boosted our knowledge <strong>of</strong><br />

inpatient medicine logarithmically. The<br />

residents are enjoying rounds treme n-<br />

dously and learning new skills each day.<br />

Dr. Ertan is an avid tennis and golf<br />

player. He is also a former Olympian as<br />

he played table tennis in the 1952 Olympic<br />

Games as the national champion for<br />

Turkey.<br />

Dr. Jobe is a board certified F<strong>amily</strong><br />

Practitioner. He finished his undergraduate<br />

degree at LSU in Baton Rouge with a<br />

degree in biochemistry in 1989. His completed<br />

his M.D. at LSU-Shreveport in<br />

1994 and he finished his f<strong>amily</strong> practice<br />

residency at UMC in Lafayette in 1997.<br />

He had a private practice in Eunice for 2<br />

years and then worked in several Emergency<br />

Rooms until 2001. He also has had<br />

experience in Occupational Medicine during<br />

the last year before joining our staff.<br />

His great variety and depth <strong>of</strong> experience<br />

will be greatly utilized. He has a beautiful<br />

wife and 3 handsome boys ages 5 through<br />

8. His spare time is spent reading and<br />

with his f<strong>amily</strong>. We are delighted to have<br />

these 2 new staff.<br />

We are very pleased to have third<br />

year medical students from Tulane doing<br />

clerkships with us. We hope to have many<br />

more. Many fourth year medical students<br />

have scheduled rotations with us to experience<br />

our program firsthand as we are<br />

able to accommodate most requests for<br />

clerkships. The students continue to be<br />

especially impressed with the volume <strong>of</strong><br />

procedures that we perform. Both the staff<br />

and residents are eager to teach and love<br />

to do so. Expanding the knowledge base<br />

here is easy given our vast array <strong>of</strong> pathology.<br />

Over the last few weeks, our inpatient<br />

team has cared for a patient with<br />

an atrial myxoma and another with<br />

Miller-Fisher syndrome.<br />

Our LSUFP flag football team thus<br />

far has a winning season. Our bowling<br />

team is in sixth place and out <strong>of</strong> running<br />

for the title, but they are having fun no<br />

matter what. Our last journal club, held at<br />

the home <strong>of</strong> Drs. Chris and Amy Griffin,<br />

was a fantastic discussion <strong>of</strong> losartan versus<br />

atenolol therapy in isolated systolic<br />

hypertension in the elderly. After our serious<br />

discussion, a ping-pong tournament<br />

ensued with all involved having a great<br />

time. I am very happy to say that I beat<br />

Dr. Ertan but he has revenge planned.<br />

Dr. Laura Fairbanks and Dr. Amy<br />

Griffin are heading our Tar Wars program<br />

with tremendous success. Each resident<br />

has given a presentation to local fifth<br />

grade classes in an effort to prevent tobacco<br />

use. Over 20 schools throughout<br />

the central <strong>Louisiana</strong> area have heard this<br />

message from our program this year. This<br />

anti-tobacco message is conveyed clearly<br />

and concisely in a way that fifth graders<br />

can understand. We look forward to continuing<br />

the Tar Wars in years to come.<br />

BATON ROUGE GENERAL<br />

FAMILY MEDICINE RESIDENCY<br />

Joshua Summers, PGY III<br />

The streak continues. Congratulations<br />

to our most recent graduates on their<br />

success with the F<strong>amily</strong> Practice Board<br />

Examination. Since the birth <strong>of</strong> this program,<br />

we have maintained a 100% pass<br />

rate for first time takers on the boards. We<br />

thank our faculty and community preceptors<br />

on their hard work in preparing us for<br />

future success.<br />

We are prepared to enter another successful<br />

recruiting season. Our recruiting<br />

committee is hard at work reviewing<br />

many exceptional applicants. We will begin<br />

our interview process in November,<br />

and conclude at the end <strong>of</strong> January with<br />

an unprecedented “second look,” where<br />

we will highlight our ability to host the<br />

ALSO course.<br />

We also use this time to recruit LSU<br />

undergraduates and expose them to the<br />

joys <strong>of</strong> f<strong>amily</strong> medicine. Twenty, highly<br />

motivated, pre-med students spend one<br />

evening each month with us to gain an<br />

understanding about f<strong>amily</strong> practice. The<br />

program’s highlight is “A Day in the Life<br />

<strong>of</strong> a Physician,” where students will follow<br />

resident physicians as they see patients<br />

in the <strong>of</strong>fice. We hope to someday<br />

be recruiting these bright students to our<br />

program to provide care for the citizens <strong>of</strong><br />

Baton Rouge.<br />

As fall comes upon us, we see an<br />

increase in collegiate and intramural athletics<br />

here in town. We have been afforded<br />

a tremendous opportunity in the<br />

sports medicine domain. Our program<br />

Director, Derek Anderson, was recently<br />

named one <strong>of</strong> the new LSU team physicians.<br />

As residents, we now can see LSU<br />

athletes at an evening clinic on campus.<br />

This has significantly improved our sports<br />

medicine exposure when combined with<br />

the work we do for Southern University,<br />

many local high schools, the Senior<br />

Olympics, and the <strong>Louisiana</strong> State Troopers<br />

Association.<br />

The F<strong>amily</strong> Medicine Residency<br />

does as much as we can, to aid the community<br />

we serve. Each month we provide<br />

meals for the women and children <strong>of</strong> Baton<br />

Rouge, who have fallen on hard times.<br />

With the help <strong>of</strong> our residency coordinator,<br />

Connie Rome, our <strong>of</strong>fice manager,<br />

Loretta Pouciau, and my wife, KC, we<br />

prepare and serve meals at the St. Vi n-<br />

cent’s DePaul Women and Children’s<br />

Shelter. It has been an excellent opportunity<br />

to see people turn their lives around.<br />

We have seen many women gain new<br />

employment and move into their own<br />

housing. The greatest reward is to see<br />

those you have helped turn around and<br />

help others get back on their feet.<br />

We have also made great strides to<br />

rebuild the community around us. With<br />

the help <strong>of</strong> the Mid City Redevelopment<br />

Alliance, we have helped to renovate<br />

houses and paint schools, in order to improve<br />

the lives <strong>of</strong> our patients. We have<br />

gained a reputation in the community as<br />

not only quality physicians, but also people<br />

who truly care for others.<br />

It is easy to see why a community<br />

based training program is superior to a<br />

university setting. It has been an excellent<br />

opportunity to see the private patients <strong>of</strong><br />

our community preceptors in their <strong>of</strong>fices.<br />

Patients are very receptive to having<br />

young d<strong>octor</strong>s take an interest in their<br />

care.<br />

We are also more prepared for life<br />

after residency training, as our program<br />

runs like a group practice. The F<strong>amily</strong><br />

Health Center is a freestanding facility<br />

with lab and x-ray capabilities. Along<br />

with excellent patient care, the residency<br />

provides extensive training in practice<br />

management and coding. This has certainly<br />

decreased my anxiety, as I prepare for private<br />

practice in the summer.<br />

I have thoroughly enjoyed my time<br />

training in Baton Rouge. Though initially<br />

drawn to return to my Florida roots, the<br />

relationships forged and great <strong>Louisiana</strong><br />

cooking, will likely keep me in Baton<br />

Rouge. I suppose I’ll need to form a new<br />

Gatorclub, with whom I can spend my Saturdays<br />

next fall.<br />

Happy Holidays from the LSU F<strong>amily</strong><br />

Practice Residency at Lake Charles Memorial<br />

Hospital. It has been an exciting year<br />

for everyone. The new interns are quickly<br />

coming out <strong>of</strong> their shell and have truly<br />

been remarkable and aggressive in taking<br />

care <strong>of</strong> patients and getting plenty <strong>of</strong> procedures<br />

under their belts. Our third year residents<br />

have their futures determined. Chris<br />

Achee, MD and James McNally, MD will<br />

be opening a joint practice in Jennings, LA.<br />

Dan C<strong>of</strong>fee, MD will be moving back to<br />

his home state <strong>of</strong> New York and doing ER<br />

work. Stacey Mayeaux, MD has signed<br />

with D<strong>octor</strong>s Hospital <strong>of</strong> Opelousas to join<br />

fellow alum Dr. Laura Braham. And finally,<br />

Carolyn Hutchinson, MD has tentative<br />

plans to move back to her hometown<br />

in Monroe, LA.<br />

The Second year class is excited about<br />

all the moonlighting opportunities here in<br />

SW <strong>Louisiana</strong> and we have been working<br />

hard to get all <strong>of</strong> our numbers and papers in<br />

order to venture out into this new part <strong>of</strong><br />

our lives.<br />

Again, we are also proud parents; Michael<br />

Jeansonne, Elizabeth Achee, Evan<br />

(Daughter <strong>of</strong> alum Darren Duet), and Alyson<br />

Baquet round out the new additions to<br />

our f<strong>amily</strong>, all <strong>of</strong> which were born in the<br />

past 2 months. Expecting a baby at the<br />

New Year is Michelle McNally, wife <strong>of</strong><br />

“Chief” James McNally.<br />

November 8-9 marked the Annual Fall<br />

Festival Weekend in which we entertained<br />

over 17 medical students and spouses. Friday<br />

night was the Gumbo and Bonfire on<br />

the Calcasieu River, with live music by<br />

some <strong>of</strong> the residents. I also wish to add<br />

that no wildlife were harmed in the musical<br />

fiasco, as Tuan Pham, MD sang “Living on<br />

a Prayer” by Bon Jovi (his trademark). Saturday<br />

night was the tailgate party at my<br />

house followed by a walk to the McNeese-<br />

SW Texas Football game, with tickets<br />

complimentary <strong>of</strong> our Sports Medicine faculty,<br />

Dr. Bryan Barootes. Numerous faculty<br />

were present from the departments <strong>of</strong><br />

Cardiology, OB-GYN, Medicine, and Surgery<br />

to stir the excitement <strong>of</strong> our program<br />

in the hearts <strong>of</strong> interested students. It looks<br />

like it’ll be another great year <strong>of</strong> interviewing<br />

and the applicants get better and more<br />

numerous each year. We should have six<br />

great new residents again next year!<br />

All is quiet on the western front (<strong>of</strong><br />

<strong>Louisiana</strong>), and we wish everyone a Merry<br />

Christmas and a Blessed New Year.<br />

LSUHSC-Shreveport<br />

Residency Program<br />

Kim J. Hunter, Coordinator<br />

Can you believe it is already November,<br />

our PGY-1's are nearing the 6 month<br />

mark. They are doing a great job, adapting<br />

well to seeing patients and learning a lot<br />

during their rotations. ERAS is up and running,<br />

fast and furious. We have begun interviewing<br />

and have scheduled several future<br />

interviews. This is a very exciting<br />

time, so far thirteen <strong>of</strong> our applicants are<br />

LSU students. Debbie Henry, Academic<br />

Coordinator did a great job hosting and<br />

arranging our resident fair in October. It<br />

was a great success. We had 20 F<strong>amily</strong><br />

Medicine programs to participate from<br />

Texas, Arkansas, Mississippi and <strong>Louisiana</strong>.<br />

Student participation was outstanding.<br />

We would like to welcome to our staff,<br />

full time faculty member, Patrick B. Wood,<br />

MD Assistant Pr<strong>of</strong>essor <strong>of</strong> F<strong>amily</strong> Medicine,<br />

Co-Director, Primary Care Pain Management<br />

Clinic Adjunct Assistant Pr<strong>of</strong>essor,<br />

in the Department <strong>of</strong> Anesthesiology.<br />

Dr. Wood completed his residency in F<strong>amily</strong><br />

Medicine at LSU Health Sciences Center<br />

in Shreveport following graduation<br />

from medical school from the same institution.<br />

Following his residency, he undertook<br />

a yearlong fellowship in Psychopharmacology<br />

Research in the Department <strong>of</strong> Psychiatry.<br />

His primary research focus is the<br />

Fibromyalgia Syndrome. Dr. Wood serves<br />

as coordinator for the Behavior Medicine<br />

curriculum for the F<strong>amily</strong> Medicine residency<br />

program. We would also like to<br />

welcome home Randy Richter, MD. Dr.<br />

Richter has been in Germany since July<br />

2002 serving our country as a member <strong>of</strong><br />

the Army Medical Unit. We look forwarding<br />

to him returning to work November 18,<br />

2002. Several <strong>of</strong> our faculty members have<br />

submitted packages for promotions: J. Bart<br />

Pope for Pr<strong>of</strong>essor and Rush-Kolodzey,<br />

Minao Baudoin and Jeffrey German for<br />

Associate Pr<strong>of</strong>essor. Congratulations to<br />

each <strong>of</strong> them.<br />

Congratulations goes out to Dr. Patrick<br />

Deere, 2nd year and bride-elect Kelly, announced<br />

their engagement and will tie the<br />

knot December 7, 2002. Our resident, faculty<br />

and medical students were actively<br />

involved again in the “Tar Wars” effort.<br />

Along with the support <strong>of</strong> AHEC they presented<br />

this program to over 500 fourth and<br />

fifth graders in October.<br />

Hurricanes, hurricanes, hurricanes . . .<br />

and the occasional tornado! Those should<br />

become our bywords, some <strong>of</strong> us here in<br />

Lafayette feel. While we experienced the<br />

arrival <strong>of</strong> tropical storm Isidore relatively<br />

unscathed, the arrival <strong>of</strong> Lili exactly a<br />

week later was another matter. Thank<br />

goodness she had calmed down from the<br />

anticipated level IV to a level II before<br />

making landfall. Still, her arrival was<br />

worse than many/most expected. While the<br />

serious damage was generally confined to<br />

the “other” living things we share our globe<br />

with (namely trees) the toll on them was<br />

great. At this writing 6 weeks later, the<br />

cleanup <strong>of</strong> debris in local neighborhoods<br />

seems to only now be winding down. It<br />

almost seems unusual to no longer see that<br />

material piled car-high almost continuously<br />

along the sides <strong>of</strong> many residential streets.<br />

Still, we were lucky. No one was<br />

killed. The inconvenience <strong>of</strong> the entire city<br />

being without electricity from hours to a<br />

week or so and the extra work <strong>of</strong> cleaning<br />

up seem insignificant compared to what<br />

could have been. Buildings can be repaired,<br />

and other trees can be planted.<br />

During the storm, F<strong>amily</strong> Practice<br />

Center personnel were required to report<br />

for duty. Children and/or other f<strong>amily</strong><br />

members could be brought along; although<br />

no one actually brought their children. Personnel<br />

stayed for 12 or more hours until the<br />

worst had passed. Two <strong>of</strong> our residents,<br />

Tom Price and Vanessa Thomas were on<br />

hand at the Heymann Center to provide<br />

care to medically fragile homebound patients<br />

who had been brought to that facility.<br />

Reports are they were quite busy.<br />

It was anything but an ill wind that<br />

enabled us to bring Alan Broussard, MD<br />

onto the full- time faculty on 1 September<br />

’02. Alan, a former graduate <strong>of</strong> this program,<br />

has been practicing in this area for a<br />

number <strong>of</strong> years. He was voted “Preceptor<br />

<strong>of</strong> the Year” by the residents and given the<br />

award for it at last year’s graduation ceremony.<br />

We are excited about his being here.<br />

Results were relatively recently obtained<br />

from the ACGME advising us <strong>of</strong> the<br />

continuation <strong>of</strong> our full accreditation. We<br />

are proud <strong>of</strong> that and appreciate the hard<br />

work <strong>of</strong> all in the program, hospital and<br />

accomplishment.<br />

This year’s American Board <strong>of</strong> F<strong>amily</strong><br />

Practice In-Training Examination was<br />

taken by all residents nationally on Friday,<br />

November 1 st . As always, we are eager<br />

to see those results.<br />

Recruiting season appears to be well<br />

underway. We have been attending various<br />

residency fairs and enjoying interviews<br />

with applicants. It would be a bit<br />

easier if the Electronic Residency Application<br />

System was fully functional in all<br />

<strong>of</strong> our <strong>of</strong>fices here, but we are managing,<br />

somehow.<br />

The Pennington Biomedical Research<br />

Center held an obesity workshop at UMC<br />

on September 30 th . First year FP residents<br />

from Lafayette, Alexandria and Lake<br />

Charles attended the half-day workshop,<br />

as did the new Lafayette IM residents.<br />

The workshop, “Interactive and Practical<br />

Approaches to Managing Your Overweight<br />

Patient,” is actually a production<br />

<strong>of</strong> C.O.R.E (Centers for Obesity Research<br />

and Education). C.O.R.E. encompasses<br />

eight <strong>of</strong> the leading centers involved with<br />

the medical management <strong>of</strong> overweight<br />

and obese persons in the United States.<br />

Pennington and the other nationally recognized<br />

centers have combined forces to<br />

disseminate knowledge about obesity<br />

treatment to the medical community. We<br />

appreciate and thank the Pennington people<br />

for providing this experience to our<br />

residents; and also thank Al Rees for his<br />

efforts in scheduling and facilitating the<br />

activity.<br />

Missing M.D.s<br />

Wanted: more geriatricians to treat the aging<br />

By ANDREW GOLDSTEIN<br />

Stephen Bzdok began experiencing seizures<br />

at age 65, and d<strong>octor</strong> after d<strong>octor</strong> was unable<br />

to discover the cause. A Las Vegas lounge<br />

singer for more than 20 years, Bzdok nearly<br />

became homebound. The internists and neurologists<br />

he saw never took the time — and<br />

didn’t have the expertise — to figure out what<br />

was wrong. Then an especially fierce seizure<br />

put Bzdok in a coma for four months. His<br />

f<strong>amily</strong>, assuming the worst, sold most <strong>of</strong> his<br />

possessions. One day, miraculously, Bzdok<br />

woke up. This time he consulted a geriatrician<br />

— a d<strong>octor</strong> with special training in care<br />

for the elderly — who soon discovered the<br />

problem: a heart murmur. Bzdok had a pacemaker<br />

implanted, and has been seizure free<br />

ever since. The fun-loving sexagenarian says<br />

he feels as if he were 50 again. He lives on<br />

his own, walks half a mile a day and spends<br />

weekends on a speedboat.<br />

If only the rest <strong>of</strong> the nation’s seniors were as<br />

lucky. When politicians and journalists talk<br />

about problems in elder care, they usually<br />

point to horrific abuse in nursing homes or to<br />

retirees who are forced to skip meals to pay<br />

for prescription drugs. But the most serious<br />

crisis in elder care has been largely ignored:<br />

not enough d<strong>octor</strong>s who know how<br />

to diagnose and treat aging’s special ailments.<br />

Out <strong>of</strong> 650,000 physicians in the U.S.<br />

today, just 9,000 are certified in geriatric<br />

medicine. And that number is shrinking, as<br />

many quit the poorly paid field or retire. Only<br />

3 out <strong>of</strong> 144 U.S. medical schools have a<br />

full department in geriatrics (all have a<br />

department in pediatrics), and fewer than 3%<br />

<strong>of</strong> medical students take even one course in<br />

geriatrics, according to the International Longevity<br />

Center. During the next 30 years,<br />

more than 70 million baby boomers will<br />

reach age 65. Who will care for them<br />

Medicine for the elderly is a different science<br />

from medicine for the young or the middleaged.<br />

Seniors face a range <strong>of</strong> age-related<br />

ailments — dementia, Alzheimer's, arthritis —<br />

and typically swallow more than four medications<br />

a day. A d<strong>octor</strong>’s attention to how drugs<br />

interact can mean the difference between<br />

saving a life and taking one. Many geriatricians<br />

say they devote much <strong>of</strong> their efforts to<br />

sorting out the complications caused by their<br />

patients' previous d<strong>octor</strong>s. Each year 17% <strong>of</strong><br />

elderly Americans are hospitalized after experiencing<br />

a dangerous drug interaction or<br />

taking incorrect dosages. The American<br />

Medical Association says nearly 7 million<br />

seniors a year are routinely prescribed drugs<br />

that are too risky for them.<br />

Mary Richert, 79, <strong>of</strong> Jennings, La., had been<br />

living on her own, taking trips as far away as<br />

Alaska, until she began suffering from memory<br />

and balance problems. She stopped driving<br />

and traveling and became depressed. At<br />

the urging <strong>of</strong> her children, she spent 18<br />

months trying to find a d<strong>octor</strong> who could help<br />

her. But the message from the series <strong>of</strong> internists,<br />

urologists, neuropsychologists and<br />

neurologists she saw was always the same:<br />

“Mama's problems are because Mama’s getting<br />

older.”<br />

That wasn’t the reason. Two years ago,<br />

Richert went to see Charles Cefalu, one <strong>of</strong><br />

the top geriatric specialists in the U.S. “He<br />

was different from any d<strong>octor</strong> I had been to,”<br />

says Richert. “He took his time.” Cefalu’s<br />

thorough examination helped him figure out<br />

the cause <strong>of</strong> Richert’s seeming dementia: her<br />

brain — like that <strong>of</strong> a child born with “water<br />

on the brain”--had stopped draining cerebral<br />

fluid. Cefalu sent Richert to a neurosurgeon,<br />

who placed a shunt inside her brain to act as<br />

a drain. Her health improved dramatically.<br />

Today she gardens, travels and — to the<br />

consternation <strong>of</strong> her children — is driving<br />

again.<br />

It’s an inspirational tale, yet what saved<br />

Richert is what keeps d<strong>octor</strong>s away from<br />

geriatrics: it takes time to diagnose elders’<br />

problems. A physician is reimbursed by insurers<br />

for a typical <strong>of</strong>fice visit <strong>of</strong> 15 or 20 minutes.<br />

But a geriatrician needs on average at<br />

least an hour for an initial assessment. While<br />

a general internist can see 2,000 to 3,000<br />

patients a year, a geriatrician sees fewer than<br />

500 patients, making for a smaller income.<br />

Says Joseph Ouslander, head <strong>of</strong> Emory University’s<br />

geriatric-medicine program: “I can<br />

go into my clinic and spend an hour with a<br />

very complicated 90-year-old patient, identify<br />

her 10 medical problems, sort out her dozen<br />

medications, talk to her children. And I get<br />

the same reimbursement as the dermatologist<br />

who spends 30 seconds shaving <strong>of</strong>f a<br />

mole.”<br />

A geriatrician’s income is almost entirely dependent<br />

on the amount that Medicare pays<br />

for each patient, in effect leaving the d<strong>octor</strong>’s<br />

salary up to the Federal Government. And<br />

this year Congress has cut Medicare reimbursements<br />

5.4%. <strong>Louisiana</strong> Senator John<br />

Breaux, who chairs the Senate Aging Committee,<br />

is one <strong>of</strong> the few members <strong>of</strong> Congress<br />

to call attention to the shortages in<br />

elder care, most recently at a hearing last<br />

February. Yet Breaux, a fiscal conservative,<br />

told TIME he won’t consider raising Medicare<br />

reimbursement rates. “That’s not in the cards<br />

right now,” he says. Instead, he proposes<br />

forgiving student loans to d<strong>octor</strong>s who go into<br />

geriatric medicine.<br />

A handful <strong>of</strong> medical schools have begun to<br />

boost their geriatric programs, despite the<br />

lack <strong>of</strong> student demand. (One medical resident<br />

said he never took a class in geriatrics<br />

because “old people are gross.”) Cefalu<br />

started an expansive program at <strong>Louisiana</strong><br />

State University that he hopes will act<br />

as a model for other schools. But when<br />

Greg Sachs, chief <strong>of</strong> geriatrics at the University<br />

<strong>of</strong> Chicago, launched a geriatrics fellowship<br />

in 1986, only a few young d<strong>octor</strong>s entered<br />

the two-year program, in fits and starts,<br />

and interest dried up altogether in the mid-<br />

’90s. This year Sachs has just a single fellow.<br />

When the legions <strong>of</strong> baby boomers begin to<br />

retire, many will need the kind <strong>of</strong> geriatricians<br />

who have the skills to save people like Bzdok<br />

and Richert. Jennifer Moore, 30, hopes to be<br />

such a d<strong>octor</strong>. Inspired to go into geriatrics by<br />

watching her grandmother take care <strong>of</strong> her<br />

grandfather after he suffered several strokes,<br />

Moore followed med school with a two-year<br />

fellowship in geriatrics. But now she’s having<br />

trouble finding a satisfying job. “How can I<br />

provide good care in the manner in which I<br />

was trained,” she asks, “when all I get is 10<br />

minutes with each patient” Unwilling to compromise<br />

her standards, Moore is looking to<br />

teach geriatric medicine instead. Now the<br />

question is whether she can find some students.<br />
